what do you think about rating the FMEA Detection table with the results pf MSA when applies?

15:16 - the "3.8%" is a terrible round from the 3.86 % that is the average of all the measurements, including the first you mention where the orientation was wrong, so an average containing two stages of process optimization.... Numbers are only as good as those making them :-)

Jangoofy - I'm sorry but my number is as accurate yours. My value comes from a sample, a sample is an estimate. 3.8% is an estimate and 3.86% is also an estimate and we have no way of knowing which is the more accurate....mathematics is all estimated values, sometimes you can know good estimates from bad. In this case we can't....

@@paulallen5321 My bad, it was poorly expressed at too late an hour. What I meant was, as you said in the video, the defects are really 0 %, since they had been 0% since the last improvement, and the 3.8 / 3.9 % should not be used as a value for the end result / report.

The problem with FMEA and frankly most “quality” tools is that they are mostly spoken about in quality circles. When they should really be operational tools. I’ve seen this problem in most organizations I’ve worked with. It becomes a quality department tool and people and leadership don’t put the right focus on them. Same thing happens with things like ISO 9001
