Not particularly hard to guess just based on the context. Chase makes a quip about the drug use that just came to light and the guy strongly conveys a "don't you dare" position. Only a couple of things that would mean.

I remember this episode. He had bipolar disorder, and rather than tell his wife about that he decided to get a risky brain surgery in a foreign country and he contracted malaria while there. His wife was pissed when she found out all the lies he’d been telling her.

this episode was so sad. he loved his wife so much that he went through a risky operation so she wouldn't have to know or live with his bipolar disorder; except the surgery almost killed him and she ended up finding out anyway. the conversation is (thankfully) a lot different these days, but keep in mind that S2 of House aired back in 2006 - and even then mental illnesses were still heavily stigmatized in most facets of life. i grew up with a mother who suffered from bipolar disorder and it wasn't until 2002 that she finally got on the right medication after being in and out of psychiatrists offices for 20 years.

@deekat3279 frequent mood swings are more consistent with Borderline personality disorder. As for bipolar, people with this disorder experience prolonged periods of depression interleaved with prolonged periods of the opposite of depression - mania. I wouldn't equate that to mood swings

I have bipolar 2, so I've been depressed for a long time and was getting treatment for depression. However, the meds weren't working (SSRIS, SNRIS, etc.). Then I had a hypomanic episode (didnt sleep for a few days, did some risky things, was extremely restless and talked a lot, was cocky, and I had a day that I blacked out). I just graduated with my first psych degree (going to grad school now woop woop) and I had taken clinical psychology- I knew I had bipolar 2. So I went to my psychiatrist, told him what was up, was started on meds for bipolar. Took a year and a lot of trial and error with medication because not everyone responds to the same meds. Now I am completely stable and have medication (lamictal) that works. Diagnosis and finding medication (and for some, staying on medication) is super hard to come by. Good psychiatrists are out there, but it took me 4 people to get 1 that was thorough and listened to all symptoms and took everything step by step with me. But it's worth it, to not feel all the feelings.

You know, his speech isn't totally random. If you listen to what he's saying, you can make out some patterns. Like when he responds to a question he says "of golf," He clearly meant to say "of course," Golf Course. Or "I grapple average." depending on the context, "average" and "normal" can mean basically the same thing.

This exact thing happened to me when I was a teenager. It was not from head trauma though, from another underlying condition. I couldn't speak, but I knew that I wasn't saying the words I wanted to. I also could not read. All the letters looked like hieroglyphs. It lasted about 3 hours or so.

Lol, as a healthcare worker I was so triggered by Greta just walking into his room. Clearly the doctors, as well as the patients family doesnt know she's coming, there is absolutely no way they would just let her come back, it breaks HIPAA law and I know its a show which is why I have to laugh at myself

I would love to see an aphasia to English translation of this episode. Like from the writer's room, what all of the patient's nonsense actually meant. Like, I THINK "I displaced my function, accolade" means "I gave up using, (and) alcohol." But who knows.

I vaguely remember later in the episode, they had to figure out what he meant from the “nonsense” he said. I always thought it’d be easy to rig up a computer program he could use to talk. Get a list of English words, and set up a binary search program so that he can find a word in 14-15 simple questions that are all phrased “Is it this word, before it alphabetically, or after it alphabetically?” Speed it up by having it ask about more common words first.

My dad once had a blood clot in his brain, talked backwards (like literally you would record him on a tape and play it backwards) for 2 days. Happened out of nowhere while at the grocery store and there was no other symptom. It was the weirdest thing I've ever seen, in his mind he was speaking perfectly normally, but none of us could understand him. Went on blood thinners immediately at the ER, cleared up in 2 days...
