How we find that h12V2 is a dependent source element in equivalent circuit ??

Because V2 is the voltage on the output side. On the input side, there could be only two parameters V1 and I1. It shows that it has a dependency on the output side. Since we are applying KVL, the unit of the source has to be a voltage source, that's why it is a voltage-controlled voltage source. I hope it will lear your doubt.
