Great video! Maybe it would be possible to include malabsorption/protein-losing enteropathies among the etiologies of hypoalbuminemia, and severe burns as a cause of increased capillary permeability?

Thank you for this great summary. I'm an intern at psychiatric hospital. A patient came with leg/ ankle /foot edema, I was so confused last Friday. The patient obviously is having chronic edema . But cuz of the psychiatric complains I'm finding some issues to take a full medical history from the patient.

There isn't one yet on acute diarrhea. However, it's on my list of symptoms to cover in the future. (I estimate another 10-12 symptoms before the Approach to Symptoms series will feel more or less complete.)

I would love to make videos about biostats, but to do it right would require an entire series and I don't want to embark on a new series until I've wrapped up all the ones that are currently unfinished...(approach to symptoms, hemostasis, cirrhosis, IV fluids, ECGs, etc...). This is definitely on my list though - it just might be a while until I get to it.

Frameworks for Internal Medicine by Andre Mansoor is a solid book along those lines, but focuses predominantly on diagnosis rather than treatment.

Yes, absolutely. I didn't include it here because I try to keep these "Approach to Symptom" videos focused on the more common etiologies of each symptom, and protein-losing enteropathy is relatively uncommon. But it's a well described cause of both hypoalbuminemia and peripheral edema.

Would venous stasis explain a cronic edema of 1 leg (a mild one that came and went until the patient got a pacemaker) I heard about this from a patient and couldnt figure out what happened to her.

Edema related to venous stasis can be unilateral (or highly asymmetric), but I'm not sure how a pacemaker would improve it.

Possible explanation : Edema was unilateral ( maybe due to patient‘s anatomy shows more one the one size ) , if the pacemaker is a CRT system implanted as an HF therapy ( e.g EF< 35% + LBBB >150ms) could theoretically improve the symptoms of Heart failure including Edema .. Second explanation : patient got HF medication (ACEI, b-blocker -AT antogonist, diuretics usw) the same time as the PM implantation and thinks that the PM solved the problem ;-) I‘m just wild guessing here

Great Video as usual. I am in disagreement that lymphedema cannot be pitying though. I think at some point and in most situations all edema has involvement of the veins therefore phebolymphedema is not a bad descriptor of most edemas. Differentiation should be make with Lipedema

I included it in the framework @4:25 and later indicate that it's one of the most common causes of acute edema in the US. EDIT: Ah...Just realized you probably mean in the diagnostic framework... It's included under "other causes" for unilateral edema with normal d-dimer, although cellulitis is not typically a diagnostic mystery, except for venous stasis dermatitis that frequently gets erroneously diagnosed as bilateral cellulitis resistant to antibiotics.

It depends on the underlying cause, but yes, it can sometimes be totally reversible. It's best to speak to your own medical provider about your specific situation.
