Phentytoin is also called as diphenyl hydantoin...so the side effects can be remembered via cnemonic I.e HYDANTOIN..H:hyperplasia of gums(most common),hirsutism ,Y:lYmphadenopathy,D:decreased vit D activity(so ca decreases i.e hypocalcemia),Diplopia,A:Ataxia,N:Nystagmus,T:tremor,teratogenicity(nural tube defect),thrombocytopenia,O:osteomalacia(bcz of decrease vit D),I:inducer of enzymes ,so enzymes which metabolise vit k are induced leading to decrease vit k ),N:No folic acid absorption (leading to megaloblastic anemia).. Note:vit k is reqd for formation of bone matrix,since vit k and vit d are decreased ,so it leads to osteomalacia...note:phenytoin also crosses placenta ,so it can lead to decreased vit k in infant ,thus can precipitate hemorrhagic disease of newborn since vit k is reqd for formation of c.f 2;7;9;10..so to prevent HDN,vit k should be given to pregnant mother in 3rd trimester and to baby just after birth ..NAD I.e nystagmus,ataxia,diplopia are cns related side effects out of which diplopia and ataxia are undesirable,so if patient develops ataxia ,diplopia,then TDM is reqd and dose should be decreased within therapeutic range..
