I would call (either the toll free number or the number for your local office), before sending anything. While you can apply online, you'll need to provide supporting documentation to verify your identity. You can do this either in person or by mail, so I would confirm the details by phone with an SSA representative before just sending the application. Below is the verbatim guidance from the SSA website regarding survivor benefits. If you need to report a death or apply for benefits, call 1-800-772-1213 (TTY 1-800-325-0778). You can speak to a Social Security representative between 8:00 a.m. - 7:00 p.m. Monday through Friday. You can find the phone number for your local office by using our Social Security Office Locator and looking under Social Security Office Information. The toll-free “Office” number is your local office.

I'm not sure what you mean by both. But you can either call the office for guidance or turn it into your office.
