I've read that it's best to keep your elbow fixed to your torso while performing external rotations and initiating the rotation at the elbow ( sort of like the elbow is pivoting). What are your thoughts on this? The video seems to show the woman performing the movement with space between her humerus and torso. Thanks!

Andrew P. Tonelli thanks so much for the response! For most, leaving a little "room for air" or space between the elbow and the torso is the best position for the shoulder to compete this exercise. This allows for the rotator cuff to be optimized, without winding up the shoulder capsule. You should even notice this on yourself. If you do this drill with your elbow tight to your side vs a little bit of room, it should feel more comfortable. It all comes down to your personal anatomy - some will need more or less "room" depending. Keep in mind - changing the angle can also increase and change the difficulty of this drill in the future! Hope it helps :).

My both of my shoulders have done this my whole life. Mine do it if I rotate my arms inward. Like if I have my palms facing the ground and rotate my left are clockwise or my right arm counterclockwise it pops out.

Thanks for the comment. Unfortunately without seeing your shoulder I can't advise you specifically. Clicking can be as simple as a compressed joint needing some mobilization of certain muscles - but a clunking and shifting (as need in the video) involves a much more strength based program. I'd recommend you follow-up with a health care provider you trust to set you on the right path specific to your shoulder.

Joar Vidfält should internal rotation is super important - but also depends on what sports or activities you're doing. To sum it up - if you are doing an activity that demands a lot of shoulder internal rotation (ie a power clean), your shoulder will compensate in order to push through the movement. Hard to say without seeing you my friend. Depending on the demands you're placing on your shoulder - you can get away without surgery. But for some, they need that extra stability.

My right shoulder clicks when I lift my arm. When I wake in the morning my shoulder is stiff almost locked. I'll lift it and it will crack sound like a tree snapping sound of even wakes up my wife it is so loud. In the beginning it didn't hurt but, not my arm and shoulder hurts and I daily get some hand numbness.

Hey I appreciate the video, youre a good instructor! I do have a question, you mentioned to get it checked out, but I dont have the money to go to physical therapy (I had pysio but all she did was message basically - I stopped cause it wasn't helping and had no $). My problem is my shoulders are forward, and its hard to breathe, but when I move my shoulders back I can breath better, so what I got a couple days ago online is a comfy-posture-brace (I think certified by doctors, online on amazon) and been wearing that all day. I was hoping it would fix my problems (breathing and posture) and it does keep my shoulders back but doesn't really help breathing as it doesnt keep my neck or shoulder blades back. My question is: Its uncomfortable to wear this all day as it causes bruises, but if I do wear this will my shoulder eventually learn to be in place? I font have any family doctors and the walk in is really bad, so I'm out of all my options. What do you suggest? Is there any home stretches that I can do that'll eventually fix this? I'm at a bad spot cause I cant breath = I can barely talk = not great. Again I dont have any real access to doctors or money for physio. Thanks for reading!

How is it now? Yea the brace is acting passively to do the job or the muscles which is not something you want to do long term. I would work on strengthening certain muscles and stretching others. Look up upper cross syndrome to get more of an idea of what you may have. It could be your thoracic extension (upper back) causing you to have a hunched position making it more difficult to breathe. You want to strengthen your deep cervical neck flexors (look up exercises) if you have you have too much forward chin posture, and work on lower trap strengthening exercises (look up) , while stretching/releasing other muscles like the the pec minor (shown in this video) and upper trapezius. Hope this helps.

Don't make the same mistake I did, never talked to a doctor about it and now I have serious pain in the joint. The pain came on so fast after a lifetime of feeling no effects from the repeated dislocations. Next time you're at the doctor mention it! I wish I had said something when I was a kid, might need surgery now 😑

@@DeskJockeyPhysio Leite probably means the difference between the clavicle or scapula length, however Leite that comes down to mostly genetics and your shoulder position during your childhood. I would advise to stretch your traps and surrounding muscles and strengthen your posterior chain muscles on your upper body as those muscles are often neglected. Especially around your shoulder.

I have winged scapula and i feel like my shoulder dislocates partially when i do internal rotation. I did a labrum tear off the internet (the one where u point ur thumb downward and have someone push your shoulder in) and i felt pain. I have no pain during sleep, only pain i feel is when i train my delts and do pull ups. I was wondering if this means that i do in fact have a labrum tear or that those are common symptoms of scapular winging.

I have same her instability shoulder for both side i just fouce on internal rotation and external rotation strnghting exerses? Or you have other exersise and thank you lot.

Unfortunately, this totally depends on a case by case basis! The best place to start is a unique assessment. Same here. With my left arm, if I grip something, I can partially pop it with no pain. If I pushed it more I could probably fully dislocate it without pain but I don’t want to cause problems with it

Hi Michell, thank you for this video shedding some light on shoulder instability. My rheumatologist diagnosed me with ankylosing spondylitis a few months ago and my shoulder clicks and get sore easily. This clicking started 4 years ago and it became so bad that my whole arm tremble as i flex out which the neurologist then diagnosed it as Essential Tremor (ET). I feel ET is a wrong diagnosis. The clicking has also worsens with any movement of the shoulder. In your experience, can shoulder instability cause vigorous shaking of the arm? - Estar

Estar Rae thanks for the comment. Without seeing you in person I wouldn't be able to comment on your specific situation. That being said, generally, shoulder instability can be related to clicking, but not the regularly shaking associated with a essential tremor. An imbalance with the muscles of the shoulder blade and rotator cuff can cause some shaking, but not often to the extent that would warrant a diagnosis of an essential tremor. I hope that helps

Estar Rae Happy it helps! I would definitely continue to investigate the origins of that tremor though - it sounds like you have some great medical care, they should be able to guide you in the right direction.

Is there additional exercises that can be done? I dont have access to a resistant rope like that unfortunately. I've been suffering with what i think is a loose shoulder for over a year now and my doctors havent been overly helpful unfortunately. Sometimes I make it better through stretching but sometimes I also make it worst. Sometimes I feel pressure on my throat at the collar bone but it's in alignment with my other one bar some hardness behind it close to my neck. I'm concerned that I may have deeper damage such as a displaced rib or something as the issue started after I came off my bike and landed on my shoulder, And its progressively gotten worst as times gone on but I didn't get it seen to at the time of impact because I'm a numpty😅. It now pops and moves freely out of the socket quite often, scrapping and popping as it goes. Again i have tried to get it seen to but because my motion is fine bar some tightness behind the shoulder blade but not really that painful more just really uncomfortable it's often been overlooked. The only comment I've received is my shoulder blade seems slightly winged. I'd been incredible greatful for any recommendations as to what addition exercises can be done and how I can resolve it. Oh also worth mentioning is I have hyper mobility so that's obviously adding to the movement Issue I'm guessing.

+mcxza thanks for the comment. She has we we call "shoulder instability". Meaning when she brings her shoulder into certain directions the shoulder is missing stability. I can't say if this is the same for yourself though +mcxzsa If your unsure about your case I'd recommend popping in with a healthcare provider you trust.

She does not. That's a much more systemic condition. But she does have a loose shoulder!

@@DeskJockeyPhysio shoulders dont just become loose without severe trauma and incorrect healing. The thing that makes a joint this loose is faulty connective tissue. Take it from me, a person with EDS who had no idea they had it, and thought I just had a "loose shoulder" too ;) Not saying your corrective treatment is wrong in any way and you're not about to be in a position to diagnose her. Im just observing!

@@Crowcifixx thanks for the thoughts. EDS a congenital issue that effects a personal connective tissue globally. Some joints more than others. I am very familiar with it, and there are specific tests we can use to diagnose it. I appreciate the thoughts, but thankfully this patient does not have this condition. All the best my friend.
