I’m so sad my son had pulp therapy and resin crowns placed on all four of his front teeth. I wish I could send this video to the pediatric dentist we go to. More pediatric dentist should learn how to treat patients with this method. I am thankful for your work and information online.

just wow madam eliminating the first click n then mixing it is something new for me ( can you please share some info on that) putting opaque over the crowns is something I will wish to try soon

Thanks so much! “The capsules will remain the same, only the technique has changed. The activation step (one click) was there only to make sure that the plunger is depressed completely, but it is not necessary anymore. Also, we noticed that it may cause improper mixing in some cases. Therefore, we decided to get rid of it. This change is applicable to all capsule products regardless of their lot number.” (from a dentist/materials science colleague at GC)

Thank you for these videos! My daughter is 2 and her dentist has recommended crowns and a pulpectomy on her two front teeth. We have completed 2 SDF treatments but the dentists in my area are not familiar with covering with glass ionomer after. Is it possible for the roots to heal if the caries are addressed with a non invasive treatment such as this? And how do I encourage our local dentists to look into treatments like this? I would really like to avoid anesthesia. Thanks!

Unfortunately, it is impossible to say without radiographs or a clinical exam. Sometimes the decay is already into the nerve and this technique is not an option. You could ask your dentist if they use glass ionomer and if they can do atraumatic restorative treatment (ART)/ interim therapeutic restoration (ITR) instead, and/or find another provider that is familiar with and uses glass ionomer restorative. I lecture on this topic frequently. Not a lot of US pediatric dentists are trained with glass ionomers, so it may be a challenge to find a provider. Minimal intervention dentistry is more common in Europe and South America.

@@affiliatedchildrensdental It's a shame about GI in the US too because it's an amazing material. I use it almost exclusively in children, no composite, no amalgam (although I'm not anti-amalgam), but glass ionomer all day long. I was lucky I spent my first 2 years working on kids in a clinic in California where it was available to use any time I wanted. PS1: We all like to avoid giving kids anesthetic injections but have you considered custom compounded topical? I'd use a blend of Prilocaine, Tetracaine and Lidocaine in a viscous base and we'd almost get pulpal anesthesia if we let it sit and soak long enough (used a cotton roll in the vestibule to keep it in place and undisturbed). PS2: what's the name of the music you used? I loved it!

I'd be curious to know why a HVGIC is used instead of a RMGIC? Wouldn't it be faster to light cure instead of waiting 2min30 and have immediate maximum strenght of the GIC ? Thank you for the advice!

You could use a RMGI, but it will wear & yellow faster, so you’d just have to repair or replace it sooner.

This aren’t as durable or aesthetic. It’s an alternative if the child can’t tolerate conventional surgical dentistry & the parent doesn’t want sedation.

But what about the pulp? 52, 62 seemed to be already infected and endo needed? What prognosis, waiting for fistula?

The teeth were asymptomatic and not infected. The prognosis is good because less invasive preparation (partial caries removal) is more favorable to the vital pulp. (JADA 2008, Cochrane Review 2013)

Because glass ionomer is the appropriate material for ART (atraumatic restorative treatment). It’s hydrophilic and bioactive. Resin is far too technique sensitive, requires harsher etch, ideal isolation, and extensive caries removal, which some young children may not be able to tolerate without sedation.

A simple occlusal could take just 5 minutes in a relatively cooperative patient. A strip crown would be the most time consuming, by far!!

depends which material you use, skill of the operator, behavior of the patient, and then diet, hygiene, and activities of the child (like are they biting into lollipops, biting finger nails, playing rough, not brushing and flossing, etc.)

@@nidiachavez9022 Mom here, the crowns stayed white all the way up until she lost her baby teeth and the new grown up teeth have come in nice and white. She still has one crown left and it is still very white and clean! This procedure has changed her personality dramatically! I have been taking my 23month old daughter to the dentist since March, because of a chipped tooth and her 4 front teeth were a little yellow they said from breastfeeding at night, and they wanted to send us to a different dentist, and this dentist wanted to keep watch over them so they didn’t get worse. We had another appointment the other day and they looked at her for 1 minute and said they wanted to extract all 4 of her front teeth. But her teeth look similar to this girl in the video. I’m taking her in for a second opinion next week. Because it just doesn’t seem right. I’ve been doing a bunch of research and I think they are still able to be crowned/capped. What do you suggest? I have such bad anxiety over what this dentist said. Because she won’t have her adult teeth in for another 4 years or so. She would be able to talk the same nor eat the same And she would have to have oral sedation, but she would still be awake. I’m just so scared.

Different dentists (depending when/where they trained, CE they've taken, etc.) have very different approaches. It's a good idea to get a 2nd opinion and see if someone is willing to try to save the teeth, as long as they're not abscessed, or at least buy time with silver diamine fluoride or glass ionomer ART restorations like these (which a 23 month old will most likely not cooperate very well for). Just do not take too much time, because SECC (severe early childhood caries) is truly severe, and can progress rapidly to the point of the teeth not being salvageable. Good luck!
