When Sakuna says “back then” he’s likely just referring to when Kenjaku *ahem* “created” the cursed death womb paintings. Which makes sense and lines up with the line “Kenjaku does the grossest things.” since the guy took back shots in order to give birth to Yuji and his siblings. Gege has also come out and stated that Sakuna fostered no family. Now, while it’s not impossible that Sakuna had a sibling that later had kids it does stand to reason that GeGe is telling us that he doesn’t plan to take the “Yuji and Sakuna are relatives” route with the story.
