It's even more appalling that you can be the innocent passenger that needs to trust the pilot, the condition of the plane and the Air Traffic regulations. The plane wasn't licensed to carry passengers for profit, the pilot wasn't licensed to fly that aircraft and his license was out of date, the pilot wasn't even familiar with the plane he was flying. The pilot was color blind and wasn't licensed to fly at night. Isn't it scary that all these regulations can be breached without proper procedures being checked. And why is it that they wait for a crash to then make it mandatory to install a 15 dollar C.O warning alarm in all small aircraft. I believe regardless what airport it is and what time of day or night it is, a pilot should have to go to an in airport inspection office and have his credentials checked before he or she is allowed to fly. And also be made to present an up to date service record for the plane.

I think the pilot knew the state of the airplane. It also explains why wanted to fly low. He also forgot to check the oil pressure. To prevent gasses getting in the cabin he also could decrease the mixture.
@Mike_294
@Mike_294 3 жыл бұрын
@@arnoldmeulen how do oil pressure and mixture affect the CO gasses getting into the cabin? I looked through the POH for a PA46 and the heating system is basically the same as that of a Skyhawk with just a few extra steps. It still uses an exhaust shroud to collect heat that gets pumped into the cabin when heating is on - and therefore the C02 is more than likely coming from a crack in the exhuast pipe getting fed into the shroud. Is there something about the heating system that is affected by either oil pressure or mixture?:

@@Mike_294 you are correct, a crack in the exhaust is how it happens. Many mechanics don’t bother to inspect the exhaust. Oil pressure is a completely different system, would have nothing to do with it. Changing the mixture might change the smell of the toxic gasses, but there would still be toxic gases being pumped into the cabin

I spent nearly 6 hours searching this crash site early the next morning by helicopter. We saw nothing. The accident should never have happened had people not tried to do things ‘on the cheap’ in my opinion. As a professional pilot, I find it hard to believe that this aircraft actually left the ground that night given the circumstances surrounding the accident, from pilot qualifications (or lack thereof) to aircraft state, and weather enroute. Someone has to be held accountable somewhere along the planning chain and it wasn’t just the pilot IMO.

@@TerribleFire I don't know if it's changed then, but back in the day if a part of the flight was over water, more rules came into force. For instance the pilot should report when leaving land and making land again, and IIRC should plan so that less time is spent over water. It's all to do with what happens if the engine fails. On the big Jets all kinds of extra certification is needed to cross the pond. (ETOPS) Now I know this guy wasn't crossing the pond but he could have taken a route that greatly reduced his time over the water. By the way, you mention the FAA, I am guessing you are referring to the Federal Aviation Authority ? If so then some aviation rules and laws in the US are not quite as tight as here in Europe. If I got my PPL there, then I would have to take more lessons and tests if I wanted to fly over the UK/Europe.

That's just an argentinism... or the way we Argies speak Rioplatense Spanish. We tend to exacerbate and overpower our discourse with metaphors and exaggerate sentences in order to add some "poetry". "Estoy muerto" equals to "I'm really tired" or "I'm exhausted"... though we never use the term "exausto" (exhausted).

This is because, just like a moonless night over a horizon-to-horizon desert, there is no difference between visual and instrument weather conditions as far as pilot skill required. In both cases, there are no visual cues continuously available, so the flight must be conducted on instruments. With a rated and experienced pilot with good basic equipment, that's no problem. For others, it can be deadly. Even with CO poisoning and any other factors removed in this scenario, the risk of the flight ending this way was significant.

One aspect of this crash has been consistently overlooked. The transport arrangements to move Senor Sala between Nantes and Cardiff were made by the football agents Willie McKay and his son Mark, who were acting on behalf of Nantes. By failing to ensure that the flights were carried out in accordance with either the law of the land or the civil aviation regulations, Willie and/or Mark McKay were in breach of the Civil Aviation Act 2012. For reasons which are not clear, neither of them has ever been brought to justice for their actions, nor has either of them ever apologised for their involvement in this ghastly affair. Willie McKay was declared bankrupt in 2015 and is due to stand trial soon for fraud under the Insolvency Act.

A lot of commenters here have stressed the pilots lack of experience and the bad weather. The real problem was the level of carbon monoxide and I am convinced that regardless of weather or flying skill they were doomed if they did not do something about the exhaust fumes. The carboxyhemoglobin aka COHb level in Sala’s body was 58%. 30% is considered severe carbon monoxide poisoning. The concentration was only increasing as they flew. They were about half way to their destination when they crashed. The pilot had to still be conscious since he tried to pull out of a dive but he could not have had any kind of good judgment even at the halfway point of the journey. I can’t imagine really flying the plane much further even with better conditions or with more flying skill.

Sala returned to Nantes on Saturday 19 January, on a plane arranged by football agent Mark McKay.[40] His intention was to return to Cardiff on Monday 21 January, in order to attend his first training session with his new club the following morning.[41] Sala had been invited to attend Cardiff's match against Newcastle United by manager Neil Warnock, but he returned to France to say goodbye to his Nantes teammates and collect personal belongings.[42] On 21 January, the Piper Malibu aircraft, which was flying Sala from Nantes to Cardiff, disappeared off Alderney.[43] The same aircraft and pilot had flown Sala to Nantes two days earlier.[40][44] On 23 January, Channel Islands Air Search reported that there was "no hope" of finding any survivors in the water.[45] An audio message, allegedly sent from the plane by Sala to his friends via WhatsApp, was released by Argentine media outlet Olé. The audio message translates as follows:[46] Hello, my brothers, how are you? Boy, I’m tired. I was here in Nantes taking care of things, things, things, things, things, things, and it never stops, it never stops, it never stops. Anyway guys, I’m up in this plane that feels like it's falling to pieces, and I’m going to Cardiff. [It's] crazy, we start tomorrow. Training in the afternoon, guys, in my new team... Let's see what happens. So, how's it going with you guys, all good? The way the heating system works in these small planes is scary but simple. They put a metal shroud around the muffler . As the airplane flies through the air , ram air is directed through that shroud and into the cabin. As the air goes through the shroud it is heated by the hot muffler. The problem is that when the muffler leaks (due to rust or cracking etc), that exhaust now mixes with the ram air going into the cabin. That's what happened here. So it becomes critical to remove that shroud periodically so a visual inspection can be done to make sure the muffler is not leaking. You will be surprised how many mechanics are too lazy to do that and will just not do it and sign off the log book saying they did. Even if they do the inspection, there is a whole year that goes by before another inspection is done. Within that year a leak can occur . Commercial operations have shorter inspection intervals.

Amen to that. If Sala had 58% carboxy-haemoglobin in his blood, that would mean his body was operating under anoxic conditions. I know i have been ill from it before. If this aircraft had a separate combustion heater, which i think they don't, then they could have been gassed without knowing. Lifting the aircraft from the sea bed would have answered that question, but the AAIB/CAA chose not to. Why?

Flights under Visual Flight Rules (VFR) into flight under Instrument Flight Rules (IFR) by untrained pilot for IFR conditions is one of the biggest killers in general aviation.

Is it hard to trust the instruments? I don’t understand. You think you would just look at the screen and follow the instructions.
@billmorris2613
@billmorris2613 3 жыл бұрын
Chamberfox It’s something you have to train to do. When I got my instrument rating in 1972 it was a 40 hour course in an approved flight school. With out outside references your inner ear can be telling you one thing and you are actually doing something totally opposite. It takes a while to start believing the instruments instead of what your body is telling you. If the aircraft does no break up the lucky ones will break out of the clouds with enough altitude to save the aircraft and their own butt too.

Bill Morris, Yes, compare this crash with JFK Jr. who crashed again because he was an untrained pilot under IFR conditions , he was only proficient at flying under VFR, and the weather abruptly changed, and the afternoon turned into night, and he was flying to Martha's Vineyard in a thick coastal mist towards a cousin's marriage ceremony with his wife and sister in law, when eventually the small Piper Cherokee spiralled uncontrollably towards the sea, and all 3 occupants drowned. This was in July 1999.

So let me get this straight: The pilot was not licensed to fly that type of aircraft. The pilot was not licensed to fly at night. The pilot was not licensed for instrument flight. The pilot was not licensed for commercial flight. Nor was the aircraft licensed to fly commercially. The pilot was not (alledgedly) considered competent to fly in bad weather. The aircraft was in a bad state of repair. This is, why u only fly with licensed companies with good reputations and ALWAYS check b4 booking. I am, however, left with several questions: Why would the pilot choose to knowingly violate so many rules, there for his own safety, risking his own life in the proces? Who the fuck thought, it was a good idea to hire a completely unqualified pilot? Who booked this "grey" flight? How was the pilot even allowed to take off from Nantes? Did no1 ask for any documentation apart from a flight plan?? When this accident first happened, I felt sorry for the pilot, coz all the stories were about Sala, if the pilot was even mentioned, it was as a side note and usually as "the pilot". I dont feel sorry for him anymore. That "pilot" killed Sala with his negligence and willful lawbreaking. Sala's only fault was to trust, whoever booked this flight for him.
@jtully79
@jtully79 3 жыл бұрын
The pilot was licensed to fly the plane in the UK but not in France due to a technicality. They were not night rated and I believe their IMC rating had lapsed. While this pilot should have been ill advised to undertake this flight for a multitude of reasons, the reason for the crash and fatalities had nothing to do with the pilot’s skill/abilities and everything to do with a carbon monoxide leak into the cabin. It would not have mattered who was flying, the outcome would still have been death.

@@AVlad-eg3ds you can assume all you like. It am not denying the many red flags that relate to this case but when it comes to cause and effect you need to be precise and look at the objective evidence. Assumptions count for nought. If a guy who is unlicensed, drives a bus while under the influence of drugs and alcohol, drives dangerously and then the bus blows up due to defect not detected by maintenance and kills everyone... by your logic we say all of those red flags were contributory factors when they clearly are not. CO is what killed the occupants. Contributory factors were; lack of a CO detector, a cold night meaning max use of cabin heat, potential maintenance issues.
