This is a great explanation! As a child of the 70's it was more about "eating 3 square meals/day". We weren't allowed to snack before meals (my parents wanted us to be hungry and eat our meals) unless we had veges or fruit. We also didn't have easy access to so many unhealthy options (Starbucks, Crumble Cookies, Nothing Bundt Cakes, etc). We were not deprived, my mother was a wonderful baker. Eating at McDonald's, Pizza Hut or Wendy's was an occasional treat.

And eating 3 times a day was not the norm 100+ years ago. It was 2 meals.

Over the years tried many various “diets” with temporary results, nothing sustained. Last three years, I finally found what works for me. Pay for plans, I think, is a waste of my money. I am glad I can now manage my weight. It’s natural food, control of sugar ( limit all possible) and control of carbs has worked for me. I have cut out most of fast foods and cut off time for eating every day. Have maintained and thankful for that. Just wish I had learned this 20 plus years earlier!

Dr. I followed your advice of cutting out processed food and I lost 26 lbs in 2 months. Regulating your hormones takes care of everything. And I have had no cravings and now I can exercise harder just because I can. Etc. I like your empirically based approach to medicine. Your covid and losartan video sold me on you. And then I watched the rest. You are a real doctor. I appreciate you. And if you are not a college professor you should be because you know how to teach very complex processes.

The sad thing about diets and loosing weight is they all can work, but I think the statistic is that 95% regain their weight plus some. Even those who have done the gastric bypass. I have fought my weight all my adult life. I was a thin and active until I had my first child. I only gained 32 pounds and only had 7 extra pounds to loose after her birth. Then life happened. I’ve tried Weight Watchers, Jenny Craig, Nutria Systems, and many more. I think most of us didn’t and don’t realize how much sugar and salt is in processed foods. Very frustrating.

I stayed on keto for 5 years. I don't crave empty calories in the form of refine carbs/processed foods at all in top of having high energy and no brain fog. HFCS in processed foods actually block your satiety hormones and you'll end up eating more. It's crazy

My dietician put ne on a high fiber low sugar diet 4 months ago. Im still on it! Ive lost 20 lbs thus far and my neck is back down to 17 1/2 inches again. I can't believe how good I feel. I weighed 300 lbs before the diet, Im 6' 6" i didn't look 300 lbs. My E.F. is 35% so getting the pounds off is critical. Im enjoying short bicycle rides daily to remodel my heart. Im glad that you clarified what a "diet" is. I love you're channel!

As a nurse moving out of the sick care system into functional medicine health coaching I’d like to tell my clients that everybody’s on a diet with a small “D”, the big “D” diet like Jenny Craig etc. have always have been epic failures, like Dean Ornish, and epic fail food FOOL😮

We need more discussion on vitamin toxicity too. Many diet programs are adding way too many vitamins. Isagenix had a recall last year.

Is it possible that food all types from legumes, vegetables, fruits, nuts, seeds, whole grains, fish, lean meats, etc are being modified and causing the body not being able to loose the weight? Modified genetic food, irradiated food, processed food and such as so are the real cause of people gaining and maintaining the weight . We are seeing people eating healthy diets and exercising and still overweight or obese.

Wouldn’t it seem that once you start these diabetic drugs to lose weight; you are on them for life? Also, what are the long term side effects?

Depends whether you heed advice. Some people can wean off them with strict keto, others can reduce them greatly by reducing carbs.

Doc, you know what else improves insulin sensitivity? Not eating beyond your caloric expenditure. I know that sounds crazy. Just stop with the mental static and think about it for a sec.

Hey you want a difficult topic for your next video? Try Lipoprotein a. 25% of people have Lpa. They say its genetic but sciece shows that a low carb diet does reduce it. So which lifestyle is the best for people with Lpa? Eating low carb and using statins?

I've been eating a whole food plant based diet for 5 years. Yes, I enjoy the food. Yes, I'm not hungry. Yes, get enough protein. And yes, I have blood work & a yearly complete physical with excellent results. By the way, look up the definition of vegan in the dictionary. It isn't the same as whole food plant based.

Am on a plant based diet (no sugar, no carbs and no fat. Lost 40 lbs. the last 6 months. Want to get it off and keep it off. Taking it off very slowly (2 lbs./week). Aiming for a permanent lifestyle change instead of a diet. Failed at everry diet I was previously on, because this is not a diet. Made changes gradually. I wish everybody good luck in their weight loss.

very good video... as for diet 'ratings' look at the criteria for how they rate diets.... and who gets to decide what criteria are important. If you have a researcher who is plant 'forward' - anti meat of course they will push the vegan diet...I think rather than put people on a diet, better to find people who have lost weight (evidence of such) and kept it off for more than two years, and research them. I don't eat lean chicken or turkey the skin is good collagen.

I don't diet. I eat good, nutritious, natural foods Nature provided. I eat when I need, when I am really hungry. I don't eat because foods are available, no matter they are good food. When I was young, we didn't know what mean snaking. We didn't have cake, m&m was expensive and out of reach, no fast foods, processed foods were for wealthy group and very restricted. The only option for us to supply our body was the main meals with real fresh made foods. Everyone was active, working and moving all day. I am 70, keep a healthy lifestyle eating right and exercise since my young age , untill now never experienced any major health issues, still walking hours daily and swimming 30 laps 5 days a week. My BMI is stable in the 18-19 range all my life without dieting. Good food and the right meals never make you fat, even you don't exercise.

Instead of “low sugar” you should say “low carb.” Carbs turn into glucose in your body anyway.

I don't think any of those diets will make your insulin go down, but i do agree that the diets will not work until your insulin levels drop

So you have to stay on these drugs for life instead of letting your body learn to eat healthy to naturally lower insulin levels? Ozempic is super dangerous. My friend got pretty sick and regained it after. I’m happy at 130 lbs but I never diet I just eat healthy when I’m hungry but it took many years to learn how to eat.

The thing is you can't over eat meat regularly. Processed foods with HFCS will BLOCK your satiety hormones and not tell your brain you're full, thus over eating.

He's also internal medicine and an intensivist. They're the ones who take care of patients who are admitted to the hospital, particularly the critical care patients. Some internal medicine physicians specialize in weight loss, but usually that's a space for dietician. It's not unheard of, though. Just like some may specialize in kidney disease, but not become a nephrologist. So much more info than you asked for! 😂

You just said that Keto is very low carb, but it does 50-100 grams/day. Earlier, you said low carb was 25 carbs/day. These numbers when you search are all over the map. I just want to know if 100-150/day grams is a low or fairly low carb diet

Keto is less than 25. My low carb diet is between 50 - 70. I believe under 100 is low carb. 150 is moderate.

@@lilmsgs A keto diet is your personal carb level that will put one into ketosis as measured by a blood ketone meter. It varies by individual depending on their exercise level, etc. The level of carbs could be up to 50 g carbs of NET carbs and still produce blood ketone readings of .3-.5 or higher. If you want to guarantee ketosis then eat no more than 20 TOTAL carbs per day. That should do it without having to buy a blood ketone meter. hth

I first started watching your channel during Covid and I learnt so much from you, But unfortunately lately all you speak now about these glp1 drugs. It’s like you are conditioning your followers to take these unproven drugs. I’m not accusing you of anything but it feels like you have an incentive to push these drugs.

Doc, Calorie restriction doesnt result in continued weight loss? How much do you want to lose? How about looking like an emanciated holocaust victim? Which brings me to, if you really believe that and that hormonal changes overide a deficit, show me one picture of an overweight WWII concentration prisoner. Just one, doc.

Of course he doesn't mean that. People trying to lose weight aren't on a 0-200 calorie diet for days at a time. If they are, they are anorexic and need help. I think he's referring to the weight loss plateau. You know, when you're losing weight at, say, 1200-1300 calories a day and then suddenly stop losing after 2-3 months. Every time this has happened to me, I've been on a highly processed, low calorie diet.

@@diyeanayeah, you know why you stop losing in a plateau? Because you've lost weight and therefore your expenditure matches your caloric input. That's homeostasis. It's not that the deficit model suddenly stops working due to hormonal changes and insulin insensitivity. It's that you're no longer in a deficit. In fact, if the insulin sensitivity were the prime catalyst for weight loss, you wouldn't hit plateaus because insulin sensitivity improves as you lose weight. That's how I know this doc is a hack.
