I recently read many comments of people saying that this episode should be evidence that Merlex could not happen in 'real' life (where this refers to the series, of course!): indeed the end of that episode showed that Der and Cristina were meant to be in Mer's life, no matter what could happen in her life. However, I find that it is more realistic to say that Alex and Meredith might end up together because life brought them there: because bad things happen in life and change you and make you a new person and you could end up loving someone you hated before many things happened to both of you. So, yeah, we could have a wrong 'Merlex' if Weber chose Mer's mum instead of his wife and if mer's mum did not die of Alzheimer. Mer would have been different and Alex too. But life went differently and so they are now different people from those pairing in this episode. So Merlex could in fact be the 'right' pairing in 'real' life.
