This video is about a condition in the jaw joint called "disc displacement with reduction" - a very fancy way to refer to the clicking and popping that many of us have inside of our jaw joints. In this video, I explain in detail the anatomy of the joint and exactly what is happening when clicking/popping - which is not "normal" or ideal - is present.
If you're unfamiliar with TMJD, common symptoms include: jaw joint noises - clicking, popping, crackling, snapping, episodes of the jaw locking, headaches, jaw pain, tooth pain, neck pain, shoulder pain, specific tooth pain, ear pain, vertigo, stuffy ears, and MORE!

I absolutely LOVE your teaching style! I am very particular about how I learn and am a teacher in some respects myself as a massage therapist.. You are well-paced in your speech speed and careful to explain terms (one of my pet peeves is medically-trained practitioners online who speak in anatomical terms ie as if they are speaking to a group of medical students...they must consider their audience!). I so appreciate your knowledge and how you transfer it to others needing to learn about their condition. Like mine. I have brought right jaw clicking and popping to the attention of every dentist I have ever seen as part of my history, altho I never had any pain or other symptoms in 50 years. (Maybe they felt if it ain't broke, don't fix it). Your video helps me understand this. I have had locked jaw on my right side in my late teens. I only experienced the excruciating pain for a full year recently when my left jaw locked closed for the first time. This was blamed on my stress and grinding but I do neither (I think it's just the most common cause). I felt my jaw was dislocated and what you described here is the closest to what I think is happening. So disc displacement without reduction resonates with me. Without proper assessment I was sent for physio which did very little but helped gain another finger width in the opening of my jaw (nice, but pain still) then I saw my chiropractor and got some relief. I tried acupuncture with no change and she suggested a sports mouth guard for $4 that can be molded and in 3 weeks or so, my pain left, thankfully! I feel like I have a cross bite now, my teeth don't meet in alignment, the points of the teeth meet causing pain on eating so I am gingerly eating soft foods and my diet is suffering. I was sent for an MRI and learned I have osteoarthritis in both jaw joints, 2/3 in the R jaw and 2 in the L jaw...rather serious I understand. Surgery is last on my list, so much can go wrong, there seems to be very little here on the west coast of Canada that offers anything in between. I feel a TMJ splint to correct my teeth position would be the way to go. Going for a consult with an oral surgeon...but I feel a surgeon will suggest surgery, as a chiropractor will suggest adjustments, as a massage therapist will suggest massage....In my opinion I think I got poor dental work on my lower left molar, a crown - my x-ray shows my right side molars are not touching when closed jaw. My dentist did not mention this as what's done is done perhaps, I'm not interested in laying blame, but getting a fix. I may well be calling your office to book in for treatment! My last MRI says I have disc displacement with reduction but I have no popping or clicking and a limited opening of 28mm (I used to have disc displacement without reduction)

Thank you so much for all your videos, I actually just finished watching your livestream which was excellent. I personally have been suffering for with this for the last 6 years as I believe to be a disk displacement possibly without reduction as I am starting to notice some of the more painful symptoms you are talking about as far as the retrodiskal tissue becoming a bigger part of the equation for me. I did go to see a famous T.M.J surgeon in St. Petersberg, Florida who does not recommend surgery as he feels that the centric relation that was done for me( after I realized my disk was displaced) actually made my situation worse. His recommendation is or was to try to move the lower jaw down and more forward, although the initial splint he made me was unhelpful (he's not a neuromuscular dentist). I'm really starting to feel a bit hopeless at this point because I really have some big plans I want to keep pursuing (becoming a pharmacist) but this chronic situation is giving me doubts about moving forward with this much daily pain. I apologize for the length of my comments. I guess what I wanted to ask you is if someone did have disk displacement with or possibly without reduction, what is the best case scenario for them? Would they always have laxity in their disk(s) and have random bouts of pain, or would could the disk ever stabilize somewhat allowing them to go to a phase 2 therapy?

@@pk-wi4mg Yes absolutely... I have just gotten started with a gneuromusculer dentist in Miami named Dr..Bushdid and physical and Myofunctional therapist Robert Murdocco. I am continuing to watch Dr. Mistry's videos and may setup a consult with her as well as I feel like I have learned so much about my problem and it may help to have an extra opinion as I continue on in my journey. My symptoms have included dislocated disk ( can be extremely painful), lockjaw, dizziness, stuffy ears, pain behind eyes especially when locked, sleepiness, anxiety ,( always feeling on edge because of the pain). However alot has changed within the last few months, actually mostly the last few weeks as I would say my pain and symptoms have greatly improved since wearing the orthotic (since last week) and doing the physical therapy for the last several months! I have set aside 1 hour a night to practice the myofunctional therapy on video actually. To be honest I haven't felt like I have had this amount of hope in a very long time! I have tried over 4 or 5 different orthotics ( including 1 from my dad who is a dentist) and nothing worked until I saw a gneuromusculer dentist. The orthotic has put my dislocated disks in a position where they can heal as it is now balanced with my muscles and teeth and hitting the retrodiskal far less. Everything Dr. Mistry has educated TMJ sufferers in her videos about has been spot on as far my story is concerned and I truly appreciate her knowledge and wisdom. I still have alot of work to do as far as making sure I can breath through my nose as I feel this could be a big challenge as I have been a mouth breather ( my whole life). I have a deviated septum. I have started doing some nasal irrigation and it has started to help alot. I will also will be working on improving my alignment and posture as this can improve breathing as well . Learning the correct tongue posture through myofunctional therapy is critical as far as making the orthotic work. Lastly, I will also need to get my tongue tie released in order to be in the correct position. I hope what I said was helpful, and I look forward to being more a part of this community. Dr. Mistry has really started something great, there is not enough information and care out there about this condition and how to get better from it. I am truly grateful for it. 🙏 😊

What treatment would you recommend when someone has the habit of only clenching and grinding with no pain and locking of TMJ. Can night guard work for those?

No an nightguard will not help with locking.

Well explained !!! This video is so informative. I learn something new from u all the time lol. I never really understood what that whole with/without reduction meant. Ur videos are perfect. It's very educational and as patients helps us understand whats going on. I do think they could have came up with better terminology tho, "reduction" doesn't intuitively make sense. But u explained it so well and simply. I'm sure that's helped many others as well. Also, can MRI help to distinguish which one a patient has ?

Thank you so much for your kind words. These videos take a lot of time so I really appreciate everything you said. Yes, an MRI is the best imaging for disc position and condition. It’s important to remember that and MRI is a snapshot in time and if the disc position is changing over time, and MRI will only tell you the position at the time the MRI was taken.

@@jakec5618 well the MRI images are taken in an open and closed mouth position and they’re great. But what if the disc in your joint is in a different position the next day? Or the next week?

Is there any kind of noise that is normal when opening and closing the jaw? I don't hear clicking or popping but I can hear some kind of faint noise in the joint (or maybe from the ear) when in a quite room.

Hi Priya, What a wonderful impact you have on people's lives! Your videos are excellent. Thank you so much for creating them. If someone goes through orthotic treatment for this and it fails, what further options do they have? Do they have no choice but to live with this and all of its unpredictability (ie episodes of closed lock/without reductions, continued spasm) or take a break and try treatment again? Can one even try treatment again as I have read that too much time using an orthotic designed to relax the muscles can cause permanent changes in the bite thereby causing more problems. Also, you noted on another video that you utilize machinery to help determine where the muscles are most comfortable and then design the orthotic to position accordingly. I am seeing someone that has no machine and the orthotic has been a kind of trial and error in a way. Your feedback would be greatly appreciated. Thank you.

You can try treatment again. One thing to keep in mind is that not all orthotics are created equally. Additionally it's not "just" the orthotics that bring about success. It's the rest of the treatment too - myofascial release, jaw manipulation, and craniosacral therapy along with frequent orthotic adjustments.

It can help! Most oral surgeons recommend trying 6 months of conservative treatment first.

Mine is starting to crunch now, it user to crack or pop. I think it's called crepitus, it's getting worse, NHS are saying they can't operate on it. It's really worrying me.

Unfortunately you cannot reverse crepitus but you can prevent things from getting worse with an orthotic that decompresses the jaw joints.

Everytime i open my mouth its hurts and my jaw locks in a bad way also my jaw is stiff cant barely move it from side to side and the surrounding tissues are all swollen ive had this problem for nearly 2 years now ive noticed my tongue dont fit my mouth properly ever since ive had this issue whats the best choice to fix this problem its ruining my self esteem and health.

The only solution I know for ear pressure reduction is a well made neuromuscular TMJ orthotic, which works 80% of the time to reduce or eliminate ear pressure and 90% of the time to eliminate ear pain.

Mine used to click and pop and now as the time has past the pain has increased but the pop is no more, it has changed to a gristly, grinding sound when opening and closing 🤷‍♂️

Mine too! It all started as an ear infection! Took two different rounds of antibiotics over two months. Inching in the ear stopped but the clicking and popping has added a bone on bone painful sound in my ear. It hurts in my jaw just inside my ear just to open my mouth and chew! It’s gotten really bad and serious but my doctors have blown it off as a bad ear infection!

What is a tmd issue that only clicks sometimes when I smile wild and I can also open my mouth really wide but the everyday pain is incredible and goes from behind left eye down left side of face (clearly affects the trigeminal nerve branches) into my neck and my left arm into fingers, a dead half numb type feeling. It also when bad can make me feel nauseated and dizzy in just half my head. When I go to the dentist they say jaw on opening and closing is fine and because I can open wide they don’t offer any help. I know it’s a jaw issue as it was helped by a nightguard for a while until I had a filling and it’s flared up again day after dentist, I’ve had 10yrs plus of this pain with about 2 years relief.

Your pain sounds mostly muscular in nature rather than being associated directly with any problem inside the joint.

wonderful video and thank you as always! so i’ve been sitting in this state i think for a few weeks, just afraid to open my mouth again all the way and eating less solid foods. do you recommend just trying to open the mouth all th way gently and seeing if it’ll let me? i just felt it crack when i yawned once and got scared of it happening again.

if the disk moves out when your jaw closes, what does that mean? does that mean it is progressing worse to disc displacement without reduction??

The disc is already out when your teeth are together. It comes back into alignment when opening and out of alignment when closing.

Can braces cause disk displacement with reduction? Im going through braces. Now I got ear pain. TMJ radiogram shoes disk displacement with reduction. Is there a treatment for this ? Im typing this message with so much pain in my jaw. :(

Sorry to hear that! There is a relationship between your bite and the jaw joints and muscles. If any of those factors are altered, it can affect the others

my clicking fluctuates :/... Sometimes its a very tiny click but later during the day it's a bigger click that feels like my jaw joint moves to the side

My jaw only clicks on the right, and only when I move my lower jaw to the right a little bit. Little pain and tension in my jaw on the right, but no real pain. It doesn't click when I open and close normally. However, if I put my index finger in my right ear all the way in, and open and close normally, I feel/hear the clicking. My jaw doesn't click on opening. It has never locked. It only clicks on closing, when my lower jaw moves a bit to the right. I can move my jaw to the right and make it click click click. Each movement to the right produces a click. If I open my mouth wide and move the lower jaw to the right in that position, it doesn't click. What could this mean? Thank you

Hi Dr. Can we get click sound whil opening and closing after its entered into ddwoR also? I'm into ddwor frm 4months and when dr here pressed with his fingers on tmj forcefully and asked me to open and close my mouth i heard click sound on open and close. But when I open and close on my own, i don't hear the click.

Hi! I'd recommend seeing and ENT to check for enlarged tonsils and/or adenoids. Once airway is clear, Myofunctional Therapy! Good luck!

As a massage therapist I would say that yes, scar tissue can cause restriction. The tissue can be broken down tho to naturally repair itself, it can be very painful tho@@rainbow20112011

When did the trauma occur? Definitely a good idea to see your primary care physician and follow their instructions/referrals. A TMJ dentist may be able to help with jaw popping.

good question! I have had 3 hits to my temporal region on my left side, experienced 2 severe episodes of vertigo and have tinnitus, I think both as a result...thinking of my L jaw locking as a result of the hits, same way you are. Put out to pasture on the concussions tho, nobody can offer anything, only say how they can't do brain surgery ( no kidding! And nor would I opt for that - but there has to be something in-between besides simply monitoring....anyone who has brain trauma is aware of their limitations, we don't need to be informed as tho we wouldn't know light and noise is too much...) Wish you well

I m not able to open full mouth without click, but if i move my jaw toward a side a smaller click occurs and then i can open full mouth without click. And when i closed mouth for chewing food it again clicks.What may be the case..?

It could be loose or strained ligaments, a tight muscle (the lateral pterygoid), the way the teeth come together, poor posture - lots of factors

Are you able to treat cases where the disc itself is torn, perforated, or otherwise damaged? I've heard a 'pseudodisc' can form which is almost as good as the original disc. Is it possible that if you decompress and stabilize the joint longterm it allows that pseudodisc to take over and effectively 'cure' the patient?

Hi! great question. if the disc is torn or perforated, it's very hard to reduce the disc. In that case, it's still helpful to have an orthotic to help decompress the joint and the expectation would be that a pseudodisc forms over time.

Ma'am, I have been having clicking and popping for 4 years now, without any pain. A specialist here recommended orthodontic treatment. But i feel my muscles have become tense and click has increased post starting the orthodontic treatment. Another concern that I have is that, since the past 3 years my face has become abnormally gaunt, with almost no fat. I have been gaining weight on the remainder of my body, by face is continually losing it. The situation is such that it looks as if the body, and face belong to different people. Could this be related to tmj?

i love your videos so much! i sadly live in germany and cant drive the way to visit you :) i visited my TMJ Dentist today. she diagnosed that my Disc is displaced but in a "rare" sidewards way. I can open normal the mouth, but when i put my lower jaw to the left side my right disc is clicking. She even told me that this is the reason why my tmj is inflammation on the right side. Is it even possible that the disc can move siedwards and goes sideway out of his position? Would really love to hear from you :) kep doing your great work!

Hello! I hear the 1st click only when the jaw is half open and the 2nd when it is completely closed. If I move my open mouth, I hear scraping or crunching. I have no pain, only the clicking that I hear if I put my fingers in my ear. I wonder in which video you explain such a situation and is there any animation for that case? 👋🏼😃
