I have had a terrible time with my TKR. In fact after 6 weeks my doctor felt I was not going to be able to break up my scar tissue, so I had a manipulation at 7 weeks. I feel like I have gone back instead of forward. He put me in PT bootcamp for 12 days straight and after 6 days I am not all that farther along then I was before. I had such a tight band sensation after surgery it actually kept me from bending my knee. I can't help but feel depressed and exhausted. Thank you at least I feel a little less crazy!

Hi Linda, I'm starting my 3rd week post op. TKR on April 26th. The Constant pain and interrupted sleep are brutal and taking quite the toll. I feel very discouraged. My doc prescribed 1-2 5mg prkcets. The 90° rom is not happening! Is there acupuncture or hypnosis or stronger more effective meds? Or should I check into having MUA?

​@@janpayne4791 ​ at 3 weeks, you're still fairly early after surgery. Yes, ideally you should be farther than 90 degrees by that point, but everyone is different, so give yourself a little grace. (If I understand Linda's comment, it sounds like MUA didn't help anyway). Most importantly, you asked about hypnosis so here's a little mindset hack: If you're thinking about "what to do if I fail" rather than "what do I need to do to succeed?" It makes you more likely to fail because you've already set yourself up mentally to do so. Talk to your PT and surgeon and listen to their advice, but keep a positive outlook, and you'll be more likely to have a positive outcome.

You are still very early in your TKR journey. I agree give yourself some grace. I have only met 3 people that are having my problems. That's out of many people, I'm sure you won't join our club. Just do your exercises as best you can. There are some gadgets on Amazon that are really helpful!! Keep me posted on how you are doing. I find it a very lonely journey!!!🙏💓

Thanks for the video, it's very helpful! I had TKR about 10 weeks ago. Great progress except for continuing pain and that sensation of a tight band or cap on my knee. There wasn't much discussion in the video about scar tissue and how to work with it. I'm wondering if ultra sound would help dissolve the scar tissue. Also, what about cortizone shots for the pain?

Glad it was helpful! In answer to your questions, it's very unlikely that ultrasound would have any meaningful effect on scar tissue. The gua sha tools mentioned at around 8:40 can actually be quite helpful for scar tissue. Cortisone shots are meant to be more of an anti-inflammatory than strictly for pain relief. Since the joint surfaces have been replaced, that's probably not necessary. Dry needling can work rather well to relieve pain and stiffness after the swelling from the surgery has gone down, but that's something to discuss with your surgeon as to how long after your surgery you need to wait for that as you don't want to introduce a risk of infection (albeit small) early in the healing phase.

@ I am week 6 and can’t feel,my knee when I try cat cow. My therapist yesterday said I could “possibly “ get the feeling back, and that’s why I signed a waiver. Your KZbin gave me LOTS of HOPE. Thank you again.

I'm at the end of three months. I get pain when I overuse, but it's really getting better. I did not do PT!! I Used my knee. I walk on trails, climb small hills, stretch the muscles. Use you leg! Climb a short stairway. Strengthen your thighs. Do whatever works but deal with the pain and get moving.😊

Thank you for your expertise on tightening after a TKR. I am 10 weeks along and around 5 weeks started having a pinching freeling when going down steps or doing rotations on stationary bike. It progressively got worse as I continued on the bike and strengthening exercises through PT. Around 6 weeks, while going down the first step i felt a pop and experienced sharp pain. It felt like a band went over a capsule behind the outside of the knee at the Fibula bone below the replacement. It has happened several times since when going down the stairs or when on the bike. My chiropractor after listening to my symptoms thought it was a tight hamstring going over the fibula capsula. I have had sciatica for a couple years now and this plus tightening of my glutes, quadriceps, and lower back have impeded my progress. I have had great ROM since 5 weeks and have the ok from surgeon but the soft tissue inflammation at the fibula capsule has caused fear in me to go down the stairs properly like i used to do at 4-6 weeks. I just started strengthening again, squats, bridges and other PT and ROM exercises to keep progressing but walking outside for exercises, stairs and bike i have not progressed. Thanks for helping for better understanding of a TKR. If you have any insights to help they would be appreciated.

You're welcome. Can't tell you for sure what the issue is, but there is a joint between the tibia and fibula and a nerve behind that joint. If it happened going down stairs, make sure you have enough mobility at your ankle. Otherwise it can drive the fibula upward with deep squats or going down stairs.

Not sure if we are experiencing same thing. But I had my TKR late 2022. There were several issues with TKR. After much therapy, I had a pinch also near my knee cap (on outside of knee). 3 medical massage sessions and it was much better. I also thought I had hamstring issues until recently and new Orthopedic Dr told me it was the Sciatic nerve behind my knee! Best advice. Do not cross your legs.

Yes this video put peace inn my life… now i know why i have tightness in my knee… I went to Chinese doctor and he performed cupping with fire.. then scraped around my surgical knee… wow the results was immediatel… my right knee felt light… scraping works … this video made me understand why my knee remains swollen but it can be avoided or prevented by various techniques but first understand the rationale why our knees are swollen post surgically

Superb video. I’m having my TKR in 7 weeks and am looking forward to reliveing the OA knee pain I’ve been going through for quite some time. I know there’s going to be a painful recovery period but I’m optimistic on the benefits of the surgery. I’ve been using a massager for the “nerve” pain running down my lower leg below the knee and it gives immediate relief. Thanks for an excellent review. You answered a lot of questions on symptoms and relief that I’m sure I’m going to experience after my surgery.

@@More4Life Thanks for the forwarding link. I viewed it and it has very good information to prepare for my upcoming surgery. PS- Cortisone shot worked great on my right knee but with the left I’ve had cortisone injection, gel injection, and they didn’t help. Had some PT and ended up overdoing the rubber band side steps and strained my right thigh and flexor muscle with pain in lower back and thigh on the right side, so now I’m undergoing PT for that. I’m working through the pain and will exercise my thigh muscles in prep for the surgery.

I’m second month into TKR revision on right knee. The original prosthesis was removed and a different one implanted. The reason was for 15 months on the original TKR I was on crutches and my knee kept mechanically giving way on me resulting in many falls and fractures to ribs, neck, back and multiple contusions and concussions. The pain was 10/10 for 15 months leading to sleep deprivation and depression as I was unsafe to go out and live alone. I’m a 73 year old woman, retired emergency RN and had hip replacement 3 years ago on same side which was relatively easy to recover from. I was in a rehabilitation hospital for 4 weeks post op and am attending outpatient rehab for 2 hours twice weekly. I’m experiencing the tight band and can’t kneel and if I was to fall I would have to shuffle on my butt to pull myself up which I barely have the strength to. I’m scared to give up my crutches after 15 months on them due to all the falls I sustained. The rehab is just as painful but I shall try what you suggest to help recovery. I do get very swollen ankle oedema, if I’m on my feet too long and compression stockings have not helped and been painful at the knee. Would you suggest some lymphatic drainage massage? Also is hydrotherapy useful? Thank you for your video. It’s been helpful.

Not sure how much of this applies to me but it does make sense. I did not have a TKR. I had ACLr and meniscus repair surgery about 5 weeks ago, but I am also experiencing the tight band sensation and sharp pain in my tibia. I'm really struggling with limited ROM and I am not progressing well with rehab. I'm really hoping I wont need a MUA but at this point it might be my only option. I still feel instability in my knee like I did before surgery and I fear that there might be a failure of the graft. This recovery has been challenging to say the least.

5 weeks is still quite early after an ACL and menicus repair. Often there are ROM restrictions that you have to follow in the case of the meniscus tear, and you can't overly stress the graft during the first few months. Follow the instructions of your surgeon and PT, but it's normal not to fell "normal" yet at that point after the surgery. Good luck!

I'm two years post-op TKR. They made me exercise the next day despite severe pain. On several occasions I refused to cooperate further if my pain was not controlled. Do not submit to torture! That tight band sensation is still in my knee and only becoming worse over time. I've always been active which was why I had the surgery, but I'm becoming less active because of this pain. Was not worth the trauma and I'm not better. Just different pain.

Definitely the DVT (deep vein thrombosis). DVT's can become potentially fatal if they dislodge and go to the lungs (pulmonary embolism).

I couldn't tell you for sure without knowing your situation, but in general, it's ok to use them for 20 minutes x 2 as long as it doesn't bother you. Check with a healthcare provider near you if you have concerns.

Thank you so much! This really helped me understand why I have persistent tightness across the middle of my knee 11 months post RTKR. Oddly, it gets worse after exercise. I also have always have numbness and tightness along the outside of my knee across the joint. I have done some reframing since 2 close friends had surgery around the time I did and they have the same issue, so it’s kinda “normalized” this for me. I will try more manual massage and maybe the tens you suggest and discuss with my surgeon at my 1 year follow-up next month.

There are so many factors that affect it that it's hard to say. It could be due to differences between the sides before the surgery, the surgery itself, or other issues (hips, lower back, ankles, etc.) on one side vs the other. Keep doing your best and stay positive.

Thank you for your information! I am at two months post op! The tightness started a couple weeks ago. I’m doing PT 2x weekly & do my home exercises 3 x daily. Ice & massage diligently. I have a bit of swelling mainly at the end of the day. I started walking again & wonder if this is why I have tightness? Which is wild thinking because it’s movement. Anywho, my ROM active is good at flexion 131 and passive is 136 hoping for 140. That maybe lofty!? Extension is 0. It’s been a very painful & tiring surgery. I’m off narco pain meds & taking OVC & topical pain relief. Sometimes I get shooting pain but last only a few minutes. I do have painful moments on the outer side of my knee. Mainly going up my stairs. Still not sleeping well. I think this a big factor for my overall mental health & doesn’t help with my recovery. I appreciated your idea of not dwelling or continuing with negative thoughts around this new issue of tightness. I will work on that. Linda that’s wild they released you from PT. I do hope you’re doing home exercise. It’s a long recovery & building up our muscles, strength & endurance is important. Our bodies are different so don’t give up …getting to a higher ROM. I’m an overachiever so I need to slow down some & find a sweet spot with my exercise program. Take good care of yourself!

Try not to be scared. I’m recovering (3 weeks to the day) and yes it’s been brutal. But I’m slowly getting better. It was so painful before. What keeps me going is knowing other people personally who have had good results. I might take longer to heal but it does get better

Surgery is different for everyone range of motion afterwards depends on what you had before surgery and how well you follow your physical therapy and how you prepare your leg before surgery -yes I would say the first 3 weeks are rough with sleeping and some pain but that's to be expected. Talk with your doctor, family and friends to decide what's right for you! I had both knees done --the right one last year and just had my left knee done 2 weeks ago -my right one was a breeze compared to the left -because my knee was off center so they had to realign it

I broke my tibia and fibula in my ankle and had surgery. I felt that tight band around my ankle the day after my surgery when the surgeon unwrapped my ankle for the first time. It is now one month post op and that band of tightness is still present and feels even tighter if that is possible. The sensation feels like someone wrapped tight duct tape around my ankle and I can’t move it. I should note that I have started physio for range of motion and can move my ankle slightly in all directions. This is really scaring me and the physiotherapist says it is normal. Has anyone else had this happen to them? It really scares me and I am finding it difficult to move my ankle.

Depending on where in the ankle the sensation is, there are some superficial nerves that can get compressed with a tight cast. Keep up with your physio, and hopefully that helps. Good luck!

Tightness above the knee is most likely from stiff quadriceps or possibly the femoral nerve.

Often it's not due to a problem with the implant, but the feeling is very common.

I’m 18/16 months out on both knees and they both still hurt same place and same intensity. The pain never ends. I have ROM and mobility but the pain is ridiculous. Wish I never had this done. I still have swelling too more on one than the other.

Surgeon feels the band feeling around my knees has to do with knee caps could that cause pressure band feeling around the plat he said plastic buttons will stop that feeling could that be the case

Because careful with therapy. I was doing even better after the second TKR. They had me work with a PT student who damaged my knee. He wasn’t being properly supervised. It’s been hell. Don’t let a student near you! I gave the actual therapist a piece of my mind. I had to stop therapy. Now 8 weeks after surgery I am going back. My surgeon says the X-ray doesn’t show any structural damage but something is wrong.

Had my knee replaced on March 14 2024 and I still have the tight band around my knee then went in for a MUA on May 16 2024 and I still can’t get my knee to bend what can I do to get it to bend

@ it was a rough couple months but it’s just amazing now I couldn’t be happier. I feel like I have my life back well, I’ve had the other knee done since then but already I’m feeling so much better.

My khee is painful at the top of and bottom of my kneecap ! It is painful getting my leg in and out of the car and sometimes a sharp stabbing pain doun the outer part of my knee and pain rotating my foot inwards ! Im 1yr post TKR and Im getting so fed up ! WISH i never had it done !😮

Any ideas when sleeping, I’ve found that I have stretched in my sleep and the pain wakes me up because it feels like my knee cap has popped into the mattress.

This is one thing nobody mentions it, completely ruins the appearance of your legs the pain is another subject. I think living with the pain is better than total knee replacement as long as you can walk.

People feel it in different places. Where you describe it is often due to altered sensation in the genicular nerves. I am 36 days in and I still can’t bend my knee I am doing all of the exercises at home and in therapy nothing has helped when walking I am walking with limp

@sharonbaker6461 36 days isn't THAT long, although it feels like it. Make sure to be honest with your doctor and PT about your frustrations of not progressing fast enough. It's a difficult conversation, but they can't help if they don't know.

You can carefully walk backwards that really helps a lot. Thirty 6 days ia nothing everyone recovers differently I am 17 mths po both knees and still have struggles occasionally. I still have puffiness on both outsides of knees. When I work too much my left knee gets swollen and then I start walking with a limp again.
