@@DeukSpineInstituteI was hit by a semi in September of last year. Upon getting an MRI, it was discovered that I have a 3mm bulge in my back and 1mm bulge in neck. Them when I went to the neurologist to get an x ray, it was discovered that I have a degenerating disk. Dr said I need surgery to replace. Would you say this is proper? He also said that I could do an injection but it’s only going to mask my pain, temporarily. I’m trying an injection June 3rd….

As a responsible, Pain Management MD , I always look at the MRI prior to any injection to note any level of critical stenosis. It could be a medial branch or facet block to treat axial pain prior to you doing a multilevel fusion. It’s always important to rule out mechanically, mediate neck, pain rather than disc related neck pain. And yes, I would never inject Medrol into the cervical spine instead the better choice is dexamethasone preservative free, which is non-particulate

I’ve had multiple epidural lumbar injections and radio frequency ablations. My pain management doctor is exceptional and I’ve had zero problems after my procedures. I’m doing this to manage the pain until I have a appointment with a local spine surgeon. I’ve not found a need to be scared of my pain doctor. He saw the MRIs I’ve had and configured a plan to fit my particular requirements.

@@aradeukmedjian5957 I had office visit with a spine surgeon last month. He spent a total of 5 minutes with me. Briefly looked at the MRI and X-rays, barely addressed any of the visible problems and was dismissive. As he walked out of the exam room I had to ask him what was the result of the bone scan he had me do. Standing in the doorway he said, “ oh, it’s minus 2.2, osteopenia, so take vitamin D and calcium “ and walked away. So now I have an appointment tomorrow with a different practice and another spine surgeon. I’ll have the MRI CDs, written reports and whatever else they need. If I get treated that way again, all hell will break loose. I’m always very polite and willing to discuss any plan or recommendation a doctor has, but I do not appreciate being dismissed so cavalierly. If that occurs, I’ll give up trying to get any treatment and just live with this until I’m unable to do anything.

OMG I just had an epidural injection yesterday! It didn't really hurt during procedure.....but I was very stiff afterward and this morning I'm in worse shape then I was yesterday! Plus about $3k on a bill! I've been to 1 neurologist, who maybe spent 3 minutes tops during my first visit, then he sent me in for. MRI/X-RAY a month later.....then the next day I had go back in to see neurologist and he didn't show me my MRI/X-RAY RESULTS instead told he was sending me to another neurologist/neurosurgeon. This first neurologist didn't want to deal with me it what it was, he really didn't seem to care and his staff were very rude and acted as though I was an inconvenience! And he told me I NEEDED A SPINE FUSION SURGERY I was in a state of panic until I finally got into see the neurosurgeon and he went over my results. I have a herniated disk, arthritis, sciatica, and fibromyalgia. The herniated disc is not big enough for surgery at the time so he wants me to get these epidural injections then if I'm still in all the pain then he will look into just going in and removing the damaged disc. But after reading all these comments and what this here Dr is saying I'm always terrified of getting another epidural injection! Is it me or has anyone else had a lot of trouble getting medical professionals to actually give 2 shits to help us? I'm in tears over all this. I've been off work since February, my money is running out and I feel like I'm in a mari-go-round with getting anywhere with my problem! Any advice would be greatly appreciated

If it isn't them dealing with it, majority of them just simply won't care. Why get a job like this if they won't help? Money always plays a hand in human livelihood and who gets helped before another. If you have money, they are at your feet asking if you need anything in a nice cozy room. If you are like me and others with low income health insurance, prepare to be in pain just waiting for them to take care of you hours later just to send you home and say oh well take the pain. I'm not mad at all doctors, don't get me wrong there are great ones out there, but I am speaking on the ones who care for money and treat people rudely.

I had lasik done without being properly being informed of all the downsides and potential complications. yeah I try and stay as far away from having any procedures done as I can. No offense to the doctors but at the end of the day we are still cutting into the human body with knives etc. When looking at Epidurals I saw a video that showed they had to puncture into the spinal cord channel with a needle, basically only using feel to sense where they were. An inavertant knock of 1cm could mean you have a needle shoved directly into your spinal chord. For me I have a hydrocele but I am happy to drain it myself, after watching hydrocele surgery videos where the doctors cut into your scrote and treat it like they are working on their truck.

I had double spinal fusion sept 2013. The dr that did my procedure is the spine dr for the LSU football team. They let me lay in recovery for almost 2 hours and couldn’t control my pain. It wasn’t until I started coming to and felt a weird sensation in my feet and it moved up my body. I was paralyzed from neck down. I had a epidural hematoma blood clot from C-2 through C-7. I had emergency surgery. I fully recovered movement. I live in horrible pain. I was a captain in the fire dept. That’s how I initially got injured. At a fire. This surgeon who is a well known surgeon let that happen to me. I do have an amazing pain management dr. He is so caring and sweet. I had a epidural in Feb. it made a huge difference. After breaking sedation during a discogram in tremendous pain and getting off the OR table with a needle still in my back years ago asking the doc what he hit me with due to the tremendous pain I lost my interest in “fixing my back”. The anesthesiologist was rumored to be a poor one so maybe the blame lies there? It was horrible absolutely horrible. The report read as advancing the needle into the L3-L4 the patient actually broke sedation and came off the table writhing in pain. Patient was comforted and gingerly helped back to the table. Dr’s need to understand a patients fear after an episode like that. I’m done being poked and prodded.

Provocative discography should have fallen out of vogue years ago. Diagnostic TFESI (transforaminal epidural steroid injection) or SNRB (selective nerve root block) has just as much diagnostic value, if not moreso. You should have been counseled to how uncomfortable this procedure is beforehand and I'm sorry you went through this.
@lh98
@lh98 Жыл бұрын
@@wagdog1 thanks for your reply. I went through a battery of steroid injections and now I hear multiple steroid injections can actually cause break down of the surrounding tissues, tendons, etc in the places the steroid is injected. I’ve had them in my shoulder and knee too and now Idk what to do anymore because the last thing I need is something that gives pain relief but also damages my body. As far as my back goes the only improvement from the steroid was the sciatica but the stabbing pain was never helped. My life isn’t a great one but I’m still here trying. I can say undoubtedly that my life was forever altered when my back pain started in 2006 and that led to depression and a serious life change. There are so many ppl that judge folks thinking they look normal but they have no idea because they can’t see but a few moments of someone and should not cast judgement until they’ve walked in their shoes. Medical issues like this can completely change the course of someone’s life and has. I feel bad for humans that had to go through life with issues like back in the day this without any medical interventions but I also feel bad for those that endure it with medical professionals that are too afraid to treat a persons pain because some idiots abused things like medications. Counseled on how uncomfortable it would be? The pain was so severe I woke from sedation on the table in the OR holding my back asking what he hit me with. It felt like I was hit with a baseball bat. Yeah if he had told me that would happen I’d of never done it. He tried to get 3 specialists over 3 hours away to do it again but based on what happened they refused seeing me and I’m glad they did. I’d never subject myself to that again. I’d say medical professionals don’t always fully know what they are doing. Tried a spinal cord stimulator too and that was no help and felt like I had fish hooks in my back. Was so glad after a week to get that yanked out. Done being a guinea pig but what’s sad is most ortho docs think steroid shots fix everything. I went to a well known neurosurgeon in my area and he gave me the best advice. “I could fuse your spine but in time it would cause issues with other vertebrae and we’d be fusing more until you have no flexibility. I’d just manage it versus surgical” best advice ever even though my life is still always affected by it. Sorry to write so much, it clearly is one of those subjects... Good day to you. I did have a shot of what was supposed to morphine in the spine at one point when they considered an intrathecal pump but to my recollection it wasn’t helpful. Perhaps improper placement? Who knows.

Doctor kept telling me that the feeling will come back in a fews and it didn't , he kept telling me a few more hours and I became terrified that it was going to be permanent .. He ended up admitting me to the hospital and I did get full feeling of my leg back to normal after 30 hours ! I was terrified. This happen about 10 years ago and now my new pain doctor wants me to start getting epidurals again !! Even though I told him what happened to me and I don't want to get another needle in my back ever again because it terrified me , he is still pressuring me into injections and hinted around that if I don't I can't continue to go the pain management and then I won't be able to continue to get my pain medication... This is a scare video. Any medical procedure has risk. I have a rare arthritis form. I’ve had 3 surgeries and 44 interventional procedures, with a minor complication from one. And unfortunately, insurance forces my pain docs to do steroid injections every year before they’ll pay for a rhizotomy, which I have done in 6 areas every year. I have a great team. Research your doctor. I would have less than half the number of procedures if insurance didn’t force stupid steroid injections.

@@slc1161Yes. It is a “scare” video. A few numbers: The “success” rate for epidurals is said to be about 80%. But what is the definition of success? A peer-reviewed article in “Spine-Health” asserted the following: “An analysis of several large clinical trials indicated that 40% to 80% of patients experienced over 50% improvement in sciatica pain and functional outcome from 3 months up to 1 year when 1 to 4 injections were given in that year.” So, in a worse-case scenario 39% of subjects had less than 50% reduction in pain. It was also reported that “the effect of the first injection decreased after a considerable relief period, for example, over 1 month.” It can be inferred, then, that “considerable relief” occurs when a patient has 50% less pain for 31 days. It has been clear to me that pain cannot be subjectively assessed by a percentage. The bottom line is that epidurals for pain are a potentially devastating invasive procedure that may be of little long-term benefit for almost 40% of those treated by this intervention. I do get it: there are those unfortunate patients that experience “suicide pain” or lesser pain that is almost intolerable. I would never suggest they discard any procedure that offers the hope of less suffering.

Your own website offers epidural steroid injection as an option for spine and radicular treatment. Also can you discuss the fact that there are greater than 10million epidural steroid injections performed each year in the US. SPinal cord injury related to ESI is a very serious but very rare complication. As you mentioned need to ensure safety of a treatment. Maybe highlight the complication rate of cervical spine decompression! Although it is the right choice for some, it comes with a significant risk of complications nerve injury, paralysis, and death compared to epidural injection.

They not only look at all my previous xray slides saved from every procedure before, but they have a C arm (xray) live in the procedure, they go very slow, they use decent stuff and they use dyes to check position before they inject the steroids and local in. This is malpractice regardless of why, the procedure isn't a scam. The procedure means I can walk and don't spend my life in a wheelchair. So I would say just make sure you have GOOD consultants and make sure you look into their history, also discuss your options with them and make sure they are being thorough.

They still ignore all my tarlov cysts . I've had so many epidural spinal injections, I believe the tarlov cysts were caused by failed injections. I think the solution went into my spinal cord and had nowhere else to go but forming a cyst out the end of my spinal cord .. Did you get any relief from having them decompressed ? @@aradeukmedjian5957 The last epidural that was suppose to help my pain paralyzed my right leg for 30 hours and I was in severe pain.. I lost all skin sensation and I couldn't feel or use my leg and foot. I couldn't even wiggle my toes. The doctor kept telling me my feeling would come back in an hour over and over. He had to admit me to the hospital and I was terrified the paralysis was going to be permanent. Thank God at around 30 hours I got my feeling back in my leg and it's back to normal , thank God , but I was still in extreme pain . So, my primary doctor ordered a MRI and I now have numerous medium - large tarlov cysts on both sides of sacrum area that weren't there before. The cysts are filled with my nerve endings and spinal fluid ! The epidural needle and medicine accidentally went into my spinal cord and there wasn't room for the meds , so it blew out at the end of my spinal cord and formed all the cyst's in my sacrum area. The cysts have my nerves endings inside them with spinal fluid. The size can increase depending on what activity I'm doing that would cause my spinal fluid pressure to change. Heavy physical activity, flying in an airplane , storms; bariatric pressure changes during storms , so could my spinal fluid pressure etc .. Changes in spinal fluid pressure causes even more pain because there's more pressure on the nerves inside the cysts. Only 2 surgeons in the country will do surgery on them for obvious reasons; you don't want to drain your spinal fluid and nerve endings are inside. One time I got up during the night to get a drink of water and I was in severe pain; next , I found myself at the end of my bed on the floor ! The pain was so severe I fainted from the pain !. That's only happened once , thank god. Anyways, When all the large tarlov cysts showed up on my new MRI right after my botched low lumbar spinal epidural to help my pain all my doctors ignore the cysts and never mentioned them. Then when I ask about them they tell me their incidental findings and they don't cause any pain and I was born with them . But, when I Google to find information , they do cause damage and lots of pain and I have most all the terrible symptoms. I've have other things going on with my back degenerative disc and facet disease , compression vertebrae fractures , bulging and herniated discs, bilateral stenosis , arthritis , my body has fused together my L5-S1 in a bad position and now severe osteoporosis and Tarlov cysts. .It's been a struggle and a nightmare experience and has opened my eyes concerning doctors. I have very little faith left for doctors . Not just doctors , but our whole healthcare system (USA) So, beware of spinal epidurals there's very serious/dangerous consequences . Focus on fixing the issue (surgery ) while you still can , because the epidurals speeds up bone loss and then they can't do surgery and you're then a lifetime pain patient. It's all about the money. Doctors make a lot more money giving epidurals , physical therapy , burning and destroying the nerve , implanting a spinal cord stimulator....etc. They don't make money prescribing cheap pain meds that work . So, they're pushing/forcing the other options that make them a lot more money. That's what their "War on Drugs" is all about. Sorry this is so long , please beware of spinal epidurals , there dangerous and have severe consequences .

I asked for an updated MRI last August and of course Medicare denied, the PA ( haven’t seen the actual Dr. In over a year) refused to appeal, said there was no purpose if I wasn’t getting a procedure. Now he brought in a third party to conduct some sort of viascan 3d scan to check our hearts. When I wanted to opt out I was told by nurse it was mandatory as part of my care plan. Meanwhile, I’m suffering, can barely walk and they don’t care. Meds are held hostage. My primary just got MRI approved and I’m going to start looking to move on.

I've tried acupuncture, physical therapy, medications, chiropractic lots of other things for YEARS, and everything works except only temporarily. The best thing that has worked for me has been progressive exercises. But the problem is the recovery phase is very slow. For example, I've been doing them non-stop for the past six months, and while I have seen improvements, I'm pretty sure I'm gonna have these limitations for at least another six months or so. Limitations such as not being able to sit pain-free and not being able to sit for more than, let's say, 20 minutes at all. If I sit longer, then I'm gonna have to pay for it, and this is affecting my relationship. At this point I don't remember how being normal feels like any more. when the doctors see my MRI they treat me as if I'm normal and nothing is wrong with me and they don't take me seriously. one of the most even put me on antidepressant drugs saying that it might be mental but that didn't help either. Do you think if I take an injection now it would help? Or what else do you suggest?

A few important points to consider evident in this video for those paying attention to the details. 1. Deuk Spine Institute doesnt have complications from ESI procedures. Other doctors do have complications like making patients paralyzed. If i needed an ESI, I would go to a very reputable doctor and not just the one nearby for convenience. 2. No one else talks about ESI serious complications. People have a right and need to know. 3. Deuk Spine Institute never said "Dont get an ESI." Did we? 4. Deuk Spine Institute has the technology to cure back and neck pain. We use ESI to " bridge" a patient in pain until they can have a surgery to cure their pain. We dont use or sell ESI as a definitive treatment. Our patients know the truth, ESI is temporary relief at BEST. you are welcome for the knowledge imparted here.

Dude, there is usually more risk with a spine surgery versus a spinal injection. Most of the time injections should be attempted first unless the symptoms and exam findings are very severe. There are very skilled pain management specialists. The big money is with the spine and neurosurgeons, so that opinion can also be biased. I've seen many patients with failed spine surgeries. They are obviously worth the risk for some people but literally just today my patient who got an RFA today was asking why is she still having so much pain when she already had spine surgery. How frustrating it must be when you spend so much money, take time off work, have a painful surgery, go to all the PT, only to not feel any better? It happens too often.

The former pain management I went to pushed these epidural shots. This doctor kept telling me she was right there where she needed to be. I was begging her to stop it was hurting so bad. She said she was going to numb me better but I was out cold. When I came to my left arm was hurting like it did before I had surgery. I couldn’t feel my left leg and it was numb but burning all the time. It took 3 months before the pain went away with physical therapy. My leg still goes out from under me and I fall. This company went out of business from insurance fraud. I’ll never get one of these shots again. I still say to this day when she put me under she messed me up. When you’re hurting you’ll do almost anything to get out of pain. These epidural shots don’t last and they’re dangerous.

Sounds like a radiofrequency ablation could drastically help you, but being that you don't want a needle in your back, it's not possible. I was in pain for a long time. I woke up from the procedure pain free. It's minimally invasive, no cuts, and they don't stick the needle in your spine. They stick it in the nerves along the spine. That said, I'm just assuming based off your listed symptoms. You may or may not be a candidate for the procedure, but it can be a life saver.... Gave me my life back.
@kimberlymarie3651
@kimberlymarie3651 Жыл бұрын
@@JCJ34 radio ablation is still putting a needle in your spine. The FDA has never approved spinal epidurals in the spine because it's too dangerous yet doctors have been doing it for decades and the medicine injected speeds up bone loss and causes cartilage loss making the person a life long patient. The last one I got paralyzed my right leg and foot for 30 hours and had to be admitted to the hospital. Thank God it wasn't permanent, but others aren't so lucky. Why kill a nerve to block a pain signal to your brain ? All that does is you won't feel the pain while the pinched nerve continues to get damaged more until you start to lose important functions like bladder and bowl control , weakness in legs and paralysis if your nerve is in your lower back. But if the pinched nerve is higher up it will affect other functions . I've had about 10 spinal epidurals years ago and I now have severe osteoporosis and bones are to soft I can't have a fusion to correct my pinched nerves . A screw won't hold in a soft vertebra bone. I get compression vertebrae fractures from doing nothing because my bones are soft from all the spinal epidurals I've had. I will never get another needle in my back . I explained to my doctor I had my leg paralyzed for 30 hours from my last epidural and I won't get another one and the doctor still was pushing me to get another one by saying he'll send me to a good doctor !!! ummm no that's how I found the doctor that paralyzed my right leg for 30 hours.. I won't be bullied into getting another epidural. So, everyone having pinched nerves ; don't put off surgery to correct the problem. Have surgery to correct the pinched nerve while you still can because your bones are still strong !! If you put it off surgery with multiple spinal epidurals it will give that toxic medication time to destroy your cartilage and bones. Focus on correcting the pinched nerve , not cutting off the pain signal to your brain because the nerve continues to be damaged more .. My back pain experience has made me lose faith in doctors and the whole healthcare industry. The doctors are taking pain meds away from people who legitimately need it. It's all about the money. Doctors don't make money prescribing cheap pain meds , so they're taking pain meds away and pushing expensive procedures like spinal epidurals that damage your back making you a lifetime patient. Focus on correcting the pinched nerve , not destroy the nerve that sends pain signal. Pretty sure that nerve does more than just send a pain signal ?! I compare the doctors approach to back pain to being like this... If you're driving down the road and your "CHECK ENGINE SOON" warning light comes on in your car and you pull over and open the hood and pull out the wire to make the "Check Engine Soon" warning light to go out and you continue to keep driving the car !! Just ignoring the warning sign , but you know the damage is still there and the car is going to breakdown. Don't put off surgery to correct the pinched nerve. Correct it while you still can . Epidurals never helped my pain; they've only done damage
@JCJ34
@JCJ34 Жыл бұрын
@@kimberlymarie3651 ... The ablation is not in your spine, it's beside it. While I agree that it's not correcting the issue, my issue isn't one that can suddenly get worse without some traumatic event so it works for me. I feel no pain, get regular check-ups to keep an eye on things, and can now move and lift things without issue. For me, it's a win win. I'm not familiar with the spinal epidural, so I'll take your word for it. I go to the Spine Institute where they only focus on spinal issues, and my doctor does not want to cut on anyone unless it's a last resort, which I agree with. There are risks with any procedure, but my cousin has debilitating issues caused by back and neck surgeries. I'll go under the knife if I need to, but it isn't my first choice of things to try.

If you aren't an appropriate candidate for, or aren't to the level where they feel surgical intervention is needed yet (when the nerve pain outweighs the damage), this can make so much difference! I think this is a bit misleading. Surgery has it's own huge set of risks and the severity of the symptoms matters as well. To my mind it doesn't make a whole lot of sense to do surgery on a patient with mild symptoms when a properly administered injection might be effective. However, injections obviously need to be administered by a practitioner with the knowledge and skill to do it properly, with the correct medicine. Point is, sometimes surgery is the right treatment and sometimes it's not. The best policy is to get a 2nd and even a 3rd opinion before going forward with ANY treatment.

You can get that from any needle going into your body anywhere, even taking blood, the reason is likely to be bad hygiene practices.
