Will this fit on a 5.0? I looked into their website and it showed the only fitment is the raptor and eco-boost.

I imagine it would. It's a drop in replacement for the OEM one. And I doubt Ford has a different trans cooler for the 5.0 vs the 3.5. you could always reach out to them to confirm. I have always found them very helpful

Do I need to add fluid, how?

Yes you will need to add extra fluid. They recommend just putting the extra in the regular oil cap and then cycling the engine with your foot on the gas to build oil pressure. With your foot on. The gas for some cars, it does not start
