I would love to post this comment on all of your videos. I wanted to thank you so much for the effort you put into making this content. I truly feel your educating helped push my studying and understanding to the next level and made me pass my USMLE STEP 1 exam. I swear I accredit so much of my success to your videos. The week prior I watched every KZbin playlist you made. The day before the I downloaded all of your question bank walkthroughs and just walked around Target, pushing a shopping cart with my headphones on, listening to the entire playlist. Truly, thank you so very much for what you do and for the message you promote. I wish everyone the best during their studies. Stay positive and keep it simple.

Alcohol Withrawal (AW) has such a prevalance, but the Differential is astounding: Malignant Hyperthermia (MH), and Neurologic Malignant Syndrome (NMS)/ and surprisingly Seratonin Syndrome (SS)....

Great question. Uworld needs more like these that force you to always consider big picture rather than “can you remember this specific factoid?” NBME has so many of these big picture and common-sense oriented questions it’s crazy

malignant hyperthermia is a misnomer, it can happen with a normal temperature.

37.0 is normal temperature. How do you get a barely arousable patient to stand up for a BP ? This question points to anesthesia reaction . There is more in this question to point to NMS than alcohol withdrawal.

You are incorrect. Barely arouseable patients can have supine or prone blood pressures taken.. I'm sure you could imagine a comatose patient having their blood pressue cycled in an emergency situation, in fact this happens all the time. Alcohol withdrawal temperatures are generally normal or only mildly elevated, usually between 37.0 and 38.3.

@@DirtyMedicine The next to last line says "172/100 standing" At 5:01 you call 37.0 degrees "a low grade temperature" I found that wording confusing and wondered whether 37.0 was a typo.

how about delirium instead of delirium tremens?, because of the time after surgery was only 12 hs(48-72hs for delirium). of course B is only resonable answer but just asking.

Question: So if a patient is underattacked of serotonin Sx, first step is using BZD, then 2nd step is Cyproheptadine, right?

Acutely to get symptoms under control you give a BZD. Then if you still need, Cyproheptadine. You are correct.
