Hey Sam, I was inspired by your content to pursue a career in EMS. I got my EMT three months ago and am now working for a private ambulance service as a stepping stone into EMS. We routinely transport a lot of dialysis patients, and I think a lot of people would benefit from some content on caring for dialysis patients, as well as complications that go along with hemodialysis. Thanks!

Assessment tools(good bp cuffs/steth, ent kit, pulse ox) kind of stuff… also some sim stuff. Wound packing simulators would be amazing

I had dudes down range who had the prototype for this bag. A lot of them used it for meds/blood admin and their HABCs were in a large fanny pack, or offloaded to their guys. Military friends who may be considering this bag, remember, your dude’s IFAKs should be stocked well enough so you shouldn’t really have to use any of your stuff anyway, minus specialty interventions and treatments, obviously. My HABCs were in a NAR CCRK that I wore around my waste and my aid bag was just used for IV access and blood. I understand that things are different civilian side, just something to keep in mind.

I love seeing how experts roll and learning about kit. I wonder how much closer to a pack you'd use if it had that two extra inches of depth inside, the outside pouches had a bit more room, the shock cord holes were a little bit easier to lace up and if it had some useable MOLLE to attach a sub-pouch... I could see something like that being the basis of a hiking survival pack.

Definitely agree if your working off your kit first its probably big enough. But for the extended care alpine rescue... probably not. But hey I'm just a 1 year EMT so love to hear everyone's opinions.

Teardown systems just released a tegris insert for the middle compartment that seems a lot better for organizing than the individual Spiritus ones

A lot of the comments nail one of the target end user needs of this bag being more of an ALS only setup, but one thing I think people are missing is that a lot of the special dudes who are using this bag abroad are part of a two or more provider team where more common items can be split and specific items can be carried by one or the other 14:07

With iGell’s and AirQ3’s sizing is based on weight. I have found that someone might match the weight requirements for a size 4 but the size 4 might be to big for their throat and it can cause blood in the airway if not lubricated properly. Just food for thought.

1st line kit should contain : cric kit , TXA , sam medical IO , tourniquets and chest seals / hemostatic dressings . Larger fanny pack with additional STB items ( mystery ranch makes good ones ) . Pack should be larger ALS , additional TXA , assessment tools pulse ox / cuff and scope , realistic meds , no fluids , have an additional bag for rehydration that’s back at the insertion vehicle , collapsing BVM , hypothermia kit are essential for survival .

No ex mil but have worked with some and they still have to do a paramedic course if Im not mistaken to transfer to civilian role.

@@danielkinsler46 It all depends on the branch and MOS. Most 18Ds and PJs will have their paramedic when they come out of their respective pipelines but they dont necessarily need to maintain their national registry. 68Ws are traditionally the equivalent of EMTs stateside but the Army is transitioning more to the paramedic scope. Corpsmen are again, hit or miss, but I don't think they are inherently have any equivalent civilian certifications unless they pursue it themselves.

I agree this bag is to small. But I want to say something. Allot of people saying the spiritus delta bag is to expensive need to realize. Although it is a lot of money. It’s comparable to step packs and merit USA. Almost every ambulance in America uses stat packs or merit.

Interesting that chest seal goes in airway for you. In my mind that’s a cat haem.

Chest seals are a breathing intervention and are performed at that point in MARCH so I almost always put them with my airway and breathing interventions

@ would you not pack the wound and then chest seal over the top if it was somewhere in the chest? Going HOTT principles for trauma rather than ABCDE or MARCH? ‘Treat as you see’ style?

@martinausterfield8597 no. You never pack a chest wound. The only time you might put a seal over wound pack would be the neck. The entire purpose of a chest seal is to allow for negative pressure respiration in the presence of a sucking chest wound.

This bag is weird because it’s clearly for a very specific person- I just can’t figure out who, based on its design and price. And that’s coming from someone who has LOTS of Spiritus stuff.

It’s for SF medics who A: work primarily off their plate carrier and belt B: have a larger aid bag stocked in a vehicle/CCP and C: are able to distribute some of their larger supplies between their teammates

@@PrepMedic @literateduck In the info/promotional video that spiritus did with an SF medic going over his set up, he had a ton of (especially ALS) stuff on his carrier and belt. The bag was just a part of his kit. I think non-medic SF guys have a fair amount stuff (esp BLS) in their ifaks, so the medic can supplement that with advanced stuff from his kits and use the bag as a backup
