There’s also a command input for the secret hard mode (lb +rb) hold < press at same time (up, left, down, right, X, Y, B, A A) if done correctly you should get a prompt for the secret difficulty (same buttons on PS (L1+R1) hold + (square, triangle, circle, X, X)

The TMP is in the demo- From my experience you have a chance to spawn with it in the attache case as soon as you frag the first guy and if you keep it equipped you can just have it the entire run. Additionally there is a well (right of the tower , left of the las plagas church door they all go through in the ending cutscene). If you discard all of your items in the very beginning and pick up no items, that well should be uncovered and you should be able to pick up the TMP. It's a lot of fun to use!
