EPSTEIN BARR VIRUS----Testing🧪🧪
Viral Capsid Antigen (VCA)-IgM antibody--Appears first after exposure to the virus and then tends to disappear after about 4 to 6 weeks
VCA-IgG antibody--Emerges during acute infection with the highest level at 2 to 4 weeks, then drops slightly, stabilizes, and is present for life
Early Antigen (EA-D) antibody--Appears during the acute infection phase and then tends to disappear; about 20% of those infected will continue to have detectable quantities for several years after the EBV infection has resolved.
Epstein-Barr Nuclear Antigen (EBNA) antibody--Does not usually appear until the acute infection has resolved; it develops about 2 to 4 months after the initial infection and is then is present for life.

I just had aHSCT/chemo for MS and am unsure of my status of EBV, and whether knowledge of this would matter or be of use? Hi Dr. Sung. Been having fatigue for 7 weeks now. Checked for EBV and my blood test EBV VCA IgM high, EBV VCA IgG high and EBNA IgG negative. They did not check for EA-G IgG. It was said that I have an acute current EBV infection. During one point (a couple weeks after the antibody tests), we checked for EBV DNA (quantitative?) from whole blood and it was not detected. Could you speculate as to why that would be? I would really appreciate any feedback. Been struggling mentally from this situation.

I have had a very high EBNA and VCA IgG titer for 3+ years (600 u/ML for EBNA and 218 u/ML for VCA IgG) have been having digestive problems and rashes. Is it possible that I have CAEBV or HLH?

Dr. Sung or anyone reading, I am in the 5th category (the last one) where all of my antibodies are positive except for IgM. I also had an EBV PCR quantitative whole blood test done and it was negative. Is the EBV PCR reliable to conclude that I do not have a reactivated infection? I do have chronic low WBC in the 2's and 3's as well, but viruses like COVID-19 I was able to get over just fine, and I do not get sick very often, just have other signs like chronic fatigue and brain fog.

@@alybrinkley2 A couple docs think I do have a chronic reactivated EBV even in light of the EBV PCR test being negative while another doc does not think I have a chronic EBV because of the PCR test was negative. Don't know what to think! Let me know if you find anything out!

So you don’t have one case on there.. I have been having positive IGG, positive IGM, positive EBNA BUT negative early antigen and it’s been like that for a year. My doctor says it means the virus is not actively replicating. what’s your opinion?

So, if I understood this correctly, you could still have chronic even if the EA-D Igg is negative? I'm positive for the igg and necular. My symptoms have slowly gotten worse over the last year. I was diagnosed with CFS but now I'm wondering if it's chronic EBV.

Positive and negative doesn't show anything. Very important is the testing to be quantitative to showing the number in the body, to be count. Exp. normal from 0 to 5 . Your count show 500. Need to talk more. More exp. You have thank in the car. You say the gas is positive. You want to drive 500 km, but in your than only you are on reserve. Are you negative or positive!
