This lady has clearly worked very hard with her speech therapy, so thank you Dr J for not second guessing her replies or prompting her. Giving her time to find her answers shows respect for all the work she must have put towards her recovery. And now she has pretty feet too! You were excellent as always.

This is what my wife does for a living. Works post stroke. Not just speech, but SLP’s also do a lot of work on teaching how to chew and learn to swallow food again. It’s a huge part of her practice. They do 6 straight years to become a speech language pathologist. She’s now director of rehabilitation. So proud of her. :). She is always talking about patients she’s trying to help while maintaining privacy even with me. Love that woman. ❤

I am a stroke patient myself. I love the amount of patience you had with her. Not enough people take the time to listen at their speed. She is coherent, but the vehicle to take words from the brain to the mouth is affected. It took me a long time to speak correctly after. I had an SAH, and most of the damage was in Broca's area. I am a one in a billion case of success. She had two back to back, and that does a lot of damage to the brain and how it functions. Patience is the number one biggest thing every stroke patient needs. You are a perfect example of how to do that. I thank you on her behalf and for other stroke victims who have had to suffer through impatient providers!

Strokes are so devastating. Head to toe care is essential. Podiatrist’s aren’t just”foot docs”. You and those in your profession do so much more! You treat the entire person, not just their feet. I wish all those in the medical profession were like you!

Yeah. My paternal grandma had a stroke when Dad was 13. He was alone in the house with her when it happened and was so traumatized by the experience that most of that *year* is blocked off from conscious memory. According to Grandma, Dad saved the use in her right arm by rubbing it until the ambulance came. Grandma was never the same after that. She managed to maintain most of her dignity until the last couple years of her life, but her mind and body slowly started shutting down. It took *decades.*

My grandmother (mother to 10 children) had a stroke right at age 65, retirement age here in Belgium (EU). She was paralyzed on one side of her body. She could still speak and think, that was all alright. She just couldn't move. It put her in a wheelchair, incontinent as well. And age 65 is the cut-off point for receiving government aids (you have to pay for everything yourself). Even though we have generalized health care, this is still scandalous. Strokes are tough and not only on the person themselves, but on everyone involved. My grandpa cared for her at home for the next 15 years (until he died at the table from a massive coronary). Each of the kids had a different day to cook lunch and dinner. They all helped out. It was tough on all. My mom had a full-time job so she had Sunday to care for them.

Excellent care, Dr Jefferson, for the whole person and how those other issues affect the feet. (And vice versa!) 👊 I always love that you send the nail clippings to the lab to ID which organism(s) are responsible for problems. As a former “lab rat” (clinical lab assistant) who worked in microbiology-where all those “great” specimens like blood, urine, sputum, wounds, stool, and yup… even nail clippings go to incubate for *identification* - I’ve always been puzzled by why more docs don’t do that! Seems like a no-brainer to me.🤷‍♀️ Sending a specimen like those nail clippings to the lab, not only does the lab identify the organisms, they also do a “sensitivity” test to confirm which antibiotic or anti fungal (or other drug/treatment) will be effective against it! It’s the best “2 for 1” deal you’ll ever get.😂 (Except for maybe the Arby’s ‘buy one get one free’ roast beef sammich coupon.😂) Way to go on educating your patients as well as the rest of us, Dr Jefferson. As always.👊

The Case managers, and care giver terms may differ depending on what area you live in. For example in the state of Oregon, case managers, handle things like finding medical insurance, finding appropriate and qualified daily care givers, submitting hourly rates for those care givers, and anything else that their client might need to improve their level of care. Care givers are the people who do the daily care for their client. Things like, bathing, cleaning house, brushing teeth,. Making ends... Etc. In other states around the US, these terms may mean something completely different.
