Okay, from the start, take the door to your left (East) up the stairs. Immediately on the other side of the door, on the left is a throw switch. That switch raises the 3 columns in the nukage on your left. The first one is where you teleport to, the middle one has a Bezerk pack (I never understood why that counts as an item), and the last one has a Baron. That's your missing Item and Kill.

ooh, i had to go back and check that :D didn't notice that switch at all - thanks! re: Berserk - i kind of think it's because it's a power-up that can always be picked up (and the problem is, CAM is a power-up that can only be picked up once (which might be because it was meant there's no more than one per map)).. whereas non-item power-ups can only be picked up when one actually needs them. but there may be more exceptions to this (non-)rule - these kinds of rules in Doom seem advisory at best :D
