In addition to the question above,on question #5 of the IOL adviser form,how large is the font you are referring to on the paper used to measure the distance by which a person is most comfortable reading from? If the writting is large obviously one can hold the paper farther away so I wanted to understand you question better in knowing what size one should assume the font to be.Thank you

After taking the IOL Adviser test and questionare,it was reccomented that I get the full range of vision IOL,but you also stated that intermediate vision may still require the need for glasses.This surprised me because I was told that ready glasses would only be neccesary for fine print or activities requireing sight closer than intermediate,more within the near vision range. But your statement about personality hit the nail on the head so to speak because it is the adaptation period which scares me.Specifically this statement,"PLEASE NOTE: you have indicated, that your personality is not an easy going person, and you might be a perfectionist in some cases. Practical experience shows, that some patients with that type of personality might be unhappy during neuroadoptation period, or underestimate drawbacks of premium IOL technologies. So, if you decide to implant that lens type, please make sure that you are emotionally ready to face with some imperfections of your vision at certain circumstances either temporary or permanent, and your freedom of glasses is more important for you." What occurs during this neuroadaptation period and what if I am never emotionally ready to move forward?This is why I am doing so much research and asking questions but since there are no guarentees as to the results its difficult for me to accept the unknown.If the full range of vision iol is considered the best choice to be free of glasses then why do most end up needing at least reading glasses,or am I wrong?If I choose this option as my choice of iol,at what distance outward will my distance be corrected? I feel paralized by my fear of losing my sight,my near sight,which is the only sight I still have. I wish it was easier to decide.I would pay anything to have no drawbacks and get a iol that was closer to perfect vision. ​

@@babybear4524 I never told that for intermediate vision glasses are needed, it might be for some trifocal lenses, that at certain distance vision is not good for particular needs. Neuroadaptation means that for some distances you may have slightly blurry vision, you may have night time dysphotopsias. The main challenge that the brain has to know how to "select" the image which your perception needs.

Which IOL type would you suggest for a person who likes to golf and would tolerate reading glasses for near? Any comments on pro and cons of each technology for this sport? EDOF and trifocal mainly. Thank you and congratulations on the information you provide regularly.

Which IOL will you suggest for my 60Y old Mother, who is high myopic (Even after Cataract Surgery, she will need glasses for myopia, that's what Doctors said) and hyperopia with weak retina? standard monofocal or Enhanced Monofocal? these are my options: Hoya Vivinex / Johnson & Johnson Technis / Eyhance or Zeiss ? So, if she will have to wear glasses for less number, is enhanced monofocal worth it? Any disadvantages of EDOF , like reduced night vision or halo or glares?

Around 10 years ago I recall seeing an article claiming that nanotechnology had been used to create a lens that worked just like a natural one. I wonder what happened. Did money-grabbing CEOs decide it was too expensive to fabricate a perfect replacement?

Alex, I've read some comments where someone said that doctors prefer regular non-toric IOLs to toric ones because toric ones have more issues and sometimes the vision with toric may not be clear. What is your opinion on why this was said? Is this accurate info and if so, why? Thank you

Toric IOLs require more precision in their placement within the eye compared to non-toric IOLs. If a toric IOL is not aligned correctly, it may not fully correct astigmatism, leading to suboptimal vision outcomes. But, if there is a need to place toric lens it shall be placed to avoid future astigmatism correction with the glasses. but every case is clinically unique, and you shall consult discuss that with your doctor

Very interesting video. I'm a bit puzzled as to why these lenses are widely referred to as having extended depth of focus instead of being correctly described as having extended or increased depth of field ?

it's correct observation. the reason is same why nowadays surgeons say "multifocals" to any presbyopia corrections IOLs, like "Xerox" to any copy machine

Doesn't extended range or depth of focus come at the cost of quality of vision (in terms of contrast and colour saturation)?

Hi thank you for your videos! I have cataracts and high myopia but otherwise healthy eyes (healthy retina, corneas, only minor astigmatism). My surgeon said multifocals are not an option and he wants me to get the Tecnis Eyehance lens, but he did not mention EDOF at all. Are EDOF lenses not a good option for high myopia? Thank you!

Is it difficult to adjust your brain for EDOF Toric lens or I should go just for Toric IOL? At the moment we have established that I will need Toric IOL of some sort. I’m worried what doctor would say on 2nd consultation, after 1st consultation where he examined my eyes he wrote that we need to do an operation with Toric lens, I’m still worried about an intermediate vision as I’m a piano teacher but don’t want reduced contrast sensitivity at night. I am of course will tell him about it but I wish to know your opinion.

Toric IOLs are designed to correct preexisting corneal astigmatism, and it is just a clinically needed option for some patients. EDOF is a technology which improved depth of field which patient may see sharply without use of glasses, as with any IOL we loose natural accomodation mechanism and we need pseudo-accomodation to be less dependent on glasses. Contrast sensitivity is a personal perception and ability to see a low contrast objects which might be affected by eye deceases and IOL design and type, as some IOLs are decreasing image contrast thus inducing contrast sersitivity loss. Coming to EDOF, there is number of lenses available with different contrast influence. Some reduce contrast significantly, some not, so the lens choice will depend on the options available. Coming to the brainn adjustment question - generally EDOF doe not require specific neuroadoptation comparing to the trifocal lenses. Discuss with your doctor your life and work conditions and try to get an answer what visual experience in terms of visual acuity and vision quality you may get with particular IOL model in critical conditions/environment for you. I have some IOL models review on my channel, it may help.

Why would my surgeon not even give me the option to get an extended range vision lens and only want me to choose from basic or toric monofocal or multifocal. He shook his head no when I asked if this type of lens was an option. I don't understand why.

Sorry to hear that. Where are you from? What country / region? Normally at least for advanced monofocal EDOF there is no limitations at all.

@@iol-adviser I live in Southern CA. Between Los Angeles and San Diego. He also did not like the light adjustable lense option for me or the Vivity multifocal lens which I said I read good things about. He said it's only for people with certain eye conditions and since my eyes are healthy he wouldn't consider that lens either. He only wants me to choose between the Basic monofocal or the toric monofocal by Tecnis or the multifocal lens using the toric Panoptix .He offered a near vision correction with the toric monofocal but I am concerned that my distance vision will still be really bad like it is now even with glasses. How can I decide and what would you expect my distance vision to be like if I did choose the near vision correction using the toric monofocal lens? Thanks

@@iol-adviser you know what,as I listened to your other video just now and you spoke about Symfony being an EDOF lens I realized that my surgeon did mention this lens as an option for me along with the Tecnis toric monofocal or the panoptix toric. So I was wrong in my first comment to you since I didn't realize this. So given these 3 options what would you suggest are the biggest differences and what questions should I ask myself in order to decide? Thank you again.

Regarding your experience, that is sad, that doctor did not discussed deeper your visual needs, and left you alone with totally different options from monofocal to Toric Panoptix. I have been trying to understand the iol adviser questions and after converting the distances you asked about in the first question I arrived at the following: You refer to distance vision as 6.5 feet and outward,refer to intermediate vision as aprox. 2-3 feet,and refer to near vision as aprox .9 - 1.5 feet. This left me confused because I wondered what the vision between 3 feet and 6 feet is considered to be.Is that intermediate or distance vision? I was also confused because my doctor always referred to close up vision as being less then 18 inches but never set a defined closer distance as you did by specifying 30 cm-45 cm. What happens to the distance between ones eyes and 30 cm? Is that not part of ones near vision? I just wanted you to clarify this for me and tell me if these distance ranges which you have defined,are considered to be the same distance ranges which are generally used to describe how each of the many iol's function. For example if I was to think about getting a monofocal iol and that my distance vision would be corrected,should I used your numerical ranges to refer to,that of 6 feet and outward,when thinking about how this iol will correct my vision.Are your defined:near,intermediate,and distance pararmetes given in the IOL adviser questionare only relevent to the questionare? Do they apply to other conversations related to where these 3 ranges of visual correction or visual loss would occur when attempting to describe or explain the ranges at which various IOL's are considered benificial as well as where they have drawbacks? EDOF is a technology which as a result gives Increased (Extended) depth of field or range of vision, other words focal point is extended (EDOF) which gives a patient insted of 50cm of sharp vision let's say 100 cm of sharp vision - as an example

depth of focus is physical measure of let's say sharp projection on retina. depth or field is a physical measure of area in front of your eyes, where you can see subjectively sharp
