Dialysis How to Call the Provider Using SBAR

Calling the provider can cause anxiety and worry for a new or seasoned dialysis nurse. Sometimes we don’t know where to start so we end up all over the place when we call the provider.
A common tool used in many healthcare settings when calling the provider is SBAR. Using SBAR as a dialysis nurse will help you stay focused and ensure you have all the information when calling the provider.
Let’s go over the SBAR: And I picked a fun/happy call.
Situation: What is happening with the patient? What is the purpose of this call?
-Hi Dr. Sonja, this is Nurse Lindsey, I am the dialysis nurse caring for Patient A.
-I am calling because because they are 5 kg under their dry weight
Background: What is their pertinent medical history?
-This is their 3rd dialysis treatment at the clinic. They are AKI. They have a history of Hypertension and CHF.
Assessment: What is my assessment of the patient?
-The patient has not started dialysis yet. The patient states their urine output has increased dramatically and they can finally put on their regular shoes, they have been wearing their slipped because of their swelling. No complaints of shortness of breath, no edema, blood pressure is 110/54, pulse 80. Their last labs were drawn 2 days ago before dialysis, GFR 20, Creatinine 3, Potassium 4, phosphorus 5.
Recommendations: What do you think is happening?
-I think the patient is recovering kidney function. Should we hold dialysis today?
New orders:
Yes hold dialysis today. Draw Renal function panel on Friday STAT before dialysis. Call me with results before starting dialysis treatment.
