What I think is that either the ghost engine somehow heard about the plan and decided to give Duncan an extra scare or Duncan got caught in one of the ghost engine's endless cycles across that bridge.

Just out of curiosity, was the ghost engine warning Duncan about something? Spoiler warning in 3...2...1...The reason I asked was because in the fan made thomas story Old Haunts, Rheneas says that at the spot where the engine fell, there was a crack in the tracks. So was this a warning, a way to prevent another tragic death, or was this just another failed attempt at making it to the other side?
