After several desperate attempts to keep his old mentor alive, Dr Halstead realises he needs to let her pass to relieve her pain.

Everyone complaining about Dr Manning saying "I'll break her ribs": come on guys. Her character is a doctor and a good one at that, it's not like she doesn't know that breaking ribs while doing CPR is fine. The whole point of showing her hesitation, and April's hesitation too (if you notice), is to present the argument that was the primary focus of this clip: does quality of life matter more than life itself? Especially for someone that old, is it really worth it? Tough question with no correct answer.

NO ONE refuses to do compressions because they'll break a rib. That's literally one of the FIRST things they tell you when you learn how to do CPR; Even if you break ribs, keep doing compressions. If you are resorting to CPR, they are already on deaths door, a few broken ribs doesn't matter.

It's just an error in the script and they didn't run it by whoever they consult on the medical side of things. I just find it particularly annoying because it's a really simple thing and there are so many people that know how to do CPR properly and breaking ribs is not a valid reason to not do CPR.

In this case though, there appears to be no next of kin available to make that decision so the doctors have to treat her like she wants to be saved regardless of the means they have to go to to keep her alive.

I think it was more the patient would rather die peacefully than have more pain and suffer the whole time. She would have died anyway and having a machine expanded your diaphragm when you have broken ribs would hurt like hell. The doctor was thinking about what the patient wanted not what the grieving friend was insisting on. This is why doctors aren’t allowed to treat family. Keeping someone alive becomes their first priority, even if it isn’t in the patients best interest.

QoL isn't something you discuss while your patient is literally flatlining in the same room. You resuscitate, unless given a DNR order. That is a decision only given to the patient or anyone they deem fit to decide in their stead.

To anyone who ever has to do CPR on somebody one day (I hope you won’t have to), breaking a rib(s) is completely normal. It’s recommended push down 5cm (2 inches) when doing compressions, plus most times it will be elderly people with more fragile bones. So don’t hold back because of possibly breaking ribs; after hopefully surviving a heart attack, a broken rib should be the least of their concerns.

The reason why nat had issues with cpr is because if she does it right, it'll break her patient's ribs. Who knows how many they would have broken since the patient is so fragile. Patient: Intubated (meaning the machine was doing the breathing for her) Kidney Failure (Leads to many different complications including heart conditions, medication complications, blood pH fluctuations) broken Ribs ( in loads of pain most likely cannot do surgery since the patient is so weak, she wont be able to handle it, most of the time you dont do surgery you just have to let it heal, with the patient's age and physical state it will take a very very long time to become healthy, by that time she will pass away from other health conditions) Shes bed ridden (pressure injury leading to skin necrosis, muscle atrophy (muscle degeneration which can happen as fast as 1 week bed bound)) Of course quality of life will be a concern. Especially in palliative care, quality of life is a big concern. Thus all the pain medications and the rest. People say they want to live but other people just want rest after so much fighting. Sometimes its just ignorance, naivety and the lack of knowledge about how the body works.

That's not what that sentence means. Some doctors treat a disease, but without really considering the well being of the patient. I've personally seen this in operation rooms, where the surgeon does an amazing job, but the rest of the body comes out a wreck because of it. And then there are doctors that put the patients mental state and well being before anything else, and that's what the sentence means.

I think the point of Dr. Manning's comment was to the futility of CPR in the patient's situation. Although I agree, in this case, advanced directives or the subsequent decision maker would have to clarify what would occur in such a situation, however, I think Dr. Manning's comment is to contract against Dr. Halsted's actions. It's a hard choice between but sometimes ensuring a peaceful death is more kind to the patient than life while subject to various treatments that do not cure and only prolong the suffering- for instance, CPR and the subsequent pain from broken bones. I think that's why Dr. Manning made that comment.

Tiny Butterfly Yes, she passed away after they extubated her! If they put her on dialysis, she still wouldn’t have survived on her own! She would be bed ridden and she would have had to have a tracheostomy as she wouldn’t be able to breathe on her own! They treated the patient and not the disease! They considered her quality of life and knew that if she survived, she wouldn’t have wanted to live like that!

That lady who they were treating helped her not to give up! She meant a lot of Dr Halstead and that nurse (I don’t know her name) They give guidance and that kind of thing!
