Hello. Thx for amazing explanations of this important topic but i have a question for you as well for other colleuges regarding this postoperative sensitivity which forced scientist to create a new 6th genereation of bonding because as said the 5 th total etch system reported many cases of postoperativ sensitive tooths. However there is a study and you can find it i Cohens Pathways to pulp chapter number 13 page number584 that has shown no significant differences between two adhesive systems ( total etch or self etching system ) but the most important variable that affected the pulp was bacterial leakage in both systems.

Respected sir, Thank you very much for the inspiring lecture.. I am an orthodontist and senior to Dr Ajay praveen in ug by whom I got to see the videos.. Since we too deal with bonding it is really helping us sir... Sir, I have a doubt...we use bondable Buccal tubes in molar as alternative to banding is there any primer or etching with Hf acid and primer application on the brackets surface will improve the bond strength

But we do have a bonding agent which helps composite bonding the metal crown. Ideally we cant bond the composite to metal but with this you can do it. Google ceramic repair meterials. I think you would get it

lecture was just superb we just read in buks blindly v dnt understand anything how it applies clinically but here i can imagine everything how it actually works clinically But i have 1 more doubt if postoperative sensitivity after composite restoration exist wat is the treatment for it do v need to redo restoration or use any liner ??

It depends on the cause. Base can be given if the preparation was deep, 7th gen bonding agent can also be used to reduce sensitivity since 5th generation may cause sensitive for dentin. If microleakge is the cause redoing helps.

Sir how to selectively etch the enamel only and not the dentin as you mentioned in selective etch and bond technique?

U can use gel.form if etchent to apply only on enamel and wash it off after 15 to 20 sec.. then apply 7th gen bonding agent to enamel.and dentin. This is called as selective etch. Is the answer helpful ?

Yes. They are similar. But bonding agent may comprise.of.primer, conditioner and adhesive.... adhesive is mostly resin..

You can cover it as dentin bonding agent. Usually it talks about the generation, differences, self etch, self adhesive dentin binding agents, hybrid layer, self adhesive Composite.. its usuall seminar.. if you want good reference for flow chart check sikiri book. Also if you are preparing for pg seminar then you have to include the antibacterial bonding agents, and few other research like nano leakage

Bt sir when i read this topic from my book i found diffrences in some points like- •in book 4th generation bonding agent is written as total etch •5th gen. bonding agent is written as one bottle bonding agents. •6th gen.bonding agents is written as-single bottle adhesives combining etchant,primer and adhesive. Sir now i m confused plz help me and made me underatand the correct one.My book :2nd edition Nisha garg operative dentistry. Nd sir i request u to make vedio on cast metal restoration and tooth prep in composite. I hav my final exams soon nd i m not able to undersatnd these topics.

Outstanding sir!! I am so happy after seeing ur vedio all my doubts are cleared.. But I'm left with one pls help me with this sir i searched a lot but fed up! As the etchant when applied to enamel then it is gonna demineralize only 10-20 micron in length and say the enamel thickness was around 0.5mm so how can now the etchant reach the dentine ... And when we have a irregular ly fractured tooth then some places have enamel and some have dentine then what to do sir.. How can we selective ly approach..?? As etchant in enamel demineralize upto 20 micron and in dentine upto 7.5 micron so I'm wondering this could occur only when do this process seperate ly??? Sir pls pls help me and clear this 😞...

Thank you for your great explanation But I have aquestion about selective etching bond system how we can know that phosphoric acid that we applied on enamel it work only on enamel not worked on enamel and dentin as usual to be able after that adding acidic primer?

You are right. Its not possible to different always . But there are few things which can help. 1)Enamel and dentin colour is different. That cab help you to apply etching on enamel 2) u can use gel form which is easy to restrict and allows u to apply at specific area. Inspite of this dentin may be etched but it may be still less than applying all over

No.. its a strong acid..

Reducing the etching time will.not etch the enamel properly and gives less bond strength.. pls check etchent video.

6th gen uses non rinse etchent meaning you dont wash it..

Best is selective etch as mentioned at the end of the video. If not that then I see if there is more enamel and less dentin then total etch 5th gen, if more dentin then 7th gen

Ok , suppose I wanted to etch, then use a bonding agent for underlayment, to then apply a GIC type 2? Will a GIC adhere and form a bond with the bonding agent? is is it even necessary to use a bonding agent with a GIC type restoration? I think ething may be all that is needed, but I feel a bonding agent might help with affixing the GIC.

That mostly wont happen since the layer is already coated with a bonding agent and gic will be prevented from coming in contact with the tooth

Bondint agents are not required with the restorative gic. Only conditioner is needed

Yes. But it creates microporosities which are not penetrated by GIC.. Gic just needs removal of smear layer which can be done with weaker acid. Thsats the conditioner

Since those are not used now I had to cut it out. The bonding of new composite to old composite is similar to total etch technique. One should use 5th generation for it.

Desensitiser is basically a primer with gluteraldehyde which precipitate the protein in the dentinal tubule and block them. This was popularly marketed as Gluma Desensitiser. Along with it has HEMA.
