Great video, as usual🙂 One question: in your experience, what is the role of mr arthrography for this type of the lesions, often being "hidden", as you said, on standard MR examination? So, in the case of the ACL rupture, no evident ramp lesion, and positive clinical sign of menisceal lesion, should we call for mr arthrography? We know that a lot of the ACL tears will not be active surgically repaired, but if we have (mr arthrogaphy?) the proof that we have associated ramp lesion, that could be the "game changer", right? Another thing that crossed my mind right now, writting "hidden"..you said that (roughly) every 4.-th ACL tear is accompanied with the ramp lesion..did you mean cca 25% MR diagnosed or intraoperative diagnosed associated ramp lesions? Thank you. And..can you cover in some future episodes the role of mr arthrography in general, where/when/why...pittfalls etc.? Thank you, again🙂

thanks petar for the suggestions. i don't think MR Arthrography adds a lot of value for knee MRI studies unless you specifically want to assess for re-tears after meniscussurgery. but even then...

What's different between ramps lesion with meniscocapsular contusion or tear?

ramp lesion is the broader term, describing a lesion of the meniscocapsular attachement, tear itsself can be a ramp lesion, but can also "just" be a tear of the PHMM (if it is in the white zone for example). contusions are not easy and typically in the literatur they do not use the term meniscocapsular contusions, do you mean separation? separation would be a detachement of the PHMM from the posteromedial capsule.

I have a question. If a patient has a symptomatic meniscocapsular tear for a year and half would it be in the patients best interest to explore surgery? The mesicocapsular attachment shows scarring but the patient complains of chronic joint line pain and instability. The pairing also as a small focal lesion on the patella cartilage. Thanks for any ideas
