My husband had a mastoidectomy at age 9. He’s about to be 73. A lifetime of not getting water in that ear. Many ear infections over the years. Only 10% hearing in that ear and 40% in the other. Maybe this explains my obsession with the ear videos. I truly enjoy your stories too. The smells would be my kryptonite of nursing. Thank you for all you do.

You know, when it’s your job, you just do it. And then you get a fry tray and spray it full of shaving cream and then put powdered instant coffee on it, then spread those around the room/unit. Colostomies though, those can be pretty smelly.

I can still remember while I was a student nurse back in the 60s, the first time maggots appeared in the wound of a diabetic who'd had to have some toes removed. We were all very shocked, wondering how it could have happened , & were going to remove them until a doctor told us to leave them as they would eat the dead flesh. The wound healed beautifully.Great job as usual, Richard.🕊

As if the procedure and content removal weren’t enough, we get more personal glimpses into you and your life as a nurse. Loved the reveal of the mastoid cavity! The furry hyphae and thick horrible slimy skin were enough for me to skip dinner in favour of a smoothie. 😓 Better now. From a young age I went on farm calls and saw surgeries as my late dad was a veterinarian. Visuals were much less unsettling than smells in the animal world. Blood is blood, but the sheer volume can give one pause. A prolapsed uterus of a mare is messy, but if she delivers a healthy foal and then has a good recovery, it’s brilliant. You’ve outdone yourself with this episode! Thanks ever so. ❤

That was a fabulous video Richard. Huge cavity after the mastoidectomy. My pet dislike as a student nurse in the 1960’s was sputum. No disposable mugs in those days, just the stainless steel mugs with a lid. We had to empty the mug onto newspaper to be disposed of. I retched every time and just thinking about it now churns my stomach. Same with trachy suction, especially when the patient’s cough reflex was triggered. Have also seen many bedsores that you mentioned, that you could put your fist in, often very oderous. I agree that the odour of pseudomonas infection is something one never gets used to! So many stories aren’t there for nurses 😂 great video 👏❤️

This probably experience is nothing close to the sputum you deal with but some people are into spiting up sputum when I hear it. I don't know how you didn't vomit. There was a neighbour of mine would go on his deck repeatedly. I could not keep the windows open or I would have been vomiting. I think he was smoking something but not cigarettes.

Thank you so much for your stories. I especially appreciated hearing about the suctioning of the tracheostomy tube, as that was done to me for about a year. When I was 3 months old, I got such a bad case of croup that I required the surgery and had a tube(such a tiny one!) for 11 months or so. My grandmother, who was a former VNA, moved in to take care of me and had to suction it clean three times a day. I can't even begin to imagine the kind of care that she gave me. Of course, that was 66 years ago, so it's obvious she did a great job. Thank you, dearest Grandma.

Smells I'll never forget are clostridium difficile and GI bleeding. Things that made me gag were trach suction and a patient allergic to sutures had an abdominal surgery that dehisced (busted open) after she returned home and sat on the toilet. She was admitted to ICU with wet dressing changes to an open, infected abdominal cavity. All that was fine, but as soon as I noticed intestinal peristalsis (food moving through the small intestine), I got light headed.

I love this video! Ty for being so careful with the fungal ear. Very tender and itches like crazy. Do you ever give a systemic antifungal? If it keeps coming back do you think it's a transfer from another part of the body? For example from itchy skin folds for example? The only thing I have to comment on negatively is you said disorientated. That's NOT A WORD! What? I can't believe that. It's just oriented or disoriented. Not orientated or disorientated that's like saying irregardless (also not a word) instead of just regardless it's just adding 2 prefixes to a word but it's the same word. And disorientated instead of disoriented just adds a useless syllable. Interesting enough those words are also used frequently used by black Americans. Because, a lot of the slang comes from their close roots to indentured servants of English, Australian, Scottish, and, Irish people. Language is crazy right? I'm going to look up videos of people who say this again. I love the language roots rabbit hole. Still hundreds of years later the English and black Americans still say these 2 words that aren't words. (I guess you technically said disorientating instead of disorienting. Like adding a "tate" syllable. Still love the videos. But I'm into language roots and it cracked me up to see a doctor who actually says this word that is not a word at all. 😊 My daughter used to say commercinal I stead of commercial. So she added a syllable all on her own. It's genetic for Scottish and Irish people I guess to add a syllable.

Loved this can’t of videos, not fun for the patient as for you, but you did a fantastic job ! Last Friday I had to go to hospital for pain management and my dog is suffering with a cyst on her back that was unfortunately infected so the veterinary gave us some antibiotics for it before he can remove it , wat happened 45.minutes before I had to leave it exploded 🌋 and it wasn’t a funny smell and a lot of pus that I had to clean up I also used a antibacterial solution for those 2 big holes in it , and put a bandage around here. She already had a can’t of swimming bandage around her neck luckily . And this al in a short amount of time because I didn’t want to be too late for my appointment😱 with putting 8 needles in my back to make the nerves warming up. So if something goes wrong it’s always bad timing , 😂 Sorry if this is boring but I just wanted to tell someone, 😉 Thanks and see you soon , 🇧🇪😘

RN for 20 years…..the tracheotomy suctioning got to me more then anything. But I did find something that helped. All the other nurses and CNA’s that worked with me and knew that I quickly had to suction someone, they would get a styrofoam cup full of ice water and a straw, for whatever reason, drinking ice water during suctioning help keep me from gagging….nursing can be so weird sometimes.

Omg! I'm a nurse and I can do blood, body fluid, puke, poop and pee, amniotic fluid, puss, anything EXCEPT for snot. I get queezy even thinking about it. Had to turn my head and close my eyes even to wipe my own children's snotty noses and.... Often gagged or puked after just wiping their noses just at the thought of it! I can do ICU nursing even and just traded tasks with another nurse. Because the suction of snot even into the lungs, the sound, the smell especially if infected, of SNOT or thick mucous even, really puts me down. Makes me feel like I will pass out. I would rather change an adult males poopy diaper than suction snot or even help with a tissue to the nose. Also in my line of work which is hospice, wounds can happen in 30 minutes or less than from from bad circulation and diseases that affect that. So despite turning a patient, even every 2 hours, wounds DO occur at the end of life if someone lives long enough or has certain diseases of circulation including kidney and heart disease . In fact, the newest wound research says that if a patient doesn't go into REM SLEEP and sleep at least 4 hours straight, then the patient cannot build the white blood cells to heal a wound or even keep circulation and wounds at bay. Also, turning someone every 2 hours causes psychosis due to lack of REM sleep. (the US military uses waking up people every 2 to 4 hours for 1 week to make them psychotic and give up their intelligence information, so basically as torture) So now the recommendation is turn a patient every 2 hours during WAKING hours only , and at night only turn them every 4 to 6 hours during sleep time so they can reach REM sleep and rebuild cells and circulation to fight current or future wounds). Lack of protein also causes wounds despite any intervention of any kind. Just some tips for the general public. Sorry but, turning someone every 2 to 4 hours could really harm them. So thought I would tell you of the newest research and recommendations. I short pressure wounds are NOT ALWAYS PREVENTABLE AT ALL. SO, please don't say that. A lot of people who care for loved ones at home instead of placing them in a care home believe wounds are their fault and many times it isn't at all and they could not be prevented. In a heart failure patient they get wounds to the tops of their toes from the pressure of a bed sheet! In just a few hours. So be kind please and maybe give tips to prevent but they ARE NOT TOTALLY PREVENTABLE AT ALL. And no SNOT! Just to remind you what makes me puke. Bug juicy wound? Sure. Puss filled ear? Yep. Puke? Ok. Snot? Even clear snot from a sneeze? I will puke right there.

This is the first I’ve heard of these infections, and the first I’ve heard of the operation, I’m curious to know what causes the fungal infection, and if it could be related to where you live? England is pretty wet, isn’t it?

The series on TV you needed to watch was Your Life in Their Hands. In this series you saw full operations. The ones I saw were: Knee Replacement; Hip Replacement; Staghorn Kidney Stone Removal. I was surprised that with Staghorn Kidney Stone Removal how the surgeons removed the kidney, laying it on the patients back, and then encasing it in ice for half an hour to reduce the blood vessels in order to prevent the patient bleeding to death during the removal. During this time the surgeons go off and have a cup of tea while the anesthetist monitors the patient. The stone itself was like white calcified cauliflower heads being plucked out. I watched this while eating my tea. My father said, 'I don't know how you can watch that while eating your tea.' What? Fascinating. Never really affected me. Would have been a good surgeon me thinks.

Wanted to let you know that what u said about when someone’s life is at stake you just keep on going.I was a scuba instructor for 25 year,so I did a lot of rescue work.More times than I can count, when doing cpr,the person threw up in my mouth, I did exactly as u said , l spit it out and kept going. The human body is amazing in the way it heals itself and rejuvenates.I would love to do earwax removals, the peelin stuff is really cool!

That was a painstaking removal. I assume this patient's ear has been swabbed and sent off for C&S? I wonder how long the course of treatment is each time or is it just persistent. I loved orthopaedics. It's hard work but interesting. My pet hate is smells 🤢 We're you referring to external fixators. They can be used in all sorts of scenarios. My ex husband had a pelvic external fixator after an RTC. I once had someone vomit bile on me just before I went to lunch. My white uniform was a wonderful lime green afterwards 😂 Like you I have lots of stories, some amusing some incredibly sad. Nursing is a wonderful career but not for everyone. I love mushrooms too, not the magic kind. I go through a punnet a week at least

Can you use some kind of scraper wouldn't it be quicker and easier to use instead of the suction

You could, but it would be excruciatingly painful, dangerous, and most likely cause damage to the ear

Your humor is as sarcastic and dry like mine, which is why I think I get you. Question...are mastodon cavities susceptible to fungal infection due to difficult water drainage?

The only thing I couldn’t handle were live bugs/maggots in wounds/cuts…. Little moving things that aren’t supposed to be on/in a human body. Occasionally an eye issue would bother me… But bugs were my Achilles heel.
