At the time Shakespeare's sonnets were written, there was a penchant for the use of "metaphysical" imagery and concepts, meaning abstractions, often within a framework of scientific/mathematical/necromantic practices and imagery, as embodied in the poetry of the period - hence the "compass" in this sonnet. This sonnet contrasts a temporal, changeable, secular way of thinking and being to a fundamentalist concept of the unchanging nature of both eternal life and "true love", which, in doing so might be said to be heretical, in that the final couplet implies that "true love" by a human for another human is identical to the "true love" of God to humans and (ideally), of humans to God. For 70+ years before these sonnets were written, religious struggles between Catholicism and Protestantism dominated politics and people's everyday lives. All citizens had to attend church every Sunday and listen to sermons proclaiming God's omnipotence; to learn scripture by heart; and to believe in the "true" and "unchangeable" nature of God, even as the way in which it was framed drastically changed within the lifetimes of individuals. During C16, the nature of religious worship was changeful: traditionally Roman Catholic, the English Church was made Anglican Protestant by Henry VIIII, returned to Catholicism when his daughter Mary became queen, then returned oifficially to protestantism when Elizabeth I replaced her. Then, policy on this matter became somewhat ambiguous, with Elizabeth herself thought to have participated in secret Catholic as well as Judaic ceremonies at times. This sonnet can be seen to compare romantic love to religious devotion, but to do so in a context where it was difficult to maintain either. Thus, Shakespeare's sonnets, and works of literature in general, benefit from being seen, heard and analysed in their true historical contexts.

I had always thought that alteration here meant an alternative love interest, not a changed circumstance. So it would be about falling in love with someone else, not about abandoning your love when he/she is sick.

It can be both. Falling in love with someone else is also a changed circumstance. The poem isn't specific on this point so either is possible.
