I feel like fundamentally the whole addiction treatment in this country needs to change. I’m disabled with severe chronic pain with no addiction history or issues thank god but I’ve done a lot of research on the topic because doctors always bring up addiction when you need pain control. A couple months ago I thought we were 20-30 years behind Europe wirh addiction treatment. Mostly with lack of access to good addiction care and stigma of MAT and an over focus on rehab. After talking to some addicts in Europe I realize we’re more like 50 years behind the rest of the developed world when it comes to treating opiate addiction. In Europe they regularly give high dosages of morphine to addicts I think in long term Recovery the person I talked to was 5+ years sober. I don’t think they give other opiates like oxycodone was specifically banned for reasons. I mean heroin is a pro drug to morphine it makes sense. All opiates are similar in mechanism of action. Also talking with these addicts from Europe it was clear their was much more options available and more willingness by the system to try and adjust to the addict. Between morphine, traditional Mat which is a lot more free to pharmacy dispensed methadone where you skip the clinic to even experiments now being done of daily or multiple times a day controlled amounts of pure heroin being dispensed. Europe has an opiate crisis but compared to us it’s nothing per capita and it’s not because they don’t prescribe as much it’s because they have a better caring society but it’s that their addiction treatment is so much more advanced it’s like comparing how we treated aids in the 1980s to now. I hope to god the system changes. I’m sick and tired of people dying for no reason. If pains me to hear of so many dead for no reason. I also know as a chronic pain patient my life is gonna get harder and harder as the government cracks down more and more on legitimate prescriptions leaving more addicts to die and never addressing the root cause and making important fundamental changes to our medical addiction treatment system. I also feel like one day may come where I can no longer live on due to a crackdown in pain prescriptions and I refuse to go to the black market I’d rather die than do that. The crackdown on prescriptions is so bad. I mean maybe someone things I deserve to suffer in pain so excruciating it ranks higher than most types of cancer but it’s not just me. End of life patients, cancer patients who are middle aged can’t get enough pain treatment. People are going now to palliative care or methadone clinics pretending to be addicts to get pain treatment clogging up those systems of care more. And it’s not like we have enough pain management doctors much less enough willing to put patients first and not the dea and their license first and do their job properly not that I blame them it’s their passion and livelihood. This is also causing fewer and fewer physicians to go into pain management and a lot hear refuse to write opiates which they are the only doctors that can do so for chronic pain and those that do pretty much never do so enough to adequately treat pain.

Yeah you have put some great points out there. Here in the UK we have easy access to Methadone and Subutex (Buprenorphine) and we only have to collect it once a week from our local pharmacy instead of travelling miles away every day to collect it also we've had needle exchange here in the UK since the 80's. Some puts of the UK are trialing Diamorphine to addicts like they do in Switzerland and Norway.Also here in the UK the gear ain't tainted with Fentynal and it never will because it just won't take off like Crystal Meth never has don't get me wrong I not saying that's good but Heroin is a lot safer than Fentynal and Heroin and Crack are available anywhere in the UK even in little village's its rife because the prices for these drugs are in my area £5 for a point but you can buy a teenth (1.7g) for £60 it's just crazy how many people do it now compared to say a decade ago. I think maybe that is why people here are more caring because Addiction is seen as an illness but maybe as well its in our culture to look out for one another I mean the Heroin addict here will never inject drugs on the street in front of people then throw the used needle in the street like in American cities which I've seen on these documentaries (San Francisco/Portland take note) they dispose of them in and small portable plastic sharps bin. 🇬🇧

I have been on Suboxone and Subutex for years and it's caused a lot of health problems I think they are going to realize this pretty soon as far as a long study I know that methadone has been around for a long time it was invented by a German doctor during the war. I'm wondering if you can tell me if methadone has as many health risks I'm considering going back on it because I tried it for a bit and I do remember it helped me with the depression and withdrawal from people?

I was a trial patient for suboxone. When it came out, they called it the new miracle drug that doesn't make you sick. Lmao. It never worked. I wasn't feeling better, I was just sick the whole time. Methadone (and valium) saved my life. Keeps me functioning, not using illicit drugs, haven't gone backwards in 18 years. I'm a lifer, and I realize it. My new Dr wants to take me off of valium not because of anything I did. Because she doesn't agree that it's a psych med.. I don't know what to do to make her realize I'm far better off medicated. I haven't used or been in any trouble since 2011. I'm honestly afraid of losing my mind and relapse. I haven't had a craving in years. Any advice doc?

I get my weekly 70ml and I have one urine test since Covid restrictions were lifted! I mean the chemist is in walking distance but I'd still be pissed off if I had to walk there every day or even 3 times a week. America tho must be a nightmare especially if you live in the arse end of nowhere and you have to travel miles every day just for your dayly dose it makes me wonder if Americans have less of a chance of abstinence than people here?Here in the UK we are a lot more lucky than our American cousins because access to Methadone and Subutex is so much easier to get access to and waiting time from being seen and being put on a script takes a few days. Also the brown here ain't tainted with Fentynal either and I don't think it ever will just like Crystal Meth hasn't taken off here. 🇬🇧

I have been taking methadone for 10 yrs and have been going only 2 times per month but I’m tired of taking it so now i switched to buprenorphine but I asked my doctor to prescribe me subutex which does not contain naloxone and can be taken as early as 6 hours after last use of fast acting opioids and 24 hours after last dose of methadone which removes the negative aspect of having to go into withdrawal for 2 to 3 days before taking Suboxone. I think every patient when transitioning from heroin or just opioids should be given Subutex which is buprenorphine without the drug naloxone and after 3 days on subutex should the patient be given suboxone. I don’t know why more clinics don’t do this takes away the much dreaded withdrawal symptoms that people don’t want to have to go through while waiting to take the first dose of suboxone
@perpetualmotion357
@perpetualmotion357 8 ай бұрын
It's the Bupenorphine that causes the prec wd. Naloxone is only put in Suboxone so people don't try and shoot it, which will activate it at that point.
@mvmusic8467
@mvmusic8467 6 ай бұрын
You have that completely wrong, its not the naloxone that causes precipitated withdrawal if taken to early after your final dose of an Opioid its the Buprenorphine that kicks the opioids off the receptors due to its insanely high binding affinity at the mu receptor. You dont actually absorb any clinically significant amount of naloxone from sublingual or swallowing, its there purely to stop you from iv’ing it.

I was referred by my pain management doctor for methadone. It has worked better than any other pain med they had given me (you name it). I had a pretty bad back surgery that has left me with chronic pain, I'm only 38. I'm still able to take my anxiety meds and antidepressants as with suboxone they can be contraindicated. These meds are very individualized. I don't have any addiction issues though, so this was a great option for me. I hope everyone finds what works for them whether it's for pain or otherwise. I should add that my methadone is over 400.00 a month out of pocket because my insurance does not cover it! 😪

Jesus the overwhelming majority of people that start methadone or buprenorphine never come off it , because the half is so extremely long and withdrawals are so painful that you become hooked for life. Heroin, morphine oxycontin etc.. do not hold a candle to these two drugs in how long it takes to leave the body completely, and the severity of the withdrawal symptoms . Know that once you start it can take months upon months to years to fully withdraw. there is no such thing as 1 week to 1 month of physical withdrawals to come off these medications. Simply go to any clinic and ask any addict 99.999% of them all agree. Doctors always downplay how terrible the dependency cycle to these two drugs are.

@DrBAddictionRecovery

I think what is being missed here is the risk/benefit discussion of 1 choice vs the other (ie heroin vs Suboxone). I do agree that many people need to be on it very long term and the belief or demand that one needs to come off a medication asap is a product of the stigma of addiction, addicts etc. Have you ever heard of any one demanding to come off their BP meds?? Just something to think about. Really hit me when I had a 40 y/o female prostitute, 20 plus year IV user, prison time, infections all over her body and an infected hip yell at me when I suggested Subs and she informed me that I could shove the sister of liquid handcuffs up my you know what. Although these drugs are not perfect and I am no fan of big pharma I feel there is an unreasonable demand and suspicion of these medications that are unjustified. Compared to chemo and radiation these drugs are bubble gum in terms of side effects. Yet cancer patients have a very different attitude. Again, Stigma and shifting reality lens and expectations. Thank you.

@@DrBAddictionRecovery thank you so much for your reply doctor. I should have mentioned that it my discussion. My dose is very different than that of someone taking it for opiate addiction. Although my body was definitely dependent on the meds was I prescribed prior to methadone. I still experience the stigma unfortunately when people ask me what I take for pain. The benefits definitely outweigh the risks for me. I think you are an amazing doctor! I wish there were more of you in my area.

Great content doctor B! I have to agree, the system of methadone clinics in the US seems incredibly archaic and should be updated for sure. In Denmark and most of Europe as far as I know the situation is more or less as you've described in the case of France. I've been on methadone for 15 years now and it's not only saved my life (which it truly did - no exaggeration) but made it possible for me to rebuild it. Of course, with a lot of work on my part and tons of help by incredibly dedicated professionals I'll forever remain grateful to! You remind me of them, Dr. B. Anyhow, I really wanted to explain the system over here. I get methadone at my doctor's office, not at a methadone clinic. The place and the expert primarily responsible for my therapy however, is a psychiatric clinic for addiction and my psychiatrist, one of about a dozen of them working there - all addiction specialists. Currently I need to go to the clinic every 4 months for face to face talk and control, otherwise I can call anytime, as well as show up more frequently if I feel I need to. In the beginning of course, and I mean for years, I had to be there much more frequently. Over the years we've developed trust. I pick up the take home therapy at my primary care physician's office once a week, unless I need to travel for a longer period. Otherwise I technically could be picking it up every other week or about twice a month but a few years ago, when she suggested a change I actually asked my psychiatrist if she could leave me on a once a week schedule as it had been the case for many years prior at that point. I felt I still needed that structure I'd gotten used to and which worked for me. The switch to picking the therapy up twice as rare as before seemed too daunting to me and so she kept me on the old schedule. If I need to travel, I may get the take aways for up to a month. At any rate, this system does seem much more humane ans ultimately more effective.

I'm from the U.K. & I'm on methadone (65ml) & it's so different over here! 1) We don't have a methadone clinic where everyone is given their meds. We get given prescriptions where we choose which chemist we will use (local to where you live) the script lasts 2 weeks & it can be anything from daily pick up to once a week, (ie take home a week's worth of meths) you have a drug/key worker who you see every now & again. Plus because of the NHS it's free. 👍. IF you need works or narcan/naltraxone you can get those too. Either from a chemist or drug worker. A few people are getting the injection now, which i m o sounds great! They are doing really well!

My sister was killed by her methadone clinic and my parents. The clinic gave her seven days of liquid all in one sitting - she was never a street drug or Rx-opiate user but she could talk anyone into literally anything. At home, she got so angry when my parents called 911, they decided not to call 911 that time. It was a “right to die” and a “we can’t take it anymore” situation. It helps to be White in a nice suburb, right? Or would a Black family get away with it, too?

If possible talk to other people who have been to different clinics because they're not all the same. Some are very professional and really care about your well being, but some are run like the department of prisons. I had a counselor who was a nasty off duty prison guard and about as useless as something foul smelling you might get stuck on your shoe. Another clinic was extremely professional and eased me right through things with no problems at all. Be careful, patient and don't expect overnight results.

I think it’s regressive even that we only have two options for OUD. in the treatment of chronic pain with opioids patients have access to a diverse array of chemicals and that’s obviously because different things work better for different people. Everyone’s chronic pain is individual and so too is everyone’s biology. This is obviously also true of OUD patients clearly more options would mean more patient retention wich would mean less death. There are studies wich objectively prove that some people do better on different chemicals but I don’t feel I even need to invoke them it’s just too intuitively obvious.

At least he didn't relapse on fentanyl, oxy, or heroin. Has he been off all opiates for 1-2 years? Because if someone with no opioid tolerance takes Suboxone, they will be completely floored. Maybe your boyfriend was having cravings, and needed to get back on Suboxone which is entirely appropriate. It is harm reduction after all.

Part of me is of the opinion that because I’m on Medicare, I’m treated differently I’ve seen four doctors, my GP, the orthopedic doctor, emergency room doctor, not one doctor laid hands on me check my collarbone and try to move my arm to see how they can move or function. I really have no function in my left arm anymore, it hurts too much cause a huge bump in the middle of my collarbone. Is that how doctors treat patients now because of Covid are they talk or is it because I have Medicare that one doctor has even in a Physical I got no one doctor‘s check my glands done anything they just do blood tests check my blood pressure which because of the pain is a consistent 200 and something Over 170 or 60 when the pain was controlled it was almost normal but they’re not worried about that. They’d rather have a stroke out then treat me treat the pain why is this how dark doctors talk to treat patients in medical school now not to lay a hand on him of the three doctors I saw I thought it was four but three I timed it 12 minutes for three doctors, want to ask me what the problem was same doctor to give me the test results and say goodbye and I said doctor my blood pressure is kind of high and I need a new GP know there’s plenty down the road as he’s walking away find one he said

Why would I go back to the emergency room? They know what my blood pressure is do you think they gave me any blood pressure medication? No they did. What makes you think going to emergency room is gonna be any different well they say yeah, no we don’t want to be liable if you should have a stroke instead of treating the stroke or the potential stroke believe in blood pressure medication they won’t even prescribe that and I do wonder is it because I’m on Medica

God bless him, they’re not worried about that. Either attorny says they are so greedy unless it’s a slam dunk case they don’t take it that’s why they say we want all our cases because there’s no doubt instead of advocating for someone who is being abused every day of his life, I have children, one new grandchild, who I don’t see my families destroyed, and I feel terrible. Doc, I work as a counselor at a Methadone clinic. Despite the ‘positives’ of the methadone clinics you mention here… With the ease and less regulated ability to obtain Suboxone, do you see the future of the Methadone clinics being phased out in the near future; and now you mention in Europe you can get methadone at the Doctors office. If that works its way here, we’re out of business, no? 😯

I was on Subs a while ago. The methadone route was the only one that worked for me with the Fentanyl now on the streets. My clinic has said there's actually been an influx in patients because of this. Unless regular heroin comes back and Fentanyl is taken off the streets (I don't see this happening) I very much doubt that there won't be as much of a demand for Methadone. I wish it was regulated differently though.

My experience; pure buprenorphine,(temgesic 0.2 mg .or subutex 8mg buprenorphine alone) Is that I feel a pleasant high using a quarter of a tablet,(2mg)after one month abstinence of all medecine.(terrible cold turkey I admitt!) As an old 69 polytoxicomane, I assure you that they add naloxone to deprive us of a pleasant high,totally absent in suboxone. Tell me what you think,Thank you dr B.🙏

You were only taking 10mg oxy a day, and they gave you 4mg/1 Sub? That is way overkill, for a 10mg a day use. I would just get off the oxy and feel a little uncomfortable for maybe 2-3 days than get on Subs, 10mg is nothing, you may feel like you have a slight cold at most. Trying to get off the Sub is a whole different beast, you may actually get sick off the the Sub as that is a large amount for such small Oxy use.

If you do 12mg of Sub, it will kick your ass, that is way overkill. People who shoot up 2 grams of Heroin a day, don't need 12 MG of Sub. Do yourself a favor and steer clear of Subs for such small Oxy use. That's like switching to Methadone or smoking Fent to kick Oxy, it makes no sense.

@@hds8182 Is there a reason you are getting off the Oxy, are they not wanting to prescribe it anymore? If after years of Oxy use you are good at 10MG a day you clearly are using it properly, so why get off it? Is it still working for your hip pain? I know there is a huge stigma now about pain meds, but clearly you are using them as prescribed and are not an addict. Getting of Oxy especially 10MG is physically a breeze(2-3 days of discomfort at most). Try getting off 12MG of Subs, especially if you are on them for an extended period of time. You are talking weeks or months of misery, Physical and mental pain. That's why many people have been stuck on Subs for 5, 10, 15 years. It is highly addictive, and it has a long ass half life. Look Subs have their place, they can be life savers for hardcore users, but for 10mg Oxy, I would truly advise you to stay away from them, you are trading a paper cut on your finger for a slit wrist.

Suboxone is just overall a better drug in terms of what's best for the addict. The longer time it has in the body without increasing dosages and having the associated side effects and the fact that it still works as a blocker and helps with cravings makes it superior in my opinion to Methadone. This is because if the person chooses to get off Suboxone they can simply taper down without having to worry about the withdrawl as much because it is only a partial agonist. I also believe because it's a partial agonist that it doesn't get you as high as a full agonist and therefore the associated side effects with those full agonists. I also believe it keeps people who are addicts seeking that "nod" they got from using heroin, which is something you can certaintly achieve on Methadone. For me personally I have been clean off Heroin and Meth Intravenous use for 5 years and Suboxone and medical marijuana has helped me curb all my cravings even on my worst days.
