I think this song is about a love that the narrator never gave enough thought to when she was young, and now that she's come back into his life as an older woman he doesn't feel the way he used to about her. Now they're seeing each other again, having casual sex, but she wants more but she's terrified he doesn't want it as much as she does. That's my take, but maybe I'm just projecting.
