Am I grateful I found your channel! You get it, you truly get it. I was diagnosed with degenerative labral tears and severe cartilage loss in both my hips. Was told the only option was THR when the pain would become unbearable. When I found you, my pain was an 8 on 10 and I could not sleep at night. After doing your exercises, I am now at a tolerable 0.5. My stiffness is gone, I walk for several hours without getting tired and I go up the stairs without holding on to the railing for dear life. Thank you so much! Do you offer personalized online physical therapy for those of us not in Texas?

Thank you! I am a physician who has just found out I am pre-replacement; your exercises are so practical and accurate! I plan to avoid intervention. My question is if I was doing 70-80 deep squats before my left hip pain started, should I do the glute strengthening any more than I have? I am giving up CrossFit, just have too much pain.

Thank you for your clear, kind & thorough videos. I am what I call a "3rd generation jointer". Grandmother hip replacement..Dad hips & Mom knees. My Dad & I also 'share' hammer toes. I focused on ending the legacy! Did moderate exercise, swimming & 5 mile a day walks from 25 to 60's. Hiked Camino thru Spain at 65. Then fell on ice and fractured tibial bone. Wouldn't accept pin and did natural recover with therapy. BUT my oposite hip took the toll. Now told "arthritis...bone on bone ...replacements necessary". I've been trying to delay because my research suggests we're jus a few years from various stem cell regenerative therapies, even in the U.S. But finally I have given in to the pain (do not do pain meds or shots), Family pressure and Orthopedic evaluation. I am in the double hip replacement que. YET, obviously, still exploring staying WHOLE & HEALING this too. I'm writing to share that only a few days of this exercise series have me with half the pain & incrementally improved range of motion. 😮 I intend to see if I can get outa line for these replacements and regain my strength and active Life. Wish you were in Oregon ... then I'd be sure I could.

Hi from the UK. Really find your channel valuable. Thank you -very thorough and validates everything my own chiro/ therapy team have been working with me on. I was referred for OA bone on bone hip operation and had developed severe pain after walking and when going to bed. After seeking a second opinion and some basic traction and the start of a muscle strengthening and rebalancing programme, I am now 95% pain free (now just some changing sensations rather than pain as I work through the exercises). The results were literally after a couple of of weeks and have been sustained so I cant recommend this approach enough to everyone thinking they are facing an operation. Key for me is embedding the regime and lifestyle changes. One thing I would really value your opinion on if I may please: I cant avoid some periods sitting on a chair at my computer for work. When I have to do this would you recommend using a balance/ exercise ball instead of a chair - would that make any difference (certainly don't want to make things worse). Thanks for your advice. Rob

Hi Rob, can I ask what exercises have your team suggested? I’m also in the Uk and possibly in a similar situation to yours but often the exercise prescribed are the ones that are not recommended on this channel. Certain lifts and stretches… Or do you work all your muscles and not primarily flutes? I’d be grateful for your help to hear about your experience so far, as it seems you’ve improved. 🙏🙏🙏

Hello, Thanks for your very informative and detailed videos, how wonderful but wish I had found them years ago!!!! I have HAD a total hip replacement 6 years ago and soooo wish I hadn't!!! I am very fit but I injured my hip when lifting a heavy table awkwardly when moving house and then the downhill slide of favouring the painful area and then gradual deterioration by not attending to it. Through seeing a great physical therapist we almost fixed the problems but was convinced by professionals the hip replacement was necessary. My replaced hip is now, over the last three years, as painful as it was before the hip replacement despite all the various strengthening and releasing exercises I have been doing. On a "bad" day the pain through my hip and leg when standing after sitting, is sufficient to mean I avoid sitting! I am not always aware of what triggers the problems. I have targeted strengthening my glute muscle whilst turning off all front work eg hip flexor, quads etc. which has been very beneficial. Often it's sleeping that's the biggest problem so have trained myself to not bend my legs when side lying. My other hip is still doing well despite being told that would also need a THR...

I was told I need hip surgery but because of insurance considerations I had to do limited physical therapy sessions first...Which I did...They never gave me these exercises but others that did not do a bit of good! Yours have helped immediately. It made me wonder. as the PT Department is part of the hospital where my hip surgery is to be performed. if this was intentional as, if my hip improved, the surgery would not be necessary. Is there a financial consideration here on the part of the hospital?

Hey Johnathan, Greetings from one El Pasoan to another! 🙌. We're so sorry to hear that you've been suffering from hip arthritis. We know how difficult it can be to deal with and the frustration that comes from being restricted from doing the regular activities we used to be able to do. If you are feel a tremendous amount of pinching when going down you want to ease up on the intensity. Try the exercise at 5% intensity and only go down to your point of control. When you feel the pinching or any pain it means the muscles are not at the strength they need to be at yet and you will need to ease into the exercise. So much of what you are saying makes perfect sense, and also supports what I have been saying for ages!! Hip replacement surgery is NOT a magic bullet, and if the issues arise largely due to weak muscles, they are still going to be issues after surgery! In my case, at the end of COVID19 lockdown when I could finally see my physio (he’s amazing!), he told me that all the fascia had become stuck together, due in large part to inadequate hydration and too much time sitting at my computer. In addition, the glute muscles, hip flexor, piriformis, etc had all locked up. Further complicating matters, I have a large bone spur on the inside of my knee that looks like calcification of the insertion of the muscle, and also I have a number of large lipomas distributed around my thigh, some of which feel like they are entangled in the muscle. I also have frequent issues with the sacroiliac joint, which seems to go out of alignment and affects my hip and knee. More recently I discovered an amazing woman doing myofascial release massage, which has been so beneficial for unlocking the muscles around my hip - also revealing just how weak they have become! So now I am embarking upon the work of strengthening the muscles and restoring mobility and independence. Your work on this gives me so much hope!! 🥳

One way for an older woman to get better is low dose estrogen. I had only been off HRT for four years, and all my joints were in very bad shape. Injured shoulder would not heal, arm atrophied, in pain sitting, in pain sleeping. After a few months of low dose estrogen, began to improve, injuries healed, muscle atrophy was corrected, able to walk 1.5 miles in 33 minutes, able to climb repeated flights of stairs. Also - only sat on a thick memory foam pillow and figured how to keep leg from rolling out while sleeping.

I wish I had stayed on low dose estrogen therapy. Stopped after I turned 70 and now my bones are deteriorating. THR with a bone graft and still not back to normal.

You can get bio identical estrogen; some doctors will prescribe it if they are familiar with it (and you can even get it online in some countries). Estriol is the safest estrogen; it's also used to prevent / improve vaginal atrophy. Important to take note of differences for women re our health.That can include how we exercise as well: for example, our spines are slightly more curved and, for myself, anyway, I find it hurts if I flatten my back too much (so I just adapt exercises that have you flatten your back).

Regarding the hip fidgeting, should we be bringing our legs completely together or is that not recommended? I've tried both ways and def. experience more fatigue when I bring my knees completely together, closing the gap entirely between my legs. I look forward to your prompt reply.

Hey Rex, the fidgeting should be done in a way that is comfortable, painless and it shouldn't bother you or it can make your pain worse. Just as we demonstrate in the video, it is more of a relaxed wiggle than anything that is causing fatigue. Hope this helps! 🙌🏽

Can you do these if you've had a hip replacement on one side?

Thank you very much for these exercises! Please, can you share if a hip cartilage itself is capable of regenerating and becoming healthy again? Or these exercises can only strengthen the muscles around the joint (so the pressure there would be more equal) - but cartilage itself once damaged can't regenerate anymore? I keep reading that this area pretty much doesn't get any blood supply so for this reason it can't heal once its injured :( So much want to believe that it's not true and it can actually regenerate...

@@gracie45 And what is the relation between the labrum (cartillage) and the bone ?? Not much. For the most part, when dealing with arthritis, you don't want to body to create a abondance of bone-building cells... the cartillage, once is gone, is gone. There is a ton of supplement you can pitch your money in, maybe it'll secure you, maybe it'll do nothing. When living with degenerative condition like hip arthritis, we must slow the thing down, and we can do so by multiple factors Nutrition Movement habits Muscles fitness the exercices this person gives are great ! That is what we can do. We cannot do much for the cartillage, but to try to reduce the frequence per day we do the movement that hurts/that bone-on-bone or bone-on-cartillage our articulations. For this, Feldenkrais Method of movement really helps ! Generaly speaking, somatic movement helps to regain a fluidity of movement without pain. take care !

I just found your videos! I have been doing PT for 8+ weeks for hip pain to no avail. I am (or have been ) a very active 66 yo and started with hip pain this past summer. I was teaching water aerobics 2x week and thought I just needed to stretch more. So, I’m starting with your exercises which seem very helpful. How often should these be done? Daily? Every other day? Thank you so much! I look forward to being able to walk and go up steps without pain!!!

Thanks so much for sharing your wealth of knowledge. According to xrays I have bilateral FAI with cam morph some osteophytes on the neck of femur with lack of spherical shape on left and alpha angle of 63 degrees with some mild narrowing of joint space on left side with os acetabulae , during knee raise exercises I can feel a deep pop in the joint on that left side. I normally do a lot of bodyweight training and hiit workouts with dumbells. I stopped Basketball and Soccer for other reasons. Last 4 weeks burning pain in hip on one side, muscles are significantly weaker on that side (noticeable) vmo, bicep femoris and calf. I had ultrasound 6x1mm labral tear , 2 years ago 7x2mm labral tear but then I was getting anterior pain into groin it was never followed up. I now have ache in glute and discomfort constantly. My TFL, quads and hip flexors on that side overcompensated and are dominant with weak glutes I used to get lateral knee problems and plantar facitis until 2 years ago when this was picked up and got orthotics fitted plus muscle retraining. ( My acetabulum on left is anteverted so I assume the ortotics may have made things worse). I went to a pelvic physio in septemeber as I wanted to see if I could improve . They gave a lot of exercises and a PR exam 😂 apparently my obturator internus is also inflammed. Before 3 weeks ago I never had the constant pain /in the hip. With the exercises I started getting medial knee pain on deep flexion and full extension. I had old posterior horn fraying, pes anserine bursitis and a partial tear of meniscofemoral ligament. The knee is improving . Hip I feel all the time. I stopped training as I had vein surgery on my right leg last week. The only activity I increased over the last 3 months was spin classes on the stationary bike. Interestingly I had my squats and lunges assessed, pain doesnt occur when I do these movements ( I squat to 90 max) I took a second opinion from another physio, non of the tests cause acute pain in the hip the right is defo more fleixble. If I put my hands on both sides when I walk I feel them both popping (they have for years) The relative rest seems to amplify the pain, I have started some of your vids and am trying to keep the glutes switched on for walking and sitting. I just received MRI - Tear of anterior labrum, focal grade 4 anterior superior weight bearing cartilage full thickness fissuring with sub chondral cyst 13mm, High grade chondral thinning of superolateral weight bearing aspect of fem head, all the muscles and tendons normal no synovitis no effusion. I am RN so corridor consult with Orthopod rest don't do anything for 2 to 3 weeks NSaids and Paracetamol . I was so shocked to see this as up until December always active, never any pain with squatting or lunging. Assuming bad genetic biomechanics and kineseology weak gluteal chain overdominant quads, orthotics for flat foot on left side (body trying to keep the ball centred I guess) niw made arch neutral but raised medially causing more supination. SLIGHTLY pointed outward toes for squatting contributing to friction. Sadly, the PT I been to here don't seem to really care or explain this is just my understanding of issues. I am in mid 30s and as I said have been always super active (cardio normally elipical and spin bike) Any advice appreciated, god bless and warm wishes from Straya 😊

Hey there, great question! By improving the muscle imbalances that are likely present already, you can improve the joint alignment. This will maximize the efficiency of the joint and decrease the inflammatory processes that happen because of faulty mechanics. Hope this helps!

I have been trying your squat technique and trying to make sure I contract my glutes and not use the quads. However, there is 'tension' in the front of my pelvis. Even if I do baby squats. Hardly go down, holding for 10 seconds. Is this 'normal'?
@epmanualphysicaltherapy
@epmanualphysicaltherapy Жыл бұрын
Hey Mary! Great question! Sounds like you are not ready for the baby squats yet. The tension you are feeling in the front of your pelvis might be your quads contracting along with your glutes. This is called co-contracting. I would recommend scaling back to the third exercise in the video of glute squeezes to focus on contracting your glutes instead of your quads. We talk more about co-contractions in this video here below: "Should I Be Walking Even Though It Actually Makes My Knee Pain Worse?" kzbin.infoTtZKcbvZMxM
