Okay, to whoever is curious on how you can have Farid and Harper alive, I believe you just need to complete the game and just replay the mission (not rewind the story). May only work on PC since I just replay the mission some time after completing the campaign/story and not rewinding the story, having all characters alive in the mission (Karma, Farid, Defalco, Harper).

Kravchenko, Hudson, DeFalco, and Farid die regardless of the decisions the player makes. On the other hand, the deaths of Menéndez and Mason are canonical. Woods is canonically alive On the other hand, it is not known after: Briggs, Karma, Harper and Salazar, etc.

You can actully save karma and harper in one ending. just fail to secure karma so she will lives but not recapturd and do not kill harper. by killing menendez you can have woods and mason and karma and harper all of them alive.

BO2 is so frustrating, due to the multiple outcomes. It took so much work to get the good ending, after I realized certain choices were important and more so than I ever realized until I finished the campaign initially. I always play campaigns on Normal whenever possible, so I can enjoy the story before replaying on Veteran. I couldn't find a way to get the higher difficulty achievements without completely replaying the campaign and making sure I made the right choices was really not an appealing prospect. So BO2 is one of the only 360 CoD from CoD4 to Ghosts that I didn't officially get the Veteran achievement for.

In the mission “Achilles Veil” the one where you had to either “kill” Harper or Menendez, if you kill Harper, Farid will be alive until the next mission where he dies either by DeFalco, or by Salazar. Hope this info helped you remember ;)
