My fave DMAIC handbook that has all the forms and tollgate checklists and real list examples to help you through all your DMAIC projects is: "The Six Sigma Way Team Fieldbook - An Implementation Guide for Process Improvement Teams" by Peter Pande, Robert Neuman and Roland Cavanagh. Highly Recommended.
