Well, I think their relationship started out cute, but a lot of April's behavior was kind of toxic. I thought that was especially apparent when dealing with Ethan's sister Emily. It already started with her being lowkey ashamed of being with him in the beginning, because of the stereotypes, which I can totally understand, but I didn't like the way she went about it. She knew exactly that she didn't want the same things from a relationship like he did and yet she never voiced herself, and just always judged him based on how HE was handling things.

He isn't using drugs. He is taking pain medication after his surgery in season 6 and he didn't tell anyone about the surgery so Maggie got confused.

No he doesn't. He has PTSD and also hints for Anxiety and OCD. That episode is in season 1 and it just shows Ethan's obsession with self-control because he feels like people need to control their emotions instead of taking medication that potentially harms them like it was the case for that Professor with bipolar disorder that he was treating.
