Hand​ Examination Clinical Skills OSCE - Medical School Revision - Dr Gill
Clinical examination of the hand can be quite challenging, as it is more engaged than many of the other orthopaedics examinations. We'll help with your medical school revision through a demonstration of a hands examination OSCE.
As with any orthopaedic assessment, the Hand​ Examination Clinical Skills OSCE examination will be expecting you to approach with the standard Look, Feel, Move approach which you'll be already familiar with
People often find the Clinical Skills OSCE to be daunting and the Hand examination even more so, as it is quite an engaged OSCE station. However, as with everything at medical school, repetition is going to be key in understanding how you are assessing the patient, positioning your hands, and manipulating the patient.
Please note the hand examination also includes the special tests Tinel's Test and Phalen's Test to examine for carpal tunnel syndrome, which can be worsened in pregnancy, osteoarthritis and rheumatoid arthritis
During the Hand examination, the wrist is also assessed, as you should always examine the joint above and the below the joint are focusing your initial assessment on

Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However during OSCE assessments. Different medical schools, nursing colleges, and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognized standard textbook for clinical skills.

DingusTheGenius DIDNT take it as a slight :) one of my friends said about 6 months ago - “have you seen the response to the KZbin vids” Then another friend managed to convince me to do the audio book But I’m not “doing” anything different. I wouldn’t want to be disingenuous in that way I’m me, and can’t really do anything other than that. If I’ve got people learning from the videos - great. If people are reassure what is going to happen at their doctors - terrific If people relax when listen then all the better 😊

I have a somewhat random medically-related question: when your reflexes are tested, is your leg or elbow supposed to automatically react? Because I have always had to think about it and said to myself, I’d better react, so that’s when I do. But nothing happens automatically except a slightly weird feeling in the spot that’s being tested.

I'm only 24 and my hand has begun to swell and has become painful/unusable (I've seen several doctors and a hand surgeon) and I'm beginning to wonder why I was never given a hand exam anything like this?

This is the “standard approach” to hand examinations. HOWEVER that doesn’t mean that each element is done in every patient in daily practice. It’s a little bit like driving. Some things happen automatically. Here we’re doing everything very overtly to allow students to observe

Thank you for the reply! (I saw five different doctors and none of them really tested my hand at all (just poked it where it hurts/sticks out and asked for X-rays). I think maybe they just didn't care to test it... To be fair, I couldn't really move it much anyway.)

@@shannonpolice9365 I saw a hand surgeon and he said that he thinks I have arthritis,--though he did not elucidate much about which kind or what I should do, or even mention much about the degenerative nature of it. ¯\_(ツ)_/¯

It doesn't look like he's searching for anything in particular, so under what circumstances would someone find themselves needing to undergo this particular examination?

After a recovery, perhaps? It may be standard practice to make sure nothing healed incorrectly or has resumed hurting after a break or surgery.

It would generally be done after surgery or some other kind of recovery, as well as with patients who suffer from diseases which may affect the hands. It could also be done if other issues within the arms or shoulders are detected to ensure that they aren't compromising the hands. In actuality, most doctors will do a shortened version of this exam looking at certain things they suspect might be issues but for the sake of example, he has done a full one.

Dr James Gill well, your voice helps me get 8 hours of sleep, no matter what. Your voice is like having a glass of warm milk, and after doing about twenty minutes of stretches before going to bed, I usually have no problems sleeping until morning. Just like a baby, I wake up without any fatigue or stress in the morning.
