We had a question posted: Hi.... thanks for great tutorial..... in bicondylar fractures with posteromedial and posterolateral fragments with needed anterior fixation like your example.... from where do u look into the joint surface posterior or anterior?? Pete's response: The lateral joint line is best viewed from the front. The view of either joint line from a posterior approach is limited due to the soft tissue constraints. So your anterolateral approach takes you down onto Gerdy's Tubercle, at which point you can release the capsule off the prox tibia and elevate the meniscus (usually on a stay suture). Supplementing this with some varus across the knee usually gives a lovely peek into the joint to show the damage.

If you're coming in through the fracture anteriorly (the front door analogy) the depressed part of the joint appears there in front of you, as you come in via the fracture.
