The fault in the tank was a culmination of separate events, starting with the dropping of the tank, followed by an incorrect thermostat being fitted, to a temperature gauge that was pegged at 80 degrees. Any one of those events could have been caught, and the explosion prevented, but the errors were all missed, and lead to the crippling of 13.

@@grababana No problem. I guess sarcasm doesn't transfer well, to the written word. If you haven't read Lost Moon/Apollo 13, I'd recommend it. It covers not just the Apollo 13 incident, but Jim Lovell's early life, and his progress through his career.
