Microdiscectomy is open surgery with microscope. Endoscopic surgery is not open surgery.

Hello Doctor, very informative and precise way of explain the surgery and so impressed with your deep knowledge of your subject. However, I have one question that after removing the herniated disc portion how you stick the original disc at the place. do you use laser, staple, glue? I am having same problem in L5-S1. After, seeing your explanation I have gained a good confidence in this procedure. However, I have big concern that if the annular tear is not sticked than there are chances of disc reherniation. So nice of u sir, i came across many videos but find ur explanation useful and upto the mark ,sir when u remove some part of disc doesnt that weekens the lumbar bcz of the void crrated there,as disc acts as shock absorbers...? 2. Very good question . Let's discuss in a language that everyone can understand . You are right disc acts as shock absorber for spine. If disc is not doing that shock absorbing function, patient will have backache. So the choice of treatment depends not on X-ray or MRI , it depends on patient’s complaints. If patient has only leg pain ( sciatica ) we go for Endoscopic Disc surgery, if patient has backache also , we fix the spine with screws ( TLIF ).

Sir, I am from Bangladesh. My father (78 years old) has a slip disc problem. He has pain in his right leg and at the lower back for last 2 months. The MRI report is as below "Disc herniation (Protrusion) is seen between L4-L5 label Central and both para central posterior disc herniation is noted causing thecal sac indentation, obliteration of lateral recesses, traversing and exiting and nerve roots compression." "Partial disc desiccation is noted at all lumber vertebrae." "Osteophyte is noted at L3 to L5 vertebrae." The MRI was done on 15/07/2018 He has diabetes for 35 years. Would you please give a suggestion what treatment should be best for him? Is surgery necessary at this stage?
