Do you ever personally use this technique from a standing front headlock position, and is it as effective as when you already have them snapped down?

Yes I do. As long as you can keep his head down so that you can bring his shoulder over it the technique will work. From a standing FHL, sometimes you can have more difficulty because your opponent is much more mobile. Personally, I prefer to do this against someone that is on their knees. I have countered a lot of shots with it, where I am standing but my opponent is on their knees.
