How local NHS, adult social care and voluntary sector services are working together to support people home from hospital in Brighton and Hove via 'Discharge 2 Assess' - a different approach to the conventional hospital discharge process.
This film follows patient Diane's journey as she is discharged home from Chichester Ward at the Royal Sussex County Hospital in Brighton and shows the support she receives during the Discharge 2 Assess process.
