Hi dear sir ! I have got a problem with a point in the mechanism of hematopoietic stem cells transplantation (graft- versus -tumor effect), you said that mature T- cells and NK- cells present in the transplanted bone marrow recognize the alloantigens on the surface of tumor cells and destroy tumor cells. But my question is!!! Does bone marrow contains mature T-cells and mature NK-cells? I have learned that bone marrow contains the stem cells and progenitor cells or precursor cells. Please answer me dear sir!

Thank you for your question. Peripheral blood is more commonly used as the source of stem cells in the treatment of hematological malignancy due to the potential for a more potent Graft vs Leukemia effect and the avoidance of general anesthetic risks for the donor from bone marrow harvest. In contrast, bone marrow harvest is generally preferred for the treatment of non-malignant conditions due to the reduced incidence of GvHD. In order to collect Hematopoietic stem cell from peripheral blood recombinant human granulocyte colony stimulating factor (G-CSF) is often used to make the stem cells come out of bone marrow. Then the stem cells are harvested by apheresis. Such bone marrow inoculum also contain donor T cells from donor peripheral blood. That is the source of donor T cells. Deliberate induction of graft-versus leukemia effect by infusion of allogeneic T cells can be used to treat chronic myeloid leukemia that has relapsed after HSC transplantation. So the source is peripheral blood of the donor from which the stem cells are harvested by using colony stimulating factor.
