How high should a dhol be when worn? Should the strap be a certain material to prevent shoulder pain? I find it way more tiring to play while wearing versus on the stand.

Wearing a dhol over your shoulder(s) and playing is definitely a lot more tiring. I feel the pain when I do the odd gig haha. I wear the dhol over my right shoulder when I play. My (right) hand with the tihli is almost fully extended because you want to play relaxed. The dhol is tilted in a way that the bass side points up a certain degree, so my (left) hand with the dagga probably sits at about 90 degrees. In terms of the strap, any thick cloth should work. The wider the strap is, the more weight that will be distributed over the shoulder, rather than it digging into you. I've seen some people use guitar straps because you can get them with padding

Is the tilli movement just a development of muscle memory in your dominant hand’s ring finger and pinky finger?

For me, I use just the ring finger on my dominant hand and no pinky. The best thing to do is to find the fulcrum on the tihli, which is going to vary for each tihli that you use. The fulcrum is the point at which the tihli pivots almost effortlessly. Basically what you want to do is to use the weight of the stick to do the majority of the work. Use a combination of finger and wrist control to improve your strikes
