This video is about Inferior Turbinates
Small bones in the nose covered in soft tissue that can swell and cause congestion or stuffiness
Turbinates swell with allergies, head colds, lying down
Can be treated with nasal sprays or oral medications. Afrin or Sudafed can both give immediate relief, but should not be used more than a few days only due to addictive effects and other possible side effects.
Nasal steroids (Flonase, Nasacort, Nasonex, Rhinocort, etc) are most commonly used and are safe for longer term use. Can be used as needed, or daily depending on the underlying cause.
Managing allergies can make a huge difference. Avoid allergens (the things you are allergic to), potentially taking allergy medications, or even getting immunotherapy (allergy shots).
Can be shrunk with a variety of procedures in the office or in the operating room. Most common is to use a microdebrider suction device (like liposuction) or some energy device (submucous cautery, coblation) to shrink the turbinates. Sometimes, portions of the bone can also be removed.
We do not recommend removing the entire turbinate or to damage or remove large portions of the pink lining (mucosa) because it helps heat, humidify, and filter the air that we breathe. Doing "too much" causes a lot of dryness and crusting. It also creates an "empty nose" sensation.
Sometimes we also outfracture the turbinate bone - or push it to the sides of the nose.
Turbinate procedures can be done alone or in combination with other procedures such as septoplasty, adenoidectomy, sinus surgery.
Most turbinate procedures only take a few minutes. Recovery only takes about a week or so.
Saline sprays or irrigation help the healing and keep the nose clear and moist.
Over-the-counter pain medications are usually enough.

I had turbinectomy, septoplasty and sinus surgery 3 weeks ago. First 2 days are rough, nose is very swollen and blood keeps coming out of it. In 1 week, there's no more blood dripping, but it forms these thick clots together with mucus that get expelled when you wash your nose with saline water. At 3 weeks now, still has some bloody mucus but at a much lower scale. The breathing is incredible. I didn't think this was possible tbh, there's no more air blockage whatsoever. Wife said she hasn't heard me snoring anymore. I encourage anyone that has breathing problems to strongly consider doing this surgery.

Real talk here from new Zealand haven't been able to breath properly over the last 7 years I'm 25 now went to get a surgery test and they couldn't find a deviated septum but now I'm go let them know about enlarged turbantes. I figured this out my self but putting my fingers up my nose and realized it's not a hole all my life thought there was a big whole in my nose. I was wrong it's like a thin gap and when I open it a bit I could breath untill it closed again. I will be going to get something done with my enlarged turbante. I am so greatful this Is gonna bring me back up with everything, confident, singing, talking most things I put my self down because I knew I was good at it but then I noticed something was up and I couldn't figure it out. I only figured my nose was blocked on one side like 7months ago. And then I look back at my worse moments and realize why. Thank you God and this KZbin guy for explaing. Thank you for explaining the swelling, the turbinate function and frustrations. I'm considering an in-office turbinate RF reduction procedure, because the hospital surgery sounds risky. I've had no luck with immunology, i.e., shots. Have had balloon rhinoplasty and surgery for deviated septum. CT scan says sinuses are "fine", but turbinates are too big. I swear, I'm almost ready to check out rather than have another doctor guess which part to fix this time. My sinus pressure (that I believe contributes to my depression), he says, is a headache, for which I should see a neurologist. Frankly, I think I can take care of all this myself with nine floors minimum. I can't take my depression and allergies anymore. But thanks. Your discussion was encouraging for most. Why are most ENT's so narrow in their treatment? All my life I've had allergies and depression, allergies and depression. Now add intestinal issues due to way too many steroids and antibiotics. Oh, well, tenth floor, please.
@GeneLiuMD
@GeneLiuMD 5 жыл бұрын
I'm sorry to hear that things are so rough. If you've already had all those other procedures done, a turbinate reduction will seem easy in comparison. The recovery isn't as bad and is reasonably fast. There really shouldn't be any downtime for most people (unless you have a very physically demanding job). I would recommend seriously seeking another opinion re: the ENT options. There really should be no harm in seeing a neurologist as well. There should be a treatable medical reason for the pressure sensation. If you really are feeling like you can't take it anymore, you should also call the National Suicide Prevention Lifeline 1-800-273-8255.

To your knowledge, is it true that it is a common thing to use turbinate reduction as a shortcut in patients that have old displaced (broken) nose bones and septum?
@GeneLiuMD
@GeneLiuMD 5 жыл бұрын
You pack a lot into this comment, and quite honestly, I can't always follow your train of thought. It is true that a turbinate procedure won't correct a broken nose, deviated septum, or sinus issues. It is also true that fixing a broken nose, performing a septoplasty, or doing sinus surgery also won't directly address turbinate problems. These procedures are frequently performed in a complementary manner - many patients need more than one thing addressed depending on what is going on. Every situation is different. What you do on any one patient depends on what types of problems that patient has. You completely lose me when you go on about "...entirely legally..." and so on. But I can definitely say that I can't tell you why or how other people/doctors do what they do. You would have to ask them directly. I can only provide my opinions.

@GeneLiuMD
@GeneLiuMD 5 жыл бұрын
I'm sorry that you are suffering. The more that is done to the nose and sinuses, the higher the risk of ENS. The risk of ENS also depends on what general health conditions a patient may have and how healthy the lining of their nose is at baseline. Additionally, how much to do on any individual depends on the underlying problems being treated. There are also many techniques that are more tissue sparing than what used to be done in the past - all in hopes of avoiding ENS. You are 100% correct that all issues must be carefully considered before any surgery or procedure.

This was really informative! My turbinates are blocking my nose almost completely because of an allergic reaction I had some months ago. A doctor I visited said that I needed to get surgery done, but since I wanted another opinion, I scheduled the surgery and saw another doctor. He said that I might be able to make they shrink naturally with medical treatment and a vacine. The thing is, the first doctor had given me the same medical treatment. I tried it but it wasn't very effective, and I end up never managing to stick with it for long. There's a nasal spray that helps me, but the use is not recommended and I also don't want to use it, but it's the only thing that helps rn. I can handle it during the day, but at night it's terrible, I can't breathe at all, nor can I sleep. The worse part is that the waiting times are horrible. It's been 3 months already, and now I have to wait 3 more if I want to try to do everything naturally, but the medicine is not really helping.. even if I want to get the surgery done (I was previously going to it), the only day available is in the middle of January. I've been wanting since October so..

The turbinate in the right side of my sinus was huge. I couldn't use a netty pot because the solution couldn't pass thru the right side. Also had terrible pain in my upper right teeth/molars. If I had the tool I would have pulled those perfectly healthy molars. My ent removed my turbinates and referred to one or both as concha bullosa. What's the difference, the size? 10+ years later I am still glad I had the surgery, still have open airways in my nose.

I don’t believe these medicines I was prescribed for Covid have anything to do with the relief I felt from breathing difficulties. Rather it’s the medicines from the ENT that gave me immediate relief. The swelling reduced dramatically and I started breathing okay after a few hours

Thank you for explaining this so well. I have broken/fractured my nose numerous times due to training, as well as smoke cigarettes fairly regularly. And for a year, even without cigarettes, I am constantly clearing my throat and _trying_ to blow my nose to have nothing come out. I have anxiety as it is so this really makes things worse, as far as feeling panicked and uncomfortable as it is sometimes. Sometimes with a hard sneeze, or correct pressure while blowing my nose a "chunk" of phlegm (excuse the vulgarity) will come out. But still nothing improves as far as feeling constantly having something in the back of my nose/throat, also when my chin is down to my chest there is a crackling/clicking noise, like there is air pressurized slowly escaping. Is there any tips or information you would be able to give as far as what this could be? I know that without consulting in person it's nearly impossible to tell, but any feedback would be appreciated. Thank you again for the video

Do you have deviated septum? Look if 1 nostril is bigger than the other. Dr Rahim Gonstead chiropractor on youtube is known to fix deviated septums at the end of a session for free, rather than doing surgery on the bone, he adjusts the cartilage . Just saying if you have a deviated septum, dont bother on spending 5000 for a nose job/nose surgery when he does them for free (not saying you can just go get a session in general for free and this is not an ad, just some info). I would reccomend looking up eustachian tube stretches on youtube and doing those, there is one I like where you basically take your hands and stretch your jaw out, I will send a link when I find it. This has so many benefits, and their is one where you adjust your ear which does the same thing in the long run.

Levi Douglas " I bet you use it like once a day." Only when responding to comments as foolish as the one you made about congestion being as bad as ENS. You think ENS just means you can't breathe? It also means severe sinus burning, cold painful air in the lungs (because the turbinates no longer warm the air) and more frequent infections because you have lost the mucous that once caught all the irritants we breathe. How disgusting that you would encourage anyone to have surgery just because it went fine for you, knowing that if it goes badly for them, it's no skin off YOUR ass. Selfish.

I'm sorry to hear about your results. I've performed around a thousand reductions without a single case of Empty Nose Syndrome. Although it would be exceedingly unlikely for such a minor reduction to cause such significant issues, it is certainly a potential risk. There are many contributing factors that can affect how someone does after any procedure, and choosing the right candidate and performing the right procedure for that patient is the art of medicine. I sincerely hope that you are able to find some relief.
@mellowmike9692
@mellowmike9692 6 жыл бұрын
The turbinates should not be operated on. The average person has absolutely no idea how important turbinates are to breathing and that is why they allow the operation. Out of the thousand people you operated on, I can't help but wonder how many of them could have been helped with other methods since many factors can irritate the turbinates, such as stress and diet. It's easy to blame patients for wanting a quick fix, but it is also the fault of doctors for heading straight for the scalpel instead of looking for the root cause of the problem.

@@gabshewitt1 Damn, see this is what worries me. I assume polyps are ok to remove without ENS? Just not the actual turbinates?

Sorry I did not mean to scare I just want people to know the risk. I had no symptoms until about 3 months after surgery where they have progressively got worse. Let me know how you feel in the future please

How can you tell if the constant stuffiness is being caused by the turbinates or a deviated septum? My nose is super stuffy all the time and gets a little better only temporarily if I’m in a humid place or use some nasal spray, but the actual nostril that feels stuffy varies... so I’m guessing it wouldn’t be a deviated septum?
