I worked in LTC for 13 years I absolutely love the connection you form with the residents...I d love to get hired as an NP there

Okay thank you boo. I work in a LTC and you are definitely right about that. I do my best to provide adequate care to my residents but it can't be me alone. So hard to be efficient when it's just you. Thanks for the win❤❤❤

There is no team spirit in a nursing home. At all...just confusion, coverups, and blame. Good nurses will be ate alive with the nonsense found there!

I have worked in a rehab nursing home. I have to agree with you on all the con except the driving. I don't mind the driving.

I work in a SNF facility as an RN. I’m currently in school for an NP. The nursing home setting like you said is across the board, they are all the same. In my option it’s not the staff per se. It’s the staffing ratio. They are overworked and underpaid. All nursing homes are the same. I did work at one which was awesome but they also were all Medicare patients and the ratios were half of what you’d usually see anywhere else. Better care is given when the employee has less patients.

This was so useful to me. I’m currently trying to decide whether I want to accept a job as a PMHNP at SNFs versus a clinic right by my house. The SNFs pay more but I would have to drive all over the city

Love the video, Acute Care NP student and I have worked in a nursing home about 8 months as an RN. And told myself I will never go back or send any of my family members there. I get angry and frustrated every day, and is just so much you can do. All they care about is money, just sad. God bless you for everything you do for them

I know this video is 3 years old, but I just took an Np job in a rehab/SNF, so this was helpful. I enjoyed my rotation enough, and luckily this job does not require me to take any call after hours. But definitely can see the rest of those cons. I’ll be mentally prepared

You are so right. I am a PMHNP and I often ask the DON and administration will tell them that I will be more than happy to put orders in under their licenses. It is so difficult to to advocate for you residents.

I just interviewed for a Senior Care position as an NP in 2 of the nursing homes that my hospital is affiliated with. I recently graduated in September 2020 and if I’m hired would be my first NP job! The issue with the DON is a bummer and the lack of care is sad and frustrating. But I’m hoping the good out weighs the bad! Thank you so much for these videos! Truly insightful and helpful!

You might have better luck with your DON. I have come across a few who were very good and was really there for the better of the pt. Best of luck to you my dear

I work as a nurse in a nursing home and yes it is true there are so many protocols that we must follow, It is interesting to hear the perspective of the nurse practitioner. Sometimes its scary to call NP's and physicians because some can be rude and callous ,but now I understand how the frustrations you all face day to day.

I have had that happen to me!!! And if the doctor says they go they go for me! They don’t even have a crash cart to save pts in the building! My motto is when in doubt send them out!

I work as an LPN in a LTC/SNF in one of the ‘’nicest’’ most EXPENSIVE facilities in the area. I love taking care of my people and I will tell you there are VERY few people - nurses and aids that actually care. It’s heartbreaking.

When admitting residents .You should put in orders like.Call provider if HR is below or above,, call provider if BP is above or below............This will avoid phone calls all the time

Charting seems to take me forever. How many patients do you chart on in a day? How long does it take you to chart? Any tips?

I absolutely hate charting! hate it. I see 15- 18 pt daily, for wound care I have seen upwards of 40 in a day before. Have your template ready and just plug in what is pertinent to your patient, makes life easier. Also the night before you go to nursing home you start pre charting, with the template on the pt you will see, it will so faster for you. Hope this helps!

Omg! I feel your pain. I used to be weekend supervisor/Wound care nurse and let me tell you...when I’d see the same dressing I applied a week ago yet they are “documenting” bath done. No ...no ..nooooooo. 🤯 Lawd!!! WHO was this DON challenging your judgement. 🤦🏻‍♀️🤦🏻‍♀️🤦🏻‍♀️

Thanks for sharing!! Girl I didn't even say in the video that the pt actually had a stage 3 ulcer on the coccyx! when I was told no she had nothing 😳 I was so livid that day. That DON was a piece of work she would do it with the physicians as well a whole nut case.

I’ve worked in the LTC setting and plan to as a NP. I’ve sent many patients to the hospital and update the provider and DON after the fact. I am not going to have someone go into cardiac arrest while I’m on the phone trying to get ahold of the DON or the provider. As a NP, I think I will have the same issues that you are having because I care deeply about my patients. I also feel like as RNs we have to know how to think critically, know how to do a thorough assessment, know when to use our nursing judgement to ensure that our patients get the care they need in a timely manner. I’ve met some DON and have to wonder how di hell dem get dat position. Lol.

As you should! I am not sure why some DON really think that when a pt is crashing you should be on the phone with them or me as the provider. NO! you should be calling 911 to save the pt. I agree that knowing how to do thorough assessment should be a must. Mi no know how some a dem get di position either. LOL Good luck to you

I hated being an LTC nurse because I couldn't take care of my patients. I got reprimanded for sending a patient out one night...next morning found out they were admitted to ICU. My DON said we never would have had an issue if I hadn't asked the provider to get a CXR in the first place! 🤯 she would have just let them die in misery rather than lose money.

Hi! NP student here. Have you worked with any NPs with less than one year of RN experience? If so, are they successful?

Logically by the time you get done with np school you should have atleast 2 years worth of experience because I want to hope ull be working as you go. Btw this is for people who passed their rn boards and went straight into np school. So technically no one will have less than 1 yr of rn experience by the time they’re done with np school. Correct me if I’m wrong.

No, I have never worked with a NP with less than a year experience as a nurse. Like Tilda stated by the time you finish your program you do have experience bc most NP I know kept working while in school, even if it is part time. I do know NP that start the program before they had a year as a nurse. Success for me depends on the person, I do not recommend not having clinical experience before becoming an NP, just my two sense.

Unfortunately nursing homes are businesses...the owners want to make money, they never spend on supplies and make it hard for the nurses to work. Somebody has to work in LTC and do your best to your abilities. Is all about the money

Unfortunately it sure is just that! a Business and in the end the residents are the ones that suffer, bc they only think about their bottom line. I know that there are a lot of great nurses and such at these places but they are a far and few. Thanks so much for watching
