Good question! In the context of 'a form' or 'a document', they have the same meaning and can be used interchangeably!

Hi Dan! You are in your element indeed! How could I use this last word? Please fill me in on it! Even though I've already used the word ( indeed ).

We use 'indeed' to emphasise a point! So, we must that you have used it in a great way, indeed! It adds strength to the sentence. "Are you going to do it?" "I am, indeed". In this example, the 'indeed indicates' to the listener that you are definitely going to do it or that you are eager to do it. It adds a bit of intensity to the statement. Does that help answer your question?
