This one always gets to me. A family member of mine is an ATC and there’s no way that one operator should have been on duty. The second operator should have been called back from his break when the maintenance crew came in and started disconnecting stuff. Very sad. Should never have happened.

@@davidwoermansr This 100%. What do you expect him to do walk away from his station to go get his coworker then if an accident occurs, he would have been faulted for leaving his station. The pilots should have obeyed TCAS a system designed because of the limitations of ground-based air traffic control as determined by previous accidents. You can't expect ATC to fly the plane for you too and in the end the pilot is responsible for the safety of aircraft to suggest otherwise is appalling. Of course, you need communication between both parties but there is a difference between 100 miles away and being the person who is controlling such flight with direct eyes in the air. That accident was the pilots fault, probable lack of training, but nonetheless again being responsible for the aircraft you should know what systems are being operated and performed by the aircraft and how that will affect the environment in which you operate that is literally your job. Peter did his job to the best of his ability after having many of his tools taken away sadly, I can't say the same for that one crew who had the benefit of TCAS but lacked the ability to understand how said system operates and functions and again a system designed because of the limitations of ground-based air traffic control. To fault Peter, suggest that such aviation companies failed to learn from pervious accidents and that is called being ignorant.

At first I was happy that he found his daughter and could feel he was the first to get to her after the tragedy. But, now I think this was more traumatizing for him to witness the scene and is why they were discouraged of allowing families of the victims to participate. Maybe it is better that others find deceased and deliver the news in the fashion a police officer would in a consoling manner. Plus I think after this he needed counselling and should avoid the news and not get too involved in what the media had to say as they were looking to blame people without the investigation being completed.

His mind heart and soul left this man that day. I just hope anyone reading my comment above doesn’t think I condone what he did to the tower controller. I just found it amazing in all that tragic loss of life and airplane parts he was able to find the pearls and his daughter pretty much intact. Just a horrible situation for all involved. That's always how it is. No single thing is supposed to be able to down a liner so it takes many factors coming together. There are always multiple checks for everything so something has to missed several times by several people for a mistake to happen

I don't agree. It has nothing to do with coincidence. Thats like saying it was the children's destiny to die in by flying, and the controller Peter was destined to be murdered by one of the fathers. It was a shame that psychics weren't there to intervene. The repairs should have been scheduled for a time when there were more people working not less. But would that have really mattered.

I heard that the other traffic controller was taking a nap on the clock. He should have been in there helping Peter. Especially since they were doing work on the phones etc. Many things worked together to cause this heartbreaking tragedy 😢

Peter was not to blame. Unfortunately I know that deep down he will always blame himself, no matter what anyone says. But the fact is, this was not peters fault. He was extremely let down by a large number of other people who were supposed to be working in conjunction with him to keep everyone safe. Not one single person is to blame for this. Understand that the human being makes mistakes. Mistakes should not end in disaster. Peter was overwhelmed, had communication cut off, and was left without critical systems functioning. Any human in the same position would have been very likely to end up the same. The last procedure to enact would be that a controller MUST instruct both planes. Such as telling one to descend and the other to stay level. Peter did not purposefully cause the accident nor did he know what could have happened. Placing the blame on him is merely a scapegoat for the ineptitude of the company

@@inconvenienttruths7953 Peter was overworked and in a situation where thinking clearly is impossible, he acted on instinct. If he had an earlier warning he would have had time to think of the TCAS. Skyguide should not have put him in that position. They should not have had only one worker for two screens. Maintenance should have informed him of what exactly would be taken down, he likely would have told them to wait if they did. The phones were disconnected so he couldn't reach anyone, but he tried to lighten his workload knowing it was too much. There should have been strict guidelines for all pilots to follow TCAS, to avoid situations like this. Skyguide should have had to pay out more, they should have been closer investigated. Peter was definitely a major part of what happened, but he shouldn't have been put into that situation. He was an easy target to put the blame on. He didn't deserve to be stabbed to death.

@@inconvenienttruths7953Peter was heavily overworked. He was the ONLY controller in that tower, an unacceptable position to be in for any tower controlling an entire airspace. Maintenance disconnected the telephones and made his radar system much slower than normal. He had no way of knowing ahead of time that neither aircraft had taken evasive action that they were directed to because he was ALONE. He was managing a whole airspace on different monitors across an entire room, flipping between them with barely functioning equipment. Peter panicked due to his overwhelming situation and gave an order to the Tupolev, an order THEY decided to obey over TCAS. TCAS holds superiority over ANY controller anywhere in the world. The Russians decided to listen to the controller instead of their TCAS. He was the only controller in a tower with extremely slow radar and no communication equipment and he panicked in the sudden severity of the moment. The cause was not Peter. It was the fact that Skyguide had equipment that was undergoing maintenance, not fit for managing an entire airspace and the fact that pilots did not obey their TCAS.

This was so incredibly horrible on so many different levels. First, so many children killed. That is heart breaking right there. Second, if both planes had just listened to the automation, all of this would have been avoided - LITERALLY! Third, management was stupid to allow work on an active system needed to manage flights! They essentially rendered the air traffic manager deaf, dumb and blind. He couldn't call and he couldn't see clearly. Fourth, the other air traffic manager SAW the planes on a collision course and, even though they were out of his sector, he should have reached out to the planes once he couldn't reach the air traffic person who was managing them! He could have taken the heat later, but at least people would be alive! This was just a horrible thing. The icing on the horrible cake is the father blaming the air traffic manager solely and killing him! He shouldn't have done that. Funny that he didn't focus any blame on the coworker!

Some news reporters are worse than vultures. The issue could be laid at the feet of the contrller taking an extended break. The issue could also be laid on management, scheduling two controllers and maitenance is stupid. Communications must all have redundant backup systems. My personal opinion management is responsible. There should have been policy that all workstations should be manned at all times. No exceptions.

@@budderkupp1282 Yes, absolutely true. Management is never held responsible. I worked in mining and mine safety. We were required to recommission a mine that had been shut down for being non-profitable. The equipment was already end-of-life and several years of neglect meant the system was inoperable. The entire mine control systems, and all the wiring, was intrinsically unsafe. But the choice was to patch the system back to life or walk away. Walking away meant I would never work in the industry again (the mining company saw to that). Inevitably, a train carrying ore ran away and smashed into the active mine all, killing many people and triggering a partial collapse. Several engineers went to jail - and exactly no-one who was responsible. By the time this happened I had already left the country in search of work.

That man who killed the guy on the computer's that watches the plane's should have went to the company! Because they were sent in to turn off the phone's to the plane's he couldn't get in touch with them to tell them that they were heading right into each other he tried the best he could they should have grounded the plane's until they had those lines fixed and test them to make sure they were working properly! I find it very interesting how killing as revenge is a thing among us humans. Which benefits does it got? I mean, does it feel better? Does it give your wife and kids back? Is it better to spend life in prison mourning alone than in your own house with all the happy memories before the accident? Not being able to grief with other family and/or friends? Not being able to even visit their gravestone. And this guy Peter, wasn't even guilty!! The investigation wasn't even finished yet!! He had the odds against him and his mistake was one of many problems that night. With this logic Peters family should kill this father for being a suspect as revenge. But what do i know? I've never been in a situation like this so who am I to judge? But my logic brain would rather prefer that a person I really hate and blame for killing my family stays alive. As punishment he has to live with what he's done. And I would make sure to remind him on a regular basis. Just rub it in how much damage he has done to other human beings. If I take his life it's like I'm setting him free. If he's to weak he can take his own life. I would't care less. But this is only if the person was found guilty. Peter wasn't. I can't imagine how he felt that night and afterwords. His punishment was the knowledge of being a part of a tragic accident. His murderer set him free from guilt. May Peter and all the other victims rest in peace!

Vengeance consoles and allows victims to continue with their lives. After his revenge, Haloev married again and had children. He didn't bury himself alive like the other parents. There are two airplanes that are going to collide, and you only check with one and give them instructions while completely disregarding the other plane. Does that even make sense? He was trying to save time, so that he could go back dealing with the other screen. But not only that, you don't even wait to see if the situation is under control. I understand what led to this situation in the first place, but I also believe he handled it very poorly. He did not deserve to die tho. The blame was mainly on these corrupted organizations that prioritize money over safety.

That's how it's done. You tell one plane to climb or descend that's safer than both planes changing altitude like here. Generally someone will be where they aren't supposed to be and someone is on track so the one off course is told to deviate. If both are told to react that increases the number of potential conflicts. Had only one descended it would have been fine but both did. TCAS isn't really supposed to ever be needed. If it activates several things have already gone wrong.

At the absolute minimum he should've given descend instructions to one aircraft and climb to the other. He did not. Instead he fixated on the Russian aircraft. He also gave erroneous information to the Russian crew - telling them the DHL flight was on their right whereas it was actually on the left. This would have the crew looking in the wrong direction for traffic. When they did spot the DHL plane, on their left, it was hopelessly too late. While Peter Nielsen was only one link in a chain of criminal indifference and incompetence, he was nonetheless the person who finally doomed both aircraft and he is not without blame. How anybody could blame Peter for the collision is beyond me…he did everything humanly possible. It was more the fault of the maintenance workers who disabled the phones and radar. Should have someone to replace a co worker who needs a washroom break instead of overloading another controller.

He just neglected both planes that were about to go on a collision course, never bother to check the altitude of both planes which were clearly descending at the same time, and just went on to check on the Airbus instead. It's a stupid mistake made by purely incompetence and lack of basic common sense.

It's sadly ironic that the grief- stricken father has permanently traumatized the children of Peter Nielsen and left them full of grief. From everything I've heard on this tragedy, the ATC was placed in an impossible situation with his ability to do his job being severely restricted. He may not have been properly trained on the TCAS. Having to operate two work stations with his main radar screen not working properly and phone access down, and him not knowing the limitations that maintenance would create. Then he was handed another airplane to guide, which would have been doable if not for the maintenance work being done at the time in the control tower. The people who made this situation possible are responsible for what happened and Mr. Kolaev went from being a victim to murderer. He does not deserve any accolades. He should be in prison for life.

This was a horrible mistake there’s plenty of blame to go around. But what company allows a air traffic controller to go on such a long break and expects another air controller to watch two screens. That is impossible to give your full attention to a screen. Common practice no that’s asking for a a disaster to happen. Who allows phones to be totally disconnected and controllers don’t have the resources to call out or screens to give lag time it is critical for any company screen to work properly just because you have a slower than normal air traffic in the sky it puts all those planes at risk. The company is responsible for poor performance lagged safety procedures overworked employees. Peter I’m sure never went a minute without recalling what happened that night but it wasn’t his fault sure when he contacted one plane to descend and didn’t the other haunted him. But the truth he has to many things going on doing two positions at once watching to screens which common sense tells you that impossible to give your full attention and safety to any aircraft and faulty equipment no phones it is tragic after so many lives lost changing so many people parents forever children lives lost than he was killed and added another death. I won’t say anything about the father I believe he went totally insane after the death of his entire family. He heard so many news reports blaming the Peter and in his mind not stable took his revenge on Peter. Right absolutely not. But grief and loss and unable to cope can do change things to mind. during loss as the mother of one child killed we didn’t want anything to happen to Peter! Enough deaths. The ones that should have took responsible is the company the string of events from Peter down to the pilots the company set the stage for all the events that followed. but as we know it’s the bottom line make everyone else to blame. The mighty dollar must be protected instead of lives and knew there practices were dangerous. My all rest in peace.

I wouldn't be able to live with myself in either his or the father's shoes. This show sometimes really makes me appreciate my life, kids and everything that's important to me. My ultimate nightmare is dying in a plane crash or losing loved ones ,( especially my parents right now because they've been going to Fl every other month now,). Every time I get to the Avianca Crash I'm always reminded of the Attorneys remarks and that of the Spanish Passengers that lived talking about the word Priority rather than Emergency. There are words like Pan that means a cooking utensil in your kitchen but to a controller Pan Pan means something completely different. If they had said that they were declaring a Fuel Emergency the aircraft would still be intact and they would still be alive but may have some professional troubles after the incident. Basic English is the primary language of Aviation world wide, but there are code words that you must say in order to have "priority status" there a bunch of other aircraft in the hold too that want to get down, If you decide not to divert to Boston when you had the fuel and almost run out of fuel holding in New York , YOU must say the word EMERGENCY. That's kind of it, westerners flying into China or Japan use the word Pan Pan or Emergency and those words are immediately recognized.

Even for the Avianca flight, no sense of urgency whatsoever. Especially, the pilot in communication with the air controllers. Too much attitude and arguing in the cockpit. Baffling. I was shocked when the word emergency finally left his mouth when it was too late. I thought he would go down while assuring that all was good.

i don’t think this was anyone’s fault. I mean y’all realize the phones broken, the airplanes alerts, and the poor dude going back and fourth. It’s hard because the guy told them to descend and didn’t know that the planes alert told the other plane to descend, so i guess the lack of communication there. In this case i guess the pilot took Peter’s word over the technology (which yes wasn’t right in this case), but it’s understandable you don’t know what you’d do in the situation and they decided to trust the human. Not to mention Peter didn’t get the alert for the collision another technology thing. So i just wish the mechanic told Peter the whole thing, which the video said they didn’t. Ofc the mechanic didn’t mean for it to happen, just in future should let the controlling people know that certain alerts wouldn’t work. Imagine if it was working that day, Peter probably would’ve caught it in time. But then again, i think it showed that maybe some of the controllers might rely to heavily on the technology yk? Honestly seems like a huge misunderstanding and communication. Mechanic not telling Peter everything, DHL not telling Peter the alert told them to descend, Peter telling the other plane to descend, Peter not being able to move a different plane to a different center, and the poor dude who caught it not being able to reach Peter, or talk to pilots due to the rules.

In 2004, Peter Nielsen, the air traffic controller on duty at the time of the collision, was murdered in an apparent act of revenge by Vitaly Kaloyev. He was jailed for less than 2 years. It wasnt JUST Nielsen's fault. Its never just one thing. Kaloyev went on to have a new family and career never caring about the family of the man he murdered. Tragic all around, esp for the parents. GPWS always seems to tell them too late to pull up. The L1011 could land on auto-pilot. AVianca its another case of cultural misunderstanding. Aviation language is english. Standardized terms should be used. Lots of times they arent. Just like Gol 1407. The ATC bosses telling them to take 33 (more than was safe) was also BS

Y did Peter tell pilot to descend when he knows tcas is installed in all planes. He saw on the screen that they were near to a collision course. He should have simply told the pilot to follow tcas instructions. Apart from this I don't see any other mistake of Peter. The maintenance work and under staffing was not his problem. He was doing his duty diligently.

It’s so heartbreaking to think about the terror these people felt my hearts go out to them. I hope this never ever happens again. Thinking about all the plans he had with his family and plans for retirement with his wife maybe moving to some nice place and thinking about his kids graduating and his kids getting married. He shouldn’t have murdered the air traffic controller but many people would loose their minds Im not saying it was ok for him to do it I’m just saying I think everyone could understand the pain he felt and not being able to get on with life but the controller did not want anyone to die and all that killing him did was ruin another family more children without a father, a widowed wife forced to raise children alone.
