Fever threshold
covid.joinzoe....
www.medrxiv.or...
Uninfected volunteers (twins), n = 1,089
Hospitalised, n = 1,227
Community based positives, n = 3,972
Results
Among unaffected volunteers
Lower BMI (p=0.001), and older age (p less than 0.001) associated with lower basal temperature
Basal temperature showed a heritability of 47%
In COVID-19+ participants
Increasing age associated with lower temperatures (p less than 0.001)
For each additional year of age, participants were 1% less likely to demonstrate a fever
Ageing affects temperature in health and acute infection
Older people with COVID-19 were also less likely to have a fever reaching 37.8°C
Current health guidelines, 37.8°C or more should self-isolate
Using a threshold of 37.4°C for over 65s is as sensitive and specific a sign of infection as 37.8°C in adults under 65

Experienced the exact same thing. Because of this channel I’m weeks ahead of the talking points when anyone brings up the subject yet it really doesn’t seem to help. People argue with me who don’t spend two to three hours a week listening to evidence based updates and info. Just argue from stuff they caught on cnn or some single argumentative anti vaccine video that’s been out for five months. It’s bizarre. I mean I know it’s not an education but at least its paying consistent attention with an open mind.

@@milescoleman910 That's pretty good. I don't even get that, I just see people who want to argue based on their feelings who haven't ever heard a single thing. Now I see why highly educated people don't argue with the general public as a habit.

if the less knowledgeable dont seek it out themselves, they are unlikely to listen to a (apologies) non expert. Why would they? I gave up a few months ago, and avoid either the person or the topic. I happen to be rivetted with the facts, knowing and learning makes me feel safer. Keeps me on my toes, and less worried. Plus, this is a unique time in a human's life, we are living thru something that will never not be in future history books. The medical knowledge I can gain here is like a magnet. I do realise, some do not want to know.

To be fair Guernsey sucessfully had an elimination strategy during the first wave and had pretty much closed ports to the outside world since and had had only a few cases pop up since eliminating covid and were living for many months in a normal way as an isolated island community. So yes they now have cases and its not great for them but please dont mock them as if you check their history they have been one of the most sucessful places at dealing with covid and keeping their island covid free. So they have been able to live the 'old normal' until the recent cases arrived from somewhere.

How come Sweden is at herd immunity without a vaccine and without locking down. Why have so many people accepted totalitarianism. Zero evidence that masks work. John, I hate to take issue with you but I am concerned about your reading of the Bloomberg map vaccination stats. Israel has issued 54 doses per 100 head of population but this does not mean 54% are vaccinated. If you look at the Bloomberg map, it says 33.9% have received at least one dose and 19.7% are fully vaccinated (i.e. have received two doses). The same issue arises with all countries. So for Israel you can only say that 34% of the population have any vaccination benefit and many of those will not have yet built up their immunity as they will only recently have received their first dose.
@pizzashtndragon
@pizzashtndragon 4 жыл бұрын
It's not a case of 1 vaccine provides zero benefit until you get the second. 1 vaccine provides most of the protection on its own. The second dose only boosts it. So if the second shot is just to boost it, 33.9% have had 1 dose and 19.7% have had 2, surely saying 54% are vaccinated is correct? Or does the 1 dose figure include the people with 2 doses as well?

My normal temperature is 96.8F, so it’s hard to convince anyone that I’m feverish at 99F. Strangely, US doctors are still convinced that everyone is normally 98.6F, but it’s just not true. A lot of women, especially older women, have lower baseline temperatures.
@Beth-ie
@Beth-ie 4 жыл бұрын
My brother and I always get chills, feel quite sick whenever we have a temperature of 98.6. My normal temperature is very close to 97.0F (sometimes lower). There was a study (I forget who, but heard it on NPR within last 6-9 months) that showed that the average body temperature has gone way down globally, partially due to the invention of antibiotics, much lower cases of TB, etc. So, 98.6F is not "Normal" anymore. It's more like 97.0F. Or lower in some instances. I remember the frustration that this is not widely known, and healthcare workers are still going by the very old 98.6F (which was figured out early 1900's, I believe, when infection was rampant.) Should check yours daily for a week or so to figure out your average ... always at the same time of day. Stay well, Dr. Campbell viewers!!!

I'm no expert, but I think it's not a bad idea for a country like Australia or New Zealand to wait with vaccinations. With so little infections going on in the country, they can afford to wait until there is better data, and then choose the best vaccine. That seems like a safe strategy. They have earned themselves the luxury of being able to do it that way.

@@SafireThompson It's because you don't know the situation. There was no community seeding since middle of April. No restrictions except for travelling since July. When the dance festival was on we didn't have any known cases except for couple self isolating travelers. 4 community cases were found on Friday afternoon. Saturday morning we were advised to keep social distancing and immediately cease any gatherings. From Saturday mid day full lockdown was implemented. Also our numbers look scary but our contact tracing team is brilliant and everyone who have a slightest chance to be infected has to self isolate and they are testing, testing and testing. We will start vaccination programme as soon as it is availiable, (we also do not have facilities to make our own vaccines here). The situation is not dire here & we are just waiting in line for them to arrive. Many other countries have current emergencies & it makes sense they are at front of the line, I just hope countries aren't using their clout to elbow their way up the queue so to speak. We all need this vaccine, I'm disappointed to see the nationalism that is arising around this.

There's quite a lot of preparatory work going on in Australia. TV ads about vaccination have started, quite a bit of work regarding the logistics of distribution, additional people being trained, etc. Various major hospitals have been designated as Pfizer vaccination centres, while Astra-Zeneca will more likely be distributed by GPs and pharmacies. Pfizer vaccine has been approved (for general, not just emergency use) but the stocks for 5 million people (for the higher risk end of the community) have not yet arrived as far as I know. Australia doesn't have facilities for producing mRNA vaccines. They could be developed but would take into 2022 and probably detract from churning out other types of vaccine. Some research centres ere focussing on developing future vaccines with an improved response to mutant variations. I expect similar general approval for Astra-Zeneca to occur within a month or so, but initial overseas stocks have not yet arrived. While they seem to be optimistic this will still occur (in spite of the EU) it would not surprise me to see the Australian produced AZ get there first (they're talking the end of March). The manufacturer (CSL) also does flu vaccine, which usually come out in about April here, so not sure how production efforts on one will affect the other. Last year there was a big push to get as many as possible vaccinated for flu and pneumococcal disease to avoid double infections with Covid. One article (in The Guardian) that caught my eye, but doesn't seem to have attracted much other media attention, is the case of "Damian", a 50 year old father of three from the Central Coast (a set of towns and villages around pretty coastal lakes north of Sydney). He had COVID-19 early to mid 2020, got sick enough to go to hospital and was given oxygen, but then made a spectacularly good recovery, testing negative shortly afterwards, but otherwise showing clear signs of having had the disease. Described as a 'immunological unicorn', he apparently had a "100 to 100 times better response" than average without any of the side effects of a too strong response.

The math on the space of available mutations is wrong; sure, there are only around 30 000 RNA bases in its genome, but the number of possible combinations is exponential; so the fact that the virus has explored at least 10 000 (or whatever) mutant sequences in this space is a vanishingly small fraction. Obviously, the details matter here; not every base in the genome codes for an amino acid, and possible some of those bases aren't even expressed at all (no idea); and then there's the fact that the RNA->amino acid translation has some redundancy (so the number of possible mutants isn't quite 4 to the power 30 000, but it's still huge). Even if every single viral particle ever churned out in this whole pandemic were to mutate into a unique rna sequence, we'd still have explored essentially 0% of the possibilities, to a high degree of precision. I have no idea nor any expertise in estimating how many mutations are both "viable" (can replicate) and "real" (code for different amino acid sequences), but it could well be an incomprehensibly high number. There's no reason to assume the number of possibly viable mutations is in any meaningful sense bounded. Additionally, the genome length isn't capped at its current size anyhow; mimiviruses are around 30 times larger. In essence: the number of possible mutations isn't capped by it's genome size; it's capped by the fact that evolution cannot suddenly explore radically different designs; each step must be small - and sometimes there just isn't any reasonable small step to improve fitness (but you'd expect that from a well-honed creature that's extremely well adapted to its niche, not something like a novel virus that's just made a significant species jump).

@@ME-ru4hv When John says that the virus has limited possibilities to mutate because it only has 30000 RNA bases and around 10000 mutations are already known, he's wrong: there are incredibly many possible mutations that might still occur.

Most people will be ok. That's the way to bet, though the stakes are high, and with the average age of those on ITU respirators coming down it might soon be that Elon Musk will look foolish with his refusal to have vaccine as he's the wrong demographic. I have taken my temperature daily since 28/02/2020 and the long term average is 36.6 with a standard deviation of 0.25, so it is tempting to say "I have a fever of one degree if it is above 37.6", but maybe Dr John can tell us if it is just the lower basal temperature that counts, or is it the case that older people (77 in my case) do not develop such a high fever for a given basal temperature?

It’s like being in a bad film because this shambles could never happen in real life, or so I thought for 60+ years. I’ve decided that I don’t even know / understand my immediate family. I’m convinced that I must have been adopted, although mother did tell me once that she never wanted any children. 🤷🏻‍♀️. PS. I like Freya’s portrait 🙌🌈❤️

I have shielded for a year, only place I have been is to hospital appointments. This evening I got a text saying I tested positive for Covid from NHS Track and trace. From a test taken by ONS - 28th Jan and I had my first vaccine on 29th. I have no symptoms, I am on the extremely vulnerable list. So nervous and I feel so bad that I went to a vaccination centre, not knowing that I had tested positive. I am a bit scared also, have been so careful. I don't know why I have no symptoms either.
