It has definitely been a while since I have done one of these videos in which I unbox equipment that we use on a daily basis in Interventional Radiology. The last video I did like this was over a year ago! Anyways, today I unboxed and explained everything there is to know about a hemodialysis catheter aka tunneled dialysis catheter aka Permcath aka vascath aka HD catheter. What makes this central venous catheter unique is that it is actually tunneled underneath the skin, which greatly reduces infection risk. This is a VERY common procedure we do in Interventional Radiology. We usually place a few of these EVERY single day.. Cheers for the Video clip! Excuse me for butting in, I would appreciate your opinion. Have you researched - Reyameron Linkify Blitzkrieg (erm, check it on google should be there)? It is an awesome one of a kind guide for learning how to avoid dialysis and boost your kidney health minus the normal expense. Ive heard some great things about it and my mate after a lifetime of fighting got astronomical success with it.

Winner of a video, I've been looking for "chronic kidney disease treatment medication" for a while now, and I think this has helped. My grandma was put on hemo dialysis a year ago and had a loop graft put it in her lower right arm but it got infected with mrsa after a month or two. They then put in a CVC on her neck and she’s been using it for the past 7 months. We just went in for surgery to get a AV Graft put in her upper left arm and after the surgery her hand became ice cold and numb so we took her to the ER. They admitted her and the next day the surgeon reached out to get my decision on the next step. He said it wouldn’t be a good idea to leave it as is and hope because if it gets worse when they use the graft it could lead to full loss of motor function in her hand. So the two options he gave me was one to remove the graft fully and continue using the CVC until her arm heals and talk about getting another one done. The other option was for him to go into the incision and try to bring it higher to another vein to maybe increase blood flow to the hands but he said he can’t really predict what will happen. It could cause the graft to stop working and it’ll have to be removed after anyway. So we went ahead with removing the graft completely and gonna wait and see what our next options are. My question is how long can someone have a CVC in for HD and what’s the main causes of failure besides infection? She’s had it for 7 months and we’re concerned if she can’t get another AV Graft in her arm what would we do if the CVC gets infected or something. She only weighs 95lbs and her veins are very small which is the reason they are doing grafts instead of fistulas

How do you identify if the catheter is only for dialysis or can be used to give meds or blood draws without asking anyone? They all look the same. Is it labeled VAS cath, which tells everyone that it’s only used for dialysis? How are the catheters for meds inserted either jugular or subclavian different (looks not purpose) from VAS or ASH cath?

for mine, the lumens are covered with guaze and taped off so that it is only used for the treatment. Blood draws are done with fresh pokes elsewhere. On the guaze is a label with the concentration of heparin, the amount, and date. The lumens themselves are stamped with the amount of heparin put in each one. a bold label to draw it out and not flush is also there. Its pretty strict. Mine is labeled as a CVC and used for plex

Thanks for your demonstration! I've been a Hemodialysis Technician for six years in both outpatient and inpatient modalities, and I access several of these every day. Let me tell you, sometimes these things are just the worst, even immediately out of surgery. Is there a technique you've seen surgeons employ that makes them perform better? Also, did you mean Superior Vena Cava, or do you actually run the guide wire straight through the right atrium? Are you performing consistent imaging to ensure placement, or does the J hook prevent perforation? I'm a year away from graduating to become an RN, but for some reason your explanation of this procedure made me want to go to Medical school instead to do IR.

Wow... I never realozed how far the tubing went in. I have been on dialysis 8 months now and definitly look forward to getting a transplant and having it removed! Better than the alternative, though... lol

@@Sixdays_aweek general surgeon when A patients has to start dialysis or the start Chemo they may need a dialysis tube in the stomach and Chest to get treatment and chemo patients need a port in the chest to get chemo.General surgeons work with the digestive system,transplants,Chest procedures,and organs

yeah a hemodialysis catheter is usually not permanent for this reason. eventually they will get an AV fistula (graft if needed) or peritoneal dialysis access.

yup, only peritoneal dialysis should be done by the patients. even tho the catheter is under the skin, infections are common because the ports are exposed. although it’s called a PERMAcath, it’s still meant for short term use.

Lol no. Sometimes a catheter tip can cause the heart to ‘skip a beat’ which the doctor should be made aware of

Hi Dr. Cellini, My question concerns the concentration of a substance within the dialysis catheter. Is it possible to change the concentration of glucose within the catheter, once it's been closed and without opening it?

If for instance, I've filled it with a solution containing 40g glucose/100 mL (40%) but I came to realise, I wish to have 20 % instead. Can I fix this without opening the catheter and in that case, how can I go about it?
